If this is an on-site wastewater system and a licensed Professional Engineer has inspected the installation, the original as-built inspection report must be sent to this office for review, approval and documentation. All inspection reports must be submitted within 30 days of construction completion. A new permit must be obtained from this office for a well and/or on-site wastewater system NOT installed by the expiration date. However a new permit can be issued free of charge for a second year if the application for the renewal is received on or before the date of expiration of the original permit for which a fee was paid. GENERAL: The proposed 5 bedroom home w'fll be served by a private septic system and a private well. Excavation of several test holes and the excavation of the proposed location of the foundation has confirmed the presence of shallow bedrock which limits the total depth of a drainfield. Given tlxis site constraint, we are proposing to install the Recirculating Upflow Filter (RUF) system. Three test holes were excavated and the soils are summarized as follows: 2. SOIL CONDITIONS: Attached are logs which shows the soil profile, and the percolation test results. The soils below the organic layer are a ML/SM material to a depth of 5 feet in TH#1 and TH#2 (bottom of test holes); and to 2 feet in TH#3. In TH#3, the soils below the ML/SM layer transitions to a SM material to a depth of 4.5 feet (bottom of the testhole). No groundwater was encountered during the excavation of the test holes. The percolation tests were performed between the depth 0f 1.5 feet to 2.0 feet in TH#1 and at 2.0 feet to 2.5 feet in TH#2 & TH#3. The percolation rate was 4.6 minute/inch in TH#l, 15 minute/inch in TH#2, and 7.5 minute/inch in TH#3. 3. DRAINFIELD: The intent is to install a Recirculating Upflow Filter (RUF) system that will allow the use ora s/mall dralnfield in the area around a 30 foot radius of TH #1. The size of the drainfield will be based upon the previously established criteria for the RUF systems, which dictates that soils percolating between 1 & 30 minutes/inch have an allowable application rate of 4 gpd/ft2, and soils percolating between 30 & 60 minutes/inch have an allo~wable application rate of 2 gpd/ft2. Given the perk rates of 4.6 to 15 minutes/inch the 4 gpd/ftz application rate would apply. We are proposing to install two beds that are 2 feet deep maximum (on uphill side) by 10 feet wide by 20 feet long each. Two feet of "Winter Road Sand" (Central Paving Product) is to be added and both beds will have an effective depth of 0.5 feet. The intent of installing two beds is to alternate the flow between both systems. This corresponds to an absorption area of 200 ft2 for each bed, or an application rate of 3.75 gpd/ft2 (assuming 750 gpd total flow). 6. 0RENCO PACKAGE SYSTEM: The STEP tank with the trickling and upllow filter will be'manufactured by Anchorage Tank & Welding to meet the latest design criteria established by Orenco Systems, Inc. The point of contact at Anchorage Tank is Lowell MeNutt. As with their standard STEP tank, it is equipped with a high water alarm per M.O.A requirements. 5. SURFACE WATER: There are no surface waters within 100 feet of the proposed septic system. 6. TOPOGRAPItY: As can be seen on the design (page 2 of 5), the average slope near the proposed septic area on this property is a <10% slope in a direction of approximately southeast to northwest. The proposed beds are t° be installed parallel to the slope contour. 7. CLOSING: I am open to any suggestions from your department that would be an improvement to the proposed system. Test holes were excavated and percolation tests performed. The approximate location of the test holes are located on the attached site plan. At the time of excavation no water w~s~ encountered in test hole %1 and test hole %3 ;and water was encountered at 9.5 feet in test hole %2. After seven day ground water monitoring, all monitoring tubes were found to be dry. This property has enough area for a future septic upgrade which can be seen on the attached site plan. We do not anticipate any adverse effects on neighboring wells, septic systems or drainage patterns by the installation of the proposed septic system. The scope of this project includes the installation of a 1500 gallon septic tank and a five foot wide drainfield to serve the five bedroom residence located on the referenced property. Construction shall be in accordance with the approved site plan and design drawings, Municipal permit with any special provisions or conditions, and all applicable State and Municipal Wastewater Disposal Regulations. The contractor shall be responsible for obtaining any necessary underground utility locates. 