If this is a well permit and you have dr/lled the well, a well log must be sent to this office for documentation of the installation and to close the permit. If this is an on-site wastewater system and a licensed Professional Engineer has inspected the installation, the original as-built inspection report must be sent to this office for review, approval and documentation. All inspection reports must be submitted within 30 days of construction completion. A new permit must be obtained from this office for a well and/or on-site wastewater system NOT installed by the expiration date. However a new permit can be issued free of charge for a second year if the application for the renewal is received on or before the date of expiration of the original permit for which a fee was paid. When applying for a new permit after the original permit has expired or for more than a second year, the fees are: $320.00 for an on-site wastewater permit and $120.00 for a well permit. Box 196650 Anchorage, Alaska 99519-6650 Reference: Proposed Well Location and Septic System for Township 12N, Range 3W, Section 22, Lot 46 To whom it may concern: The reference property is currently undeveloped. The owner wishes to build a 4 bedroom house with and on-site septic system and a private well. Two test holes were excavated on the property and the soils m'e summarized as follows: 1. SOILS: Test holes #1 (north) and #2 (south) were excavated on July 29, 1999 and no groundwater was encountered in either hole. Test hole #1 had approximately 1.5 to 2.0 feet of organics at the surface of the test hole followed by a SM material to a depth of 15.5' (bottom of test hole). One perk test was performed in test hole #1 at a depth of 5.75 to 6.25 feet. The soils perked at a rate of 3.1 min./inch. Test hole #2 was excavated to a depth of 14.5 feet and the soils below the organics were found to be a SM/IVlL material to the bottom of the test hole. Again, one perk test was run at a depth of 5.75 to 6.25 feet. The perk hole absorbed water at a rate of 2.0 min./inch. It is our opinion that a 0.8 application rate should be used due to the visual appearance of the soils. 2. TRENCH DESIGN: a. Percolation Rate: 2.0, 3.1 minutes/inch b. Allowable Application Rate: 0.8 gallons/day/ft2 (visual rating) c. Number of Bedrooms: 4 d. Design Flow: 600 gallons per day e. Minimum Absorption Area: 750 ft2 f. Total Depth: 8 feet g. Effective Depth: 6 feet h. Width: 2.0 feet i. Reduction Factor: N/A j. Length: 70feet k Effective absorption area = 840 ft2 3. SURFACE WATERS: There are no surface waters within 100 feet of the proposed upgrade. 4. TOPOGRAPHY-' The area where the proposed trench is to go gradually slopes from east to west at approximately 15% (see attached topography drawing). In short, there are no slope concerns.
