RE: SEPTIC SYSTEM UPGRADE PERMIT
LEGAL: LOMA ESTATES BLOCK 3, LOT 11
The septic system has been found failed and will be completely decommissioned
and per the owner we request a septic permit to upgrade the existing septic system
on the above referenced lot. We propose to install a shallow trench with an
Advantex system to serve the existing 4-bedroom residence per the attached design.
The design is based on the recent test hole conducted in the spring of 2023.
Groundwater was observed at excavation at 5’ and 4’ at monitoring of this test hole.
The slopes are moderate at 5-10% at the proposed upgrade location. The lot and
area are served by private water with no known private wells within 50’ of the
proposed septic system. Based on visual observations and review of available
information, the design will not impact any of the neighboring properties. Please

Septic System Design and Permit Application
Impacts to Adjacent Properties
Dear Onsite services Engineer:
The existing absorption trench on Lot 11, Block 3, Loma Estates Subdivision has failed
and must be replaced. This system has been in use s~nce 1977 so soils on the lot can
be considered conducive for the treatment of septic effluent. We are therefore applying
for a permit to construct a new absorption septic system on the lot to serce the home.
The attached Site Plan and backup documentation identify the location and
configuration of the new septic system and the parameters used in the design. It also
shows the location.of the ,e, xisting well on the lot and a well previously abandoned. Both
wells are more than 100 from all, components of the e~(isting and proposed septic
system.
Two test holes were rece,n, tly placed on the lot in the area of the new absorption trench.
Both holes revealed similar materials with well §faded silty gravel with a percolation
rate between 8 and 10 minutes per inch. No groundwater was encountered in either
excavation and none accumulated during [he monitoring period. We noted the soil
materials were much looser t,o, ward the top of the test holes so we have designed a 5'
wide absorption trench With 3 of gravel beneath the distribution pipe.. The total length
of the trench will be 64'. The total depth of the trench from original grade will be 5'. Fill
will be placed over the trench to provide a minimum of 3' of cover.
The ground surface on the lot slopes from east to west at between 10% and 15%. It
also slopes from north to the south at varying grades. The new trench will be
constructed parallet to the contours of the surface 'n conformance with Municipal
requirements. Grading will be accomplished to assure surface drainage is away from
the new trench.
f the system s constructed in accordance with our design the following statements
apply:
The system, if constructed as designed, will have no adverse ~mpact on the wells
in the area or tr'ose to be constructed in the future,
2. The system, if constructed as designed, will have no adverse impact on existing

septic systems in the area or those to be constructed in the future.
3. The system if constructed as designed, wil have no adverse ~mpact on reserve
space, either surface or subsurface, on any lots located in tl~e area.

patterns in the area. The current drainage pattern will be maintained.

ON-SITE WASTEWATER DISPOSAL SYSTEM
CONSTRUCTION AND MATERIAL SPECIFICATIONS
SUBJECT: LOT 11, BLOCK 3, LOMA ESTATES SUBDIVISION
GENERAL:
The scope of this project includes the exposing and verification for
continued use of the existing 1,000 gallon septic tank. If the tank is
found unacceptable for continued use work will include the
procurement and placement of a new 1,000 gallon septic tank at the
Iocation shown on the Site Plan. Work also includes the construction
of a new 64' long X 5' wide X 3' effective depth absorption trench at
the location shown. The distribution line in the trench will be placed at
2' below the existing ground surface. Total depth of the trench wilI be
5' below the existing ground surface. Care must be taken to assure
100' separation from the existing and abandoned wells on the property
to all components of the new septic system.
Construction shall be in accordance with the approved site plan,
design drawings, Municipal Permit with any special provisions or
conditions, and all applicable State and Municipal Wastewater
Disposal Regulations.
The Contractor shall be responsible for obtaining all underground utility
locates and for the layout of the septic system and verification of the
location of all lot lines.
Unless specifically agreed otherwise, the contractor shall be
responsible for final grading areas subsequently depressed from soil
settling. Property owner shall be responsible for revegetation of
affected areas unless specifically agreed otherwise.
Contractors installing wastewater disposal systems must be certified
by the Municipal Department of Health and Human Services for
system installations. Owners installing their own systems must receive
prior approval from D.H.H.S. before beginning system installation.
SEPTIC TANK INSTALLATION
A new 1,000 gallon septic tank must be be procured from an approved
source and installed at the location shown on the plans if the existing
tank is found unacceptable for continued use.
A septic tank is to be constructed by a certified septic tank
manufacturer. Construction shall include two 4" cleanouts for pumping
access.
3. The septic tank shall be sufficiently bedded to prevent settling or
shifting of the tank.
4. All standpipes on the septic tank shall extend a minimum of 12 inches
above final grade.
5. Tanks installed without 4' of cover shall have a minimum of 2" of direct
burial insulation.

