I believe the following will adequately address each. Note what "Other" means under Absorption Field section of the inspection report. This Absorption Field is a 5 -wide field with a depth of 12 -inches. The note "3 -wide" has been added to the Inspection Report. 2. Confirm 500 gallon capacity entered under Lift Station section of Inspection Report form. This should be referring to the pump vault add-on. The tank used for this installation is a 1500 gallon Fiberglass Tank (2 compartment) as noted on the Inspection Report form. The submittal form for the tank is enclosed and clearly shows the tank not designed with an integral pump vault. The submittal data for the 500 gallon pump vault is also included to document the 500 gallon capacity of the pump vault add-on. 3. Confirm benchmark location/description. Appears it should be SW Dwelling Corner, based on where bM is marked on the site plan. Please note the attached photo. The Bench Mark is, in fact, on the pavement at the SW corner of the garage. The garage is a single car garage attached to the dwelling. The rod can be clearly seen in the photo. 4. Site plan does not include a scale. Please address. Plan view was drawn at a scale of I " = 30'. A note to that effect has been added to the drawing. The subject property is served by a private well and an onsite septic system. An adequacy test performed by our firm on 10/12/17 showed both the septic tank and drainfield to be in a failed condition. The lot is 43,725 sf in size. A survey as-built was conducted by our firm,showing the existing drainfield as well as the onsite well(see attached,Survey As Built,dated 10/19/17). The design narrative is summarized as follows: GENERAL SITE PLAN AND SEPARATIONS: All surrounding properties are served by private wells. Attached are two drawings showing the proximity to surrounding properties wells:"Lot 24 Well Radius" drawing that also shows the subject lot;and,"Septic System Design"drawing that shows the well on Lot 25E to the east of the subject lot(see attached). There are no surface waters within 100'of the proposed septic system. The onsite well on the subject lot is 56 ft from the proposed Advantex Treatment Tank and 76 ft from the nearest edge of the proposed septic drainfield. Proposed waivers are discussed below,under Setbacks/Waivers. SOIL CONDITIONS: On 11/15/17,a test hole was dug on the subject lot,and a percolation test was conducted(see attached,Soils Log-Percolation Test). The test hole was dug within 30 ft of the proposed drainfield site,to a depth of 8.5 ft below grade(see attached,Septic System Design drawing). The subsurface soils were visually rated in the field by our field technician,Mr.Brandon Jones,and consisted of 2.5 ft of overburden/topsoil,underlain by 3.5 ft of sand with gravel,underlain by a dense layer of silt to the bottom of the test hole at 8.5 ft. Groundwater was encountered at 6 ft below grade,likely perched groundwater on top of the dense silt layer. At 8.5 ft,the excavator was not able to dig further and it is believed bedrock was encountered(or else a very large boulder). On 11/22/17,a 7-day groundwater check showed the groundwater level at the same depth,6 ft below grade. A percolation test was conducted between 2.5 ft and 3.5 ft,according to Falling Head Percolation Test Procedures,and the perc rate result was 1.1 min/in.Based on the percolation test results,a soil application rate of 1.2 gpd/ft2 correlates to a perc rate of 1.1 min/in. We are proposing to install an Advantex Treatment Tank with remote monitoring,and with outfall to a gravity fed drainfield with a soil application rate of 6.0 gpd/sf(see attached,Advantex Submittal Data Sheet). The existing septic tank will be pumped out and either removed from the site or crushed in place. Municipality of Anchorage Page 2 of 2 November 27,2017 DRAINFIELD DESIGN: The proposed drainfield will consist of a 5-wide trench type drainfield, 5-wide x 12 in depth x 14 ft in length,based on the following calculations for a 3-bedroom dwelling: (450 gpd/6 gpd/sf/5 ft)x 0.87= 13.05 ft See attached design drawings for the drainfield specifications. The replacement trench will be greater than 10 ft from the nearest edge of the existing trench,and at least l Oft from all lot lines. The existing trench will be abandoned in place. It is believed this trench was installed into the groundwater and has been full of water for quite a few years. TOPOGRAPHY: The general topography of the lot is such that it is level throughout. There are no slopes greater than 25%for this property or within 50 ft of the proposed drainfield site. SETBACKS/WAIVER: The attached design drawing shows the encroachments into the setbacks,all due to space constraints on the lot and the location of the existing well: • The proposed septic drainfield site is 76 ft from the existing well. We are requesting a waiver,and invoking proposed MOA code,AMC 15.65.355(H)(1)(b),that with the installation of a Category III AWWTS with remote monitoring,allows for a reduction in the horizontal setback to a private well from 100 ft to 50 ft. • The proposed Advantex tank will be located 56 ft from the existing well. We are requesting a waiver to the setback between a septic tank and a private well,from 100 ft to 56 ft. Included with this letter is the required fee for waivers. • The bottom of the drainfield will be approximately 2.65 ft above the groundwater level. We are requesting a waiver,and invoking proposed MOA code,AMC 15.65.355(H)(2)(a),that with the installation of a Category III AWWTS,allows for a reduction in the vertical setback for a gravity fed drainfield to be reduced to 2 ft. REPLACEMENT DRAINFIELD SITE: The replacement drainfield site is located in the same area where the original drainfield is located. The