Dear Mr. Wien:
Your waiver req%les~ for the above mentioned parcel has been
approved. The required 100 foot separation from a residential
well to a private septic system has been waived to 95 feet for
both wells on lot 12 and Lot 1 Block 6 Greenland Subdivision.
This waiver applies to this upgrade of the septic system on Lot
12. Any future upgrade to the septic system may require
another waiver approval from D.H.H.S.

The above referenced lot consists of a 5 bedroom single family house served with
a private well and onsite sewer system. The owner is applying for a permit for
a full upgrade of the absorption area. On 3/11/87 to 3/12/87, a test hole was
dug and drilled to 27.5 feet. Groundwater was encountered at 2~ feet and was at
21 feet on 3/19/87. We recommend a deep trench 17 feet total depth with 11 feet
of gravel. The length will be approximately 60 feet and so~ls will be verified
during the first inspection. Referring to the site plan diagram, this upgrade
would be located under the existing driveway. ~ere will be 6 feet of soil
cover over the trench and we also recommend insulation be used° The upgrade
trench also encroaches on the subject well and tile neighbors well to the north.
We request that you waive the requ~.red separation distance from these wells to
95 feet each. Attached is authorization from the owner of the neighbors well.
The septic tank is 1500 gallon capacity. The cleanout pipe is 99 feet from the
well. Tile tank was installed approximately 1964 and the required separation
distance then was 50 feet. With tile type of soils encountered, we feel there
will he adequate filtering of the effluent. The area for the upgrade is on
level ground. The general slope o~ the lot is approximately 3 to 5 percent from
the south to north.

Dear Steve:
This is in response to your request for additional {nformstion to evaluate the
waiver request for the above referenced lot.
On 4-22-87, ti~e water level in the monitor tube was at 21,5 feet below t~e
surface. A waiver of 5% of tl~e required separation distance is requested.
AttacHed are sample well logs of the area showi,qg silty sand and clay to be
present from 25 feet to 75 feet. Also attached are sieve analysis from three
different depths in the test hole. All show MI.I ~rom 6 foot to 26 feet. 'fnis ~s
a fine sandy and silty soil. The percolation rate is 25 minntes per inch.
On 4-22-~7, the subject well was probed and found total depth and casing depth
to be 83 feet. Pump setting is 75 feet and static water level is 52.6 feet.
Water sample results are satisfactory.
There is eo other location on the lot that the upgrade could maintain as much as