6. A foundation cleanout shall be installed one to four feet from the
building foundation. Two cleanouts are required between the tank and
the drainfield.
7. Final grading over the tank shall be such that a positive slope exists
away from the septic tank.
DRAINFIELD CONSTRUCTION:
The drainfield shall be constructed to the dimensions shown on the
design. The bottom of the trench shall be within 2" of level.
Distribution piping must be placed levei with perforations down atop a
level bed of drainfield rock. Rock should then be placed over the pipe
to provide a minimum of 2" of cover.
3. A silt barrier or geotextile fabric must be placed between the drainfield
rock and the natural soil backfill·
4. Monitor tubes must be 4" in diameter and installed at the locations
shown on the design. The portion below ground must be perforated.
Contractor shall verify the septic tank and drainfield are a minimum
100' away from any private water wells in the area, 150' from a Class
"C" Well or 200' from any community well.
Direct bury insulation must be placed over the distribution system if
less than 3' of backfill depth is available. Finish grade over the trench
must be mounded to prevent settlement or depressions.
7. Grade area surrounding the absorption trenches to drain away.
A minimum 2' of accepting soil is required below the drainfield rock for
a 5' wide trench. Contractor shall verify this condition prior to
placement of the rock. All pockets of unacceptable materials must be
removed and replaced.
MATERIAL SPECIFICATIONS:
1. Septic tanks must be constructed by a municipally approved septic
tank manufacturer.
2. The following pipe materials are approved for use in septic system
installations in the Municipality of Anchorage:
Cast Iron (perforated and solid), ASTM D3034 or P.V.C. (perforated
and solid), ASTM F810 or H.D.P.E. (perforated, but not solid) and
ASTM D2662 or A.B.S. (perforated and solid).
3. Insulation shall be at least 2" thick extruded direct burial polystyrene
(Dow Chemical Co. Styrofoam HI or equal).
4. Septic tank inlets and outlets shall be fitted with watertight couplings
(Caulder, Fernco, or equal).

5. A permeable geotextile fabric (Typar, Mirafi or equal) must be installed
between the final drain rock layer and the native soil layer.
6. All drain rock shall be .5" to 2.5" in diameter with less than 3% passing
the #200 sieve.
INSPECTIONS:
Municipal Ordinance requires a m~nimum of two inspections. These
inspections must be conducted under the supervisicn of a professional
engineer registered in the State of Alaska. The first inspection must be
conducted after the excavation of trenches beds or pits and before the
installation of any gravel A septic tank may be set in place, but may not
be backfilled.
The second inspection must be conducted after the placement of the
geotextile fabric gravel, distribution piping, standpipes, cleanouts and
insulation. No backfill should be in place at [he time of inspection.
Contractor shal provide a copy of alt field survey layout and construction
notes for use in preparing the certified as-built of the completed system.

accordance with the equipment's approval for operation in the Municipality.
It shall be the responsibility of the Owner during the terra of this Agreement to pay for all
repair(s), maintenance, adjustment(s), replacement costs, and inspection costs. This
DS includes an annual maintenance fee (typically $400 to $600).

AWWTS approval.
3. Term. The term of this Agreement shall begin on the date of approval by the
Municipality to operate the installed system, or upon transfer of title, and shall continue
while the AWWTS is operational or until title is transferred.
4. Nonwaiver. The failure of the Municipality at any time to enforce a provision of this
Agreement shall in no way constitute a waiver ofthe provisions, nor in any way affect
the validity of the Agreement or any part hereof, or the right of the Municipality
thereafter to enforce every provision hereof.
5. Amendment. This Agreement shall only be amended by authorized representatives of
the Owner and Municipality. Any attempt to amend this agreement by either an
unauthorized representative or unauthorized means shall be void.
6. Jurisdiction: Choice of Law. Any civil action arising from this Agreement shall be
brought in the Superior Court for the Third Judicial District of the State of Alaska at
Anchorage. The laws of the State of Alaska shall govern the rights and obligations of the
parties under this Agreement.
7. Severability. Any provisions of this Agreement decreed invalid by a court of competent
jurisdiction shall not invalidate the remaining provisions of the Agreement.

The Municipality of Anchorage Department of Health and Human Services (DHHS) issues Certificates of
Health Authority Approval (HAA) based only upon the representations given in paragraph 5 by an independent
professional civil engineer registered in the State of Alaska. Certificates of Health Authority Approval are
required for the transfer of title (except between spouses) on properties served by a single family on-site
wastewater disposal and/or water supply system. DHHS also issues HAAs upon request to home owners.
Certificates of Health Authority Approval are valid for 90 days from the date of issue for properties served by
a private or Class C well and may be reissued with new water sample results less than 30 days old. Cedificates
are valid for one year for properties served by Class A or B wells or a public water system. The Municipality
of Anchorage is not responsible for errors or omissions in the professional engineer's work.
5. STATEMENT OF INSPECTION BY ENGINEER
As certified by my seal affixed hereto and as of the validation date shown below, I verify that my investigation
based on procedures outlined in the Health Authority Approval Guidelines for the Health Authority Approval
application show that the on-site water supply and/or wastewater disposal system is safe, functional and

During a recent Health Authority Approval on-site inspection
and test of the potable water supply well on Lot /~
Block ~ of A O~ ~7-~r~&Subdivision, the well's
productivity was determined to be _~,~ gallons per minute.
The minimum well productivity required by this Department
(~MC 15.55) for a _~ bedroom residence is O, ~1 gallons
per minute. Although the subject well currently exceeds this
minimum requirement, all parties concerned are advised that the
production capacity of the well may fluctuate. Restriction
of ncn-criticat water uses such as washing cars and watering
lawns and gardens may be required.
This advisory must be attached to all copies of the subject
Health Authority Approval.
