Septic System Permit Request
Ladies and Gentlemen:
I am writing to request a permit to install a new septic system be issued for this property. The proposed
systems will serve a existing two (2) bedroom house. Currently the existing house is served by a holding
tank. The proposed system will utilize a 1500g S.T.E.P. tank and a conventional drain field. The
surrounding developed lots are served by private wells. There are no wells on surrounding lots within 100
feet of this system.
Soils. We performed one test hole on this lot, and groundwater was monitored on 5/28/2014 and ground
water was observed at 5 feet below the ground elevation in the test hole monitor tube. From 0.5' to 3'
was an sandy gravel with silt, al layer of silt form 3-4' and a silty sandy gravel from 4-8'. It is my opinion,
based on the results of the soils tests, the use of a category 3 system and overall soils appearance; an
application rate of 5 gallons/day/square feet should be used for a Advantex wastewater system.
1. Soil Absorption System Design.
a. See Sheet 2 of the design package.
2. Surface Water: There is no surface water within 100 feet of the proposed septic tank and drain
field. The proposed drain field upgrade will maintain at least 100 feet from all surface water and drainage
ditches.
3. Topography: The existing topography slopes from the northeast to southwest at approximately
4% in the area of the proposed drain field. There are no steep slopes within 50' of the proposed drain
field.

4. Waiver Request for use of Engineered Receiving Soil (ERS). I am requesting a waiver to the 1992
Code to use the proposed Code Update Section of Engineered Receiving Soils (ERS). Below I will detail my
request justifying the use of ERS:
a. Two test holes were excavated, one by PES in 2014, and one by Hery A. Elias in 1999. The
soils logs are attached. Ground water was monitored in both test holes.
b. This design proposes to use two types of ERS. The base soils that will act as an interface
between the existing soils and the soil absorption system (SAS) will be MoA Type II or Type
IIA, which has a specified gradation per MASS. This interface will be approximately one foot
in depth and will be compacted to approximately 80 percent to allow free flow of the
effluent through it into the native soil using MoA Waste Water Code application rates. The
infiltration area of the interface between the ERS and the in-situ soil was based on the 7.4
minute per inch percolation rate of the in-situ receiving soil. A two foot thick filter sand
layer will be placed over the MoA Type II material to act as an interface between the SAS
and ERS.
c. The SAS is sized assuming a percolation rate of the ERS soil, assumed 6-15 MPI, which gives
an application rate of 5 GPDSF.
d. The system will protrude above the ground surface. The cover of the system will produce a
mound, which will have at a minimum, three to one side slopes and six inches of organically
rich soil to promote the growth of vegetation.
S. Drawing Markings: The Drawings are marked "For MoA Review Only". When written notification
that the review is complete and that there are no further comments is received from MoA On -Site
Department, the note will be removed and "Issued for Construction" drawings will be issued.
The proposed installation will not affect the future development of the surrounding or existing lots.

MAINTENANCE AND REPAIR AGREEMENT
BETWEEN MUNICIPALITY OF ANCHORAGE
THIS MAINTENANCE AND REPAIR AGREEMENT made and entered into as of this

