The septic tank is very old and has started to fail. From the attached site plan the well encroaches on the septic tank but a waiver has been issued in 2000 for all of the encroachments, including the neighboring lots. On 12/6/99, a well flow test and a septic system adequacy test was performed for the purpose of obtaining a Health Authority Approval. During our site visit, it was detemfined that there was several encroachment concerns. After a detailed record research it was determined that the well encroaches 4 septic tank (including the septic tank serving this property) m~d 2 drainfields (including the drainfield that serves this property). Attached is a site plan and a detailed drawing of the encroaclunents. We request that your depat~tment issue a Health Authority Approval and grant a blanket 65 feet separation distance waiver from the well to ali encroaching septic tanks, and a blanket 90 feet waiver to all encroaching drainfields. The following items are justification for the waivers: The location of the encroaching septic tanks and drainfield are in very visible area (all are in front yards) so that if any effluent was to surface, it would be noticed and the problem corrected. · Recent water sample results indicated nitrate levels at 0.50 mg/L and 9 other bacteria with no coliform. · Attached are several well logs and print-outs of the Department of Natural Resources database of neighboring properties that show the well depths and some soils classifications. On the well logs, there appears to be some clay layers and good sand material which will act as confining/filtering material. The well for Lot 6, Block 7, Sand Lake #2 was not be able to found in the field to verify that separation distance to the drainfield serving the referenced property. The drainfield for the referenced property was installed on 10/14/77 and the well for Lot 6, Block 7, was drilled in 1978 (according to DHHS records: 12/1/78 HAA for Lot 6, Bk 7). In short, it appears that the septic system for Lot 19, Bk 6 was installed first.
