All inspection reports must be submitted within 30 days of construction completion. A ne~v permit must be obtained fi.om this office for a well and/or on-site wastewater system NOT installed by the expiration date. However a new permit can be issued free of charge for a second year if the application for the renewal is received on or before the date of expiration of the original permit for which a fee was paid. Box 196650 Anchorage, Alaska 99519-6650 Subject: Gentlemen: Upgrade Sewer Permit - Lot 191, SE1/4, Sec 8, T15N, R1W At the request of the owners, on August 27, 1999, we excavated one testhole for the subject property to upgrade the existing system. The existing system is still functioning but the owner desires to construct a new house and does not anticipate that the system will meet his future needs due to elevation differences. We installed a monitoring tube and monitored water in the testhole. The results of this test and water monitoring is attached. We propose to install a 5' wide shallow trench. The testhole registered no water during excavation and water was found at 10.5' from the existing ground in the monitoring tube after monitoring on September 4th. The existing tank will be abandoned in place. Additional fill will be provided over the system as required. This lot is relatively fiat and slopes to the south to north away from the existing field and the house at approximately 2- 5%. There are presently one well on this property. There are no other public or private wells within 100' of our proposed system location except as noted. There is neither surface water within 100' nor any known curtain drains within 50'.