herein the "OWNER," and the Municipality of Anchorage, herein the
"MUNICIPALITY." In consideration of the mutual covenants contained herein, the
parties to this Maintenance and Repair Agreement agree as follows:
1. Advanced Wastewater Treatment Systems. Municipality grants permission to
Owner to utilize and operate an Advanced Wastewater Treatment System
(AWWTS), described as
Advantex Filter Processinq System
located at (legal description).
Rabbit Creek #2, Blk 1 Lt 5 (7040 Our Own Lane)
2. Definitions.
Alteration. Any change to the design or function of an AW WTS that
includes the installation or removal of any parts, components or pieces not
included in the original construction permit and design.
Certificate of On -Site Systems Approval (COSA). An approval by the
Municipality of existing water and wastewater disposal systems given at
the time of property sale and title transfer in accordance with Anchorage
Municipal Code (hereinafter, "AMC) 15.65. These approvals certify that
the systems are adequate for the homes that they support and meet the
codes that were in place at the time of system construction.
Damage. Any man-made or natural change in a system that would inhibit
the system from performing as designed.
Maintenance and Repair. The scheduled and as needed replacement of
existing parts, components and pieces of an AWWTS that were included
in the original design which would allow the AWWTS to continue to
perform as designed.
Permit. An On -Site Wastewater Disposal Permit as required by AMC
15.65 to construct and operate an AWWTS.
3. Term. The term of this Maintenance and Repair Agreement shall begin on the
date of approval by the Municipality to operate the installed system or issuance of a
COSA, and shall continue while the AWWTS is in use or is operational or until the
property is sold or title is transferred by the owner and a new COSA is issued to the
new owner or transferee of the property.
4. Alterations. Installation and Removal of Additional Equipment. Prior to
performing any alterations to an AWWTS, the owner agrees to obtain an On-site
Wastewater Disposal Permit from the Municipality in accordance with AMC 15.65.
5. Maintenance and Repairs.
A. Throughout the term of this Maintenance and Repair Agreement, the Owner shall
maintain their AWWTS in a satisfactory condition capable of producing treated
septic effluent in accordance with the equipment's approval for operation in the
Municipality. The owner shall enter into a service agreement with an AWWTS
service and maintenance provider approved by the municipality and the
manufacturer of the AWWTS for the entire term of the AWWTS. In addition, it
shall be the responsibility of the Owner during the term of this Maintenance and
Repair Agreement, and any renewals thereof, at the owner's sole expense, to pay
for any and all: (1) repair(s), (2) maintenance, (3) adjustment(s), (4) replacement
costs, and (5) inspection costs.
B. Owner agrees to comply with all applicable ordinance, laws, regulations, rules
and orders for the AWWTS.
C. Upon request by the Municipality, the owner agrees to provide the Municipality a
written schedule of routine maintenance and repairs which have been performed
on the system. When a record of maintenance is documented and maintained by
the system vendor, the owner agrees to allow the Municipality access to this
information.
D. Owner acknowledges that the fine for failing to maintain and repair an AWWTS
may be assessed in accordance with AMC 14.60 for improper discharge.
E. Owner agrees that only maintenance and repair personnel approved by the
Municipality will inspect and make any necessary maintenance, repairs or
permitted alterations to the system.
F. Owner agrees to grant the Municipality reasonable access to test and inspect the
AWWTS upon 24 hours written notice.
G. Owner agrees that any sale or transfer of title of the property will not occur
without a new Certificate of On -Site Systems Approval.
H. Owner agrees that the AW WTS installation and maintenance requirements as
provided by the AW WTS vendor/installer and approved by the Municipality are
the governing guidelines for the construction, maintenance and repair of the
Owner's AW WTS.
6. Nonwaiver. The failure of either party at any time to enforce a provision of this
Maintenance and Repair Agreement shall in no way constitute a waiver of the
provisions, nor in any way affect the validity of the Maintenance and Repair
Agreement or any part hereof, or the right of such party thereafter to enforce each and
every provision hereof.
7. Amendment.
A. This Maintenance and Repair Agreement shall only be amended, modified or
changed by a writing, executed by authorized representatives of the parties, with
the same formality that this Maintenance and Repair Agreement was executed
with, and such writing shall be attached to this Maintenance and Repair
Agreement as an amendment.
B. For the purposes of any amendment modification or change to the terms and
conditions of this agreement, the only authorized representatives of the parties
are:
a. Owner: Henry A./Ramona J. Elias
b. Municipality: Director, Community Development or designated authority
C. Any attempt to amend, modify, or change this contract by either an unauthorized
representative or unauthorized means shall be void.
8. Jurisdiction: Choice of Law. Any civil action arising from this Maintenance
and Repair Agreement shall be brought in the Superior Court for the Third Judicial
District of the State of Alaska at Anchorage. The laws of the State of Alaska shall
govern the riehts and oblieations of the oarties under this Maintenance and Reuair
Agreement.
9. Severability. Any provisions of this Maintenance and Repair Agreement decreed
invalid by a court of competent jurisdiction shall not invalidate the remaining
provisions of the Maintenance and Repair Agreement.

SUBJECT: Subsurface Soils Study
Dear Mr.. McDowell:
Transmitted herein are the results of a test boring placed in
the project property at the specified location and depth.
The data is organized in this report as follows:

The test hole was drilled by Denali Drilling, Inc., supervision,
testing and report by Alaska Testlab.
The test hole was placed by means of a truck mounted Mobile B-50
drill fitted with continuous solid flight auger..
Grab samples were taken from the buttings and the auger_ bit as
needed.
The test hole was placed on the 17th of. July, 1975, supervised,
logged and sampled by Mr. Peter Ilerstek, technician of our staff.
The overlying material consisted of 2.0 feet of peat.
The soil from 2.0 to 7.5 feet was borderline with regard to frost -
susceptibility and was a silty gravelly sand.
From 7.5 feet to 16.0 feet (bottom of hole) the soil was a frost -
susceptible gravelly silty sand becoming more gravelly with depth
from 11.0 feet.
No free water was observed at the time of. drilling.
The Greater Anchorage Area Borough has established the amount
of seepage area per bedroom required for a particular seepage
pit based on the unified classification of the soil. We under-

Dear Mr. Sutton:
As per your request, I have performed the adequacy test on the absorp-
tion system on the above described lot in accordance with the Munici-
pality of Anchorage recommended procedures for a one -bedroom home.
The system consisted of a septic tank and approximately 60 feet of
trench in a tee formation with standpipes at both ends and at the
tee location as well as at the septic tank.
Two hundred gallons was added to the absorption system and the levels
at both ends of the drainfield and at the septic tank were monitored
at 40 gallon increments. (See attached tabulation).
Based on the test data acquired, it is my opinion that the absorption
system is fucti.oning adequately for a one -bedroom home.

ti 1) The water analysis report needs to be submitted to
this office from the Chem Lab, 5633 B Street, for
our review.
(2) The depression over the tank needs to be filled in
with dirt and reinspected by this office.
C(3) The septic tank pumped with a receipt submitted to
this office. The number of gallons pumped need to
be on the receipt and verified by a registered engineer.
This is to verify the size of the septic tank.
J(4) An adequacy test needs to be performed on the existing
leaching area. This test will_ determine if the system
is adequate according to National Standards. A listing
1�l of private firms performing the test is enclosed. This
report will need to be submitted to this office for our.
review.
