The proposed installation will serve an existing four (4) bedroom house. This lot and surrounding lots are served by AWWU drinking water. Soils: A test hole was performed on this lot by Pannone Engineering Services, LLC on 8/16/2024. Groundwater was observed during the excavation of the test hole at a depth of 16.0’ BGS, bedrock was not encountered. Groundwater was not observed in the test hole monitor tube after 7-days. Based on the results of the percolation test and overall soils appearance an application rate of 1.2 gpd/SF was used for the design of a conventional wastewater treatment system in the area of the test hole. 1. Soil Absorption System Design: See Sheet 1 of the design package. 2. Surface Water: There is no surface water within 100 feet of the proposed septic tank and drain field. 3. Topography: See attached site plan for area topography. There are slopes exceeding 25% within 50ft of the proposed drain field. The proposed drain field design conforms to the steep slope provision of AMC 15.65 4. Drawing Markings: The Drawings are marked “For MOA Review Only”. When written notification that the review is complete and no further comments are received from MoA On-Site Department, the note will be removed and “For Construction” drawings will be issued. The existing 1250 gallon tank was installed on August of 1973, made of steel and is almost 26 years old. During our site visit, there was a slight depression over the tank. In 1995 when we worked on this property, a depression was also observed. It is visually obvious that additional fill has been added over the tank since 1995. I am uncertain whether this is the result of the tank collapsing due to failure of the top or just soil settlement. Health & Human Services Ref: Health Authority Approval (HAA) Application for Lot 2, Heritage Heights Subdivision. To whom it may concern: Attached is the HAA application for the subject lot. An adequacy test was run on the septic system by introducing 7.0 GPM, for a total of 100 minutes, into the clean-out on the seepage pit. The total volume introduced was 700 gallons, and the liquid level in the "pit" rose a total of 01—inches. The recovery of the system was monitored for 30 minutes after the water was shut off, during which time the system recovered 100%. In short, the water level in the seepage pit dropped 11 inches in less than 30 minutes. Consequently, the system is adequate for a three bedroom house (450 GPD). The separation distance from the septic system to adjacent wells was only verified to be greater than 100 feet. Lots .1.,2,3,16,23,& 24, Heritage Heights Subdivision are on a community water system; therefore the separation distance did not apply to these lots. Lot 6, Crest Wood Subdivision is an undeveloped lot. Lots 7, & 8 , Crest Wood Subdivision are the only adjacent lots which have private wells. There are no records on file at DHHS for the septic systems or wells on either of these lots. I have attached a drawing which shows a 110 foot radius from my client's seepage pit clean-out. Only the northern most portion of these lots falls within this 110 foot radius. During my site visit, I did not observe any well casings within the subject area; however, the ground was covered with snow. I was able to contact the homeowners, by telephone, and verify the general location of their wells. It appears that neither well is within 100 feet of my client's septic system. The existing septic tank was installed in August of 1973, making it approximately 18.5 years old. During my site Telephone - Fax 338-3246 0 8471 Brookridge Drive 0 Anchorage, Alaska 99504 visit, the septic tank was not exposed and visually inspected for structural integrity. There was not any noticeable depression over the septic tank, indicating that it was structurally failed at this time, however, because of its age, it is reasonable to assume that it is reaching the end of its useful life. Consequently, the homeowner should anticipate that it will be necessary to replace it in the not to distant future (within the next five years). The separation distance from my client's seepage pit to absorption fields on adjacent lots was not measured since the closest absorption field is located on the western portion of lot 1, Heritage Heights, and is perhaps 100 feet away. The distance to the closest property line is 23 feet, which is greater than twice the depth of the seepage pit. In short, this separation distance is not a concern. Attached is a copy of the as -built survey prepared by "Lantech". It can be seen that there is a 30 foot slope construction and maintenance easement (in reality this is terraced slope down to Yukon Street) along the north property line. The separation distance from the seepage pit to the top of this slope (>25%) is only about 25 feet. Apparently, this installation was acceptable to the DHHS when it was inspected on 8/20/73.
