2. All requirements specified in Anchorage Municipal Code Chapters 15.55 and 15.65 and the State of Alaska
Wastewater Disposal Regulations ( 18AAC72 ) and Drinking Water Regulations ( 18AAC80 ).
3. The engineer must notify DHHS at least 2 hours prior to each inspection. Provide notification by calling
(907) 343-4744 ( 24 hours ). ( Not required for a Water Supply Permit only ).
4. From October 15 to April 15, a subsurface soil absorption system under construction during freezing weather
must be either: A. Open and closed on the same day.
B. Covered, sealed, and heated to prevent freezing.
5. The following special provisions.
THIS PERMIT ISSUED FOR THE CONSTRUCTION OF AN ALTERNATIVE ON-SITE WASTEWATER DISPOSAL
SYSTEM. THE ATTACHED PROPERTY OWNER AGREEMENTS SHALL BECOME A PART OF THIS PERMIT
PACKAGE.

Dear Mr. Roth:
This letter provides the additional narrative for the subject project, per our telephone
conversation earlier today. The proposed disposal field for Unit 4 is not anticipated to have
negative impacts on adjacent properties. The drain field is located greater than four hundred feet
(400') from the nearest well; which is located on Lot 5 of Ptarmigan Subdivision. Surface
drainage from the area around Unit 4 drains to the south of the tract.

This letter is a supplement to infolrnation previously submitted to your office dated June 11~h and
June 17'~' of 1998 for unit four (4) of White Stone Estates Condominiums. Specifically this letter
transmits the calculations for the wastewater disposal field and a nan'ative description of
probable impacts to adjacent properties.
There are no wells, septic systems or reserve areas within a two hundred-foot (200') radius of the
proposed disposal field. There are no negative impacts to adjacent properties anticipated with
the disposal field for unit four (4). Drainage in the immediate ama of unit four (4) will drain to
the north and south as shown on the previously submitted topography information.

PROPERTY OWNER AGREEMENT

This agreement, dated July 16th 1998, is made between the Municipality of
Anchorage Department of Health and Human Services (DHHS) and the property
owner(s) of: White Stone Estates Condominiums Unit 4 located at:
East 1/2 of the Southeast 1/4 of the Northwest 1/4 of Section 17,
Township 14 North, Range 1 West, Seward Meridian.
This agreement is made for the purpose of maintaining an on-site wastewater disposal
system on the subject property.
The property owners agree to th, e following:
Submk to the Mun/cipal~ty of Anchorage, on an annual basis, an inspection and
operation statement from a registered professional engineer. This inspection and .
operation statement shall verify that the engineer has inspected all effluent and air
pumps, timers, and alarms, and that any deficiencies have been repaired anil that the
system is functioning as designed.