replacement field will require excavation and removal of the original field. The septic system will not adversely affect the surrounding properties. In consideration of the mutual covenants contained herein, the parties to this Maintenance and Repair Agreement agree as follows: 1. Advanced Wastewater Treatment Systems. Municipality grants permission to Owner to utilize and operate an Advanced Wastewater Treatment System (AWWTS), described as Advantex Wastewater Treatment Tank located at(legal description). T12N R3W Sec. 33 Lt 25 W132' 2. Definitions. Alteration. Any change to the design or function of an AWWTS that includes the installation or removal of any parts,components or pieces not included in the original construction permit and design. Certificate of On-Site Systems Approval(COSA). An approval by the Municipality of existing water and wastewater disposal systems given at the time of property sale and title transfer in accordance with Anchorage Municipal Code (hereinafter,"AMC") 15.65. These approvals certify that the systems are adequate for the homes that they support and meet the codes that were in place at the time of system construction. Damage. Any man-made or natural change in a system that would inhibit the system from performing as designed. Maintenance and Repair. The scheduled and as needed replacement of existing parts, components and pieces of an AWWTS that were included in the original design which would allow the AWWTS to continue to perform as designed. Permit. An On-Site Wastewater Disposal Permit as required by AMC 15.65 to construct and operate an AWWTS. 3. Term. The term of this Maintenance and Repair Agreement shall begin on the date of approval by the Municipality to operate the installed system or issuance of a COSA, and shall continue while the AWWTS is in use or is operational or until the property is sold or title is transferred by the owner and a new COSA is issued to the new owner or transferee of the property. 4. Alterations, Installation and Removal of Additional Equipment. Prior to performing any alterations to an AWWTS,the owner agrees to obtain an On-site Wastewater Disposal Permit from the Municipality in accordance with AMC 15.65. 5. Maintenance and Repairs. A. Throughout the term of this Maintenance and Repair Agreement,the Owner shall maintain their AWWTS in a satisfactory condition capable of producing treated septic effluent in accordance with the equipment's approval for operation in the Municipality. The owner shall enter into a service agreement with an AWWTS service and maintenance provider approved by the municipality and the manufacturer of the AWWTS for the entire term of the AWWTS. In addition,it shall be the responsibility of the Owner during the term of this Maintenance and Repair Agreement, and any renewals thereof,at the owner's sole expense,to pay for any and all: (1)repair(s), (2) maintenance, (3) adjustment(s), (4)replacement costs, and(5) inspection costs. B. Owner agrees to comply with all applicable ordinance,laws,regulations,rules and orders for the AWWTS. C. Upon request by the Municipality,the owner agrees to provide the Municipality a written schedule of routine maintenance and repairs which have been performed on the system. When a record of maintenance is documented and maintained by the system vendor,the owner agrees to allow the Municipality access to this information. D. Owner acknowledges that the fine for failing to maintain and repair an AWWTS may be assessed in accordance with AMC 14.60 for improper discharge. E. Owner agrees that only maintenance and repair personnel approved by the Municipality will inspect and make any necessary maintenance, repairs or permitted alterations to the system. F. Owner agrees to grant the Municipality reasonable access to test and inspect the AWWTS upon 24 hours written notice. G. Owner agrees that any sale or transfer of title of the property will not occur without a new Certificate of On-Site Systems Approval. H. Owner agrees that the AWWTS installation and maintenance requirements as provided by the AWWTS vendor/installer and approved by the Municipality are the governing guidelines for the construction,maintenance and repair of the Owner's AWWTS. I. Owner agrees to provide and maintain a telephone connection to the AWWTS as required by the AWWTS approval. 6. Nonwaiver. The failure of either party at any time to enforce a provision of this Maintenance and Repair Agreement shall in no way constitute a waiver of the provisions,nor in any way affect the validity of the Maintenance and Repair Agreement or any part hereof, or the right of such party thereafter to enforce each and every provision hereof. 7. Amendment. A. This Maintenance and Repair Agreement shall only be amended,modified or changed by a writing, executed by authorized representatives of the parties, with the same formality that this Maintenance and Repair Agreement was executed with, and such writing shall be attached to this Maintenance and Repair Agreement as an amendment. B. The soils on the north end of the lot will not support a new system, water was discovered at -2 feet. Therefore a single test hole on the south end of the lot was excavated and had loose sandy silt SM, in the top 2 feet, then brown clean sandy gravel SP, from -2 feet to -6.5 feet then tight gray silts, ML. A pert: was completed in the brown sandy gravel, the rate was 2 minutes per inch. Due to the small arca available the new Advantcx AX 20 is the only system that can be installed. The current well is too close to the new system therefore it will be moved to the north per the plan. The old well will be dug up, cut off below grade, filled with concrete then welded shut with a steel cap. The lot slopes gently to the west at about 1 to 3 percent.