4. Unless specifically agreed otherwise, the property owner shall be responsible for final grading areas subsequently depressed from soil settling. 5. Contractors installing wastewater disposal systems must be certified by the Municipal Health Department for system installations. Owners installing their own systems must also receive prior approval from the Municipal Health Department. SEPTIC TANK INSTALLATION: A septic tank is to be constructed by a certified septic tank manufacturer. Construction shall include two 4" cleanouts for pumping access. The septic tank shall be sufficiently bedded to prevent settling or shifting of the tank. A foundation cleanout shall be installed one to four feet from the building foundation. In the line between the tank and the leachfield there shall be two adjacent cleanouts (unless an effluent pumping system exists within the septic tank). These cleanouts shall be located on undisturbed soil not more than 10 ft. from the tank. The first cleanout, in line, shall be to clean toward the leachfield. The second cleanout shall be to clean toward the septic tank. Final grading over the septic tank shall be such that a positive slope exists away from the septic tank. ABSORPTION TRENCH/DRAINFIELD INSTALLATION: Excavate the proposed trench to the dimensions shown on the design. The bottom of the excavation shall be within 2 inches of level. If the sidewalls of the excavation become smeared, they must be raked or scratched (ruffed-up) before gravel (sewer rock) placement. 0 Once the gravel is installed, the distribution pipe is to be installed level with the perforations faced downward. Gravel is then to be placed over the distribution pipe to provide a minimum of 2 inches of cover over the pipe. 3e A silt barrier must be installed between the final gravel layer and the native soil backfill. Ensure the silt barrier covers the entire gravel surface before placing backfill. e Monitor tubes shall be of four (4) inch diameter, installed approximately in the locations shown on the design, and extend a minimum of 12 inches above final grade. The portion of the monitoring tube extending through the gravel shall be perforated from the bottom of the trench to the invert of the distribution pipe. This is equivalent to the effective depth of the gravel as noted on the design. Insulation must be installed when the backfill depth is less than thirty-six (36) inches. The finish grade over the trench must be mounded to prevent the formation of a depression after settling. Se A permeable nontoxic silt barrier (Typar 3401, Mirafi 140N, or equal) must be installed between the final leachfield gravel layer and the native soil backfill. All leachfield gravel (sewer rock) shall be 0.5"-2.5" screened gravel with less than 3% passing the ~200 sieve. When sand is being used as a filter material, its gradation specifications must conform to current M.O.A. or D.E.C. requirements. Page Four Lot 8, Block 2, Prospect Heights S/D #4 June 6, 1996 INSPECTIONS: Typically there will be a minimum of three (3) inspections required during the installation of the wastewater disposal system. These inspections will occur as follows: The first inspection must be conducted after the excavation of ditches, pits, trenches, or beds and before the installation of any gravel. A septic tank may be set in place, but may not be backfilled before this inspection. The second inspection must be conducted after the placement of the silt barrier, gravel, distribution lines, standpipes, cleanouts, and insulation, but before the placement of any other backfill. The final inspection is to occur upon final grading of the property. Often there will be more than these 3 inspections required. Especially with the installation of multiple trenches, sand filters, pressurized distribution systems, etc. Thus, the inspecting engineer is to be contacted at least 24 hours prior to the start of construction. If necessary, a pre-construction meeting will take place on-site. The inspecting engineer will not coordinate, direct or control in any way the contractors activities. The owner shall contract with the contractor to perform the work outlined in these specifications and plans and in accordance with the attached M.O.A. permit. There will be no contractual arrangement existing between the contractor and S & S Engineering. S & S Engineering shall be the owner's representative and will inspect the work as stated above to document the contractors activities. Final acceptance of the contractors work rests with the owner and the M.O.A.
