Septic System Upgrade Permit Request
This is a design narrative for a permit to install a 1250-gallon S.T.E.P. tank to replace an existing 1000-gallon septic
tank and 500-gallon lift station to be issued for this property. The existing tank is 47 years old, and the existing lift
station is 35 years old. Both tanks are likely perforated and leaking and will be removed per MOA code. Currently
the lot is developed. The proposed replacement will be connected to the existing drain fields. This lot and the
surrounding lots are served by a community well. There are currently no wells within 200’ of this upgrade.
1. Upgrade Tank Design.
A foundation clean out installed if needed.
The tank will be located: 5’+ from any property line.
5’+ from any deck/stair support.
10’+ from building foundation.
10’+ from any water line.
100’+ from any surface water.
100’+ from any private wells.
200’+ from any public wells.
The proposed installation will not affect the future development of this or the surrounding lots.

As certified by my seal affixed hereto and as of the validation date shown below, I verify that my investigation, based on
procedures outlined in the Certificate of On-Site Systems Approval Guidelines, indicates that the on-site water supply
and/or wastewater disposal system appears to comply with applicable Municipal and State codes, ordinances,
and regulations in effect at the time of installation, unless noted otherwise.
In conducting an adequacy test, I attempt to provide a thorough, conscientious engineering analysis of the system in
accordance with ADEC & MOA guidelines and regulations. The reported results describe the performance of the system
under the conditions encountered at the time of the test, and separation distances measured to readily identifiable
features. The operational life of all wells and septic systems depend on the local soil condition, ground water levels that
may fluctuate during the year, and the water usage of the family being served by the system. These conditions are outside
the control of the evaluator of this system. All systems eventually fail, and satisfactory test results do not guarantee future
performance of the system, nor do they guarantee that there are no hidden defects or encroachments. Therefore, we
cannot provide any warranty for future performance, nor can we estimate remaining life of the system. The content of this
report is for the sole benefit of the owner listed above.

On December 10, 1999, we inspected and tested the septic system located on Lot 38A, Block 3,
Valli-Vue Estates Subdivision No. 2. The system was originally constructed in early 1972 and
consisted of a 1,000 gallon septic tank and a deep absorption trench. In the summer of 1988 the
trench apparently failed and a new trench was constructed on the property. A 500 gallon lift station
was also installed to transfer the septic effluent from the tank to the new absorption trench. A
diversion valve was also installed after the septic tank and prior to the lift station to allow use of the
failed system should it rejuvenate itself.
At the time of our inspection the absorption trench was dry as the house had apparently been
vacant for some time. We therefore injected over 1,200 gallons of water into the trench which
brought the water level to 48". The next day the water level had receded to 18". We then injected
450 gallons of water raising the elevation to 34". The next day the level was again at 18"
indicating the trench was capable of absorbing 450 gallons of water in a 24 hour period. It
therefore meets the minimum requirements of the Municipality necessary for certification.
Attached is a copy of the as-built of the septic system completed in 1988 for your information.
Please note the original trench on the property was not inspected or tested. We are unsure of its
current status. The septic tank was pumped on December 19, by Old McDonald's Pumping
Service. Please let me know if you have any questions.
