REFERENCE: Swanson Subdivision, Lot 2: Water Storage upgrade within the
Municipality of Anchorage (MOA).
On 2/12/2003, a site visit was performed on the referenced property in order to perform a
well flow test and septic adequacy test. Prior to the test, a total of 1200 gallons of storage
tanks that are N.F.S. approved were found in the house (6-200 gallon tanks). The property
has 2 wells serving the house. The wells are connected together outside of the foundation
and were tested simultaneously. The wells were then tested and found to have a combined
production of 0.09 gallons a minute which does not meet the M.O.A. standards. The
homeowner was then notified that an additional 300+ gallons of storage were needed in
order for it to meet M.O.A. requirements and to obtain a Health Authority Approval. The
homeowner contacted K&L plumbing to install an additional 300+ gallons of storage.
Neither the homeowner or the certified plumber were unaware that a permit was required.
Additional holding tanks were installed by K&L plumbing and are N.F.S. approved
tanks. There now is 1500+ gallons of storage inside the house which meets M.O.A.
requirements for a 5 bedroom water storage system. We request you issue a water storage
permit for the existing water storage system.

Ref: Septic Upgrade Design for Lot 2, Swanson Subdivision
To whom it may concern:
The existing 5 bedroom house is served by a private septic system and a well. The existing septic
system is surcharged and must be upgraded. Two test holes were excavated on the property.
Comments regarding the proposed upgrade design are summarized as follows:
1. SOILS: Attached are logs which shows the soil profile, and the percolation test results. In
both test holes, the soils below the organic layers are a GW/SW material (with silt) to a depth of
13 feet in TH#1 and to 10.5 feet in TH#2 (bottom of TH#2). There is a clay layer at the bottom
of TH#1 between 13 & 13.5 feet (bottom of TH#1). No groundwater was encountered during the
excavation of the test holes. A percolation test for TH#1 was performed between the depth of 4.0
feet to 5.0 feet which had a percolation rate of <1 minute/inch. No percolation test was
performed on TH#2, but was visually rated as the same as TH#1. It is our opinion that due to the
high percentage of sand, the insitu soils will serve as a sand filter. A grab sample can be provided
if deemed necessary.
2. TRENCH DESIGN:
a. Percolation Rate: <1 minutes/inch
b. Allowable Application Rate: 1.2 gallons/day/ft2
c. Number of Bedrooms: 5
d. Design Flow: 750 gallons per day
e. Minimum Absorption Area: 625 ft2
f. Total Depth: 6 feet (max.)
g. Effective Depth: 4 feet
h. Width: 5 feet minimum
i. Reduction Factor: 0.50
j. Minimum Length: 65 feet long
k Effective absorption area = 650 ft2
3: SURFACE WATERS: There are no surface waters within 100 feet of the proposed
upgrade.
4. TOPOGRAPHY AND WAIVER REQUEST: As can be seen on the design drawing, the
proposed drainfield is to be installed at the base of the 15 to 20 percent slope running from
south/southwest to north/northeast. There is a cutbank to the north of the proposed drainfield and
alternate site. We request that a 35 foot waiver be granted from the cutbank to the primary and
alternate site. The porosity of the soil will insure that wastewater moves downward rather rapidly
and does not migrate laterally and daylight at the cutbank. We are unaware of any adverse effects
the granting of this waiver would have on adjacent properties.

Dear Mr. Swanson:
We are submitting herewith the test boring results and our comments
regarding soil conditions encountered at the subject site. This
investigation was performed in accordance with the request of Mr. Paul
Piper of March 29, 1976 and those procedures outlined in a letter dated
July 15, 1975, by Mr. Rolf Strickland of the Municipality of Anchorage
Department of Environmental Quality.
A total of four test holes were put down within the area of Section 17
for the purpose of defining general subsurface soil conditions for the
proposed sanitary system. Excavation was accomplished with an auger
type drilling rig and the test holes were extended to a depth of 15-20
feet below ground surface. The final log prepared for the test hole
has been included in Drawing.A-01.
Ground water was encountered in Test Hole No. 4 only at a depth of 13.0
feet.

The Municipality of Anchorage Development Services Department (DSD) Issues Certificates of Health Authority
Approval (HAA) based only upon the representations given in paragraph 4 by an independent professional civil
engineer registered in the State of Alaska. Certificates of Health Authority Approval are required for the transfer
of title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or
water supply system. DSD also issues HAAs upon request to homeowners. Certificates of Health Authority
Approval are valid for 90 days from the date of issue for properties served by a private or Class C well and may
be reissued with new water samples. (Certificates may be reissued for a period of up to one year with valid
water samples.) Certificates are valid for one year for properties served by Class A or B wells or a public water
system. The Municipality of Anchorage is not responsible for errors or omissions in the professional engineer's
work.

In conducting this evaluation, AKWWC, Inc. attempted to provide a thorough,
conscientious engineering analysis of the system in accordance with ADEC and MOA
DSD Guidelines & Regulations. The reported results described the performance of the
system under the conditions encountered at the time of the test, and separation
distances measured to readily identifiable features. The operational life of all wells and
septic systems depend on the local soils condition,. groundwater levels that may
fluctuate during the year, and the water usage of the family being served by the system.
These conditions are outside the control of the evaluator of the system. Satisfactory test
results do not guarantee future performance of the system, nor do they guarantee that
there are no hidden defects or encroachments. AKWWC, Inc. can therefore not provide
any warranty or future estimate of how long the system will continue to meet the
operational requirements of the ADEC or MOA DSD. The content of this report is for
the sole benefit of the owner listed above. Any reliance upon or use of this report by any
other person or party is not authorized, nor will it confer any legal right whatsoever.

REFERENCE: Well Flow Test for Lot 2; Swanson Subdivision.
To Whom It May Concern:
On 2/12/2003, a well adequacy test was performed on the wells for the referenced property. Two
wells were found on the property and it appears that they are connected together outside the
foundation. Inside the structure we found, two pump protectors and a total of 1200 gallons of
water storage tanks (6 — 200 gallon tanks). There was no bypass valve or hose bib prior to the
holding tanks, so there was no way to meter the water directly from the wells. It was decided that
the only way to perform the flow test was by taking liquid level measurements in the holding
tank. In order to perform the test, all the storage in the casing and some of the storage in the tanks
had to be pump out. Once this was completed, liquid level measurements were taken and the
wells were allowed to run with the pump protectors shutting the wells off and on. The wells were
ran for 223 minutes and than the liquid levels in the tanks were checked again and found the
water level in the holding tanks had rose 1 inch in each tank. This calculates to 19.6 gallons of
production in 223 minutes and based upon this data, the well production is 0.09 gallons/minute
or 129 gallons/day. Per M.O.A. requirements, the minimum production for a five bedroom
residence is 0.52 gallons/minute. According to the M.O.A. water well ordinance, wells that
produce less than 150 gallons per day shall have a minimum of 1500 gallons of storage for a five
bedroom residence. The residence previously had a total of 1200 gallons of storage. An
additional 300 gallons of storage was added, making the total storage volume 1500 gallons.
