Ref: Septic Design for Lyndon O. Sikes Homestead
To whom it may concern:
The existing 3 bedroom house is served by a private well and a cesspool type septic system. The
existing septic system is in a state of failure and must be upgraded. Comments regarding the
proposed design are summarized as follows:
1. SOILS: Attached is the soil log which shows the soil profile, and the percolation test results.
The soils below the organic layer are an SM/ML to a depth of 2 feet. From 2 feet to 4 was a sand
and gravel mixture. The soils between 4 and 7 feet were an SM/ML material followed by a
SM/GM material to the bottom of the test hole (18 feet). No groundwater was encountered
during the excavation of the test hole. Two perk tests were performed in the test hole at depths
of 1.0 feet and 7.5 feet. The soils at 1.0 feet perked at a rate of 13.3 min./inch and the soils at 7.5
feet perked at < 1 min./inch.
2. TRENCH DESIGN:
a. Percolation Rate: <1, 13.3 minutes/inch
b. Allowable Application Rate: 0.8 gallons/day/ft2
c. Number of Bedrooms: 3
d. Design Flow: 450 gallons per day
e. Minimum Absorption Area: 563 ft2
f. Total Depth: 11 feet
g. Effective Depth: 8 feet
h. Width: 2.5 feet
i. Reduction Factor: N/A
i. Length: 40feet
j Effective absorption area = 640 ft2
3. SURFACE WATERS: There are no surface waters within 100 feet of the proposed
upgrade.
4. TOPOGRAPHY: The area where the proposed trench is to go slopes at approximately 10%
or less from southeast to northwest. Approximately 55 feet north of the test hole is a cutbank.
We are requesting a 35 foot waiver to this cutbank. The area between the proposed trench and
the cutbank is well vegetated and it is our opinion that there is minimal risk associated with
granting the 35' separation waiver.
I am unaware of any adverse impacts this installation would have on adjacent wells or septic

Nitrate Advisory
Certificate of On-Site Systems Approval # OSC 171566
A Certificate of On-Site Systems Approval inspection and test of potable
water was recently conducted on the well water supply on T12N R3W Sec
25 NE4NW4NE4NW4. This inspection revealed a nitrate concentration of
6.58 milligrams per liter (mg/L) was reported for the property's well water
sample. The Environmental Protection Agency (EPA) has established a
maximum contaminant level (MCL) of 10.0 mg/L for public drinking water
systems. While private wells are not subject to this regulation, EPA
standards are based on existing health information and can therefore be used
to gauge the relative quality of water from private wells. Please see the
attached "Nitrate Fact Sheet" for important information regarding nitrate.
This advisory must be attached to all copies of the subject Certificate of On-
Site Systems Approval.

In regards to 7900 Upper Huffman in Anchorage Alaska. We were contacted to conduct a camera inspection due to a
high nitrate reading on the well. Our inspection showed that the well had a shallow casing down to 31' due to shallow
bedrock. Water production was found at edge of casing.Total depth was greater than 200'. Due to these findings it was
determined that the well would need to be sleeved and potential water sources would be cut off to prevent cross
contamination.
In an attempt to prevent too much water loss samples were taken from the end of the casing and from the lower area to
determine what area was causing the introduction of the high nitrates and guide us to sleeve that and only that area off.
When results came back we found a much lower result the upper section yielded 3.45 mg/L and the lower yielded 5.23
mg/L.And did not require any action from the M.O.A. at this time other than potential monitoring in the future and an
advisory of elevated but not exceeding levels.
