Request you issue a permit to install a septic systeln to serve the proposed four
bedromn house on the referenced property.
One test hole was excavated and a percolation test was performed. Thc approximate
locatiou of thc test hole is located on the attached site plau.
At the time of excavation no ground water was encountered iu the tcst hole. After seven
da), grouud water monitoriug, the monitoriug tube was checked and the hole was fonnd to
be dry.
We do uot anticipate any adverse effects on neighboring wells, septic systems or drainage
patterns by the installation of the proposed septic system.

GENERAL:
The scope of this project includes the installation of a 1250 gallon septic tank
and a leaehfield trench to serve the proposed fonr bedroom residence located on
the referenced property.
Construction shall be in accordance with the approved site plan and design
drawings, Municipal permit with any special provisions or conditions, and all
applicable State and Municipal Wastewater Disposal Regulations.
The contractor shall be responsible for obtaining any necessary underground
utility locates.
Unless specifically agreed otherwise, the property owner shall be responsible for
final grading areas subsequently depressed fi'om soil settling.
Contractors installing wastewater disposal systems must be certified by the
Municipal Health Department for system installations. Owners installing their
own systems nmst also receive phor approval fi'om the Municipal Health
Department.
SEPTIC TANK INSTALLATION:
A septic tank is to be constructed by a certified septic tank manufacturer.
Construction shall include two 4" cleanouts for pumping access.
The septic tank shall be sufficiently bedded to prevent settling or shifting of the
tank.
All standpipes on the septic tmrk shall extend a minimum of 12 inches above
final grade.
4. Septic tanks installed with less than 4 fL of cover shall be insulated.

A foundation cleanout shall be installed one to four feet from the building foundation.
In the line between the tank and the leachfield there shall be two adjacent cleanouts
(unless an effluent pumping system exists within the septic tank). These cleanouts shall
be located on undisturbed soil not more than 10 ft. fi'om the tank. The first cleanout, in
line, shall be to clean toward the leachfield. The second cleanout shall be to clean
toward the septic tank.
Final grading over the septic tank shall be such that a positive slope exists away from
the septic tank.
ABSORPTION TRENCH/DRAINFIELD INSTALLATION:
Excavate the proposed trench to the dimensions shown on the design. The bottom of
the excavation shall be within 2 inches of level. If the sidewalls of the excavation
become smeared, they must be raked or scratched (ruffed-up) before gravel (sewer rock)
placement.
Once the gravel is installed, the distribution pipe is to be installed level with the
perforations faced downward. Gravel is then to be placed over the distribution pipe to
provide a minimum of 2 inches of cover over the pipe.
A silt barrier must be installed between the final gravel layer and the native soil backfill.
Ensure the silt barrier covers the entire gravel surface before placing backfill.
Monitor tubes shall be of four (4) inch diameter, installed approximately in the
locations shown on the design, and extend a ~ninimum of 12 inches above final grade.
The portion of the monitoring tube extending through the gravel shall be perforated
fi'om the bottom of the trench to the invert of the distribution pipe. This is equivalent to
the effective depth of the gravel as noted on the design.
Backfill over the final gravel layer must not be less than twenty-four (24) inches.
Insulation must be installed when the backfill depth is less than thirty-six (36) inches.
The finish grade over the trench must be mounded to prevent the formation of a
depression after settling.

MINIMUM MATERIAL SPECIFICATIONS:
Any septic tank proposed for installation must be constructed by a Municipal approved
septic tank manufacturer.
The following pipe materials are approved for use in septic system installations in the
Municipality of Anchorage:

Use of a type of pipe other than listed above must be approved by the inspecting
engineer.
3. Insulation shall be at least 2" thick extruded direct burial polystyrene (Dow Chemical
Company Styrofoam HI or equal).
4. Septic tank inlets and outlets shall be fitted with watertight couplings (Caulder, Femco,
or equal).
5. A permeable nontoxic silt barrier (Typar 3401, Mirafi 140N, or equal) must be installed
between the final leachfield gravel layer and the native soil backfill.
6. All leachfield gravel (sewer rock) shall be 0.5"-2.5" screened gravel with less than 3%
passing the #200 sieve.
7. When sand is being used as a filter material, its gradation specifications must conform
to current M.O.A. or D.E.C. requirements, which ever requirement applies.
INSPECTIONS:
Typically there will be a minimum of three (3) inspections required during the installation of
the wastewater disposal system. These inspections will occur as follows:
1. The first inspection must be conducted after the excavation of ditches, pits,
trenches, or beds and before the installation of any gravel. A septic tank may be
set in place, but may not be backfilled before this inspection.

The second inspection must be conducted after the placement of the silt ban'ier,
gravel, distribution lines, standpipes, cleanouts, m~d insulation, but before the
placement of m~y other backfill.
3. The final inspection is to occur upon final grading of the property.
Often there will be more than these 3 inspections required. Especially with the installation of
multiple trenches, sand filters, pressurized distribution systems, etc. Thus, the inspecting
engineer is to be contacted at least 24 hours prior to the start of construction, if necessary, a
pre-construction meeting will take place on-site. The inspecting engineer will not coordinate,
direct or control in any way the contractors activities.
The owner shall contract with the contractor to perform the work outlined in these
specifications and plans and in accordance with the attached M.O.A. permit. There will be no
contractual arrangement existing between the contractor and S & S Engineeriug. S & S
Engineering shall be the owner's representative and will inspect the work as stated above to
document the contractors activities. Final acceptance of the contractors work rests with the
owner and the M.O.A.
S & S Engineering shall have no liability to the owner or to others for acts or omissions of the
contractor or any other persons perforating work on this project or the failure of the contractor
to carry out the work in accordance with these construction documents. S & S Engineering's
inspecting engineer will not be responsible for the construction means, methods, techniques,
sequence, procedures or the safety precautions incident to this project.

5. STATEMENT OF INSPECTION BY ENGINEER
As certified by my seal affixed hereto and as of the validation date shown below, I verify that my investigation, based on procedures
outlined in the Certificate of On -Site Systems Approval Guidelines for this application, shows that the on-site water supply and/or
wastewater disposal system is (are) safe, functional and adequate for the number of bedrooms and type of structure indicated
herein. I further verify that based on the information obtained from the Municipality of Anchorage files and from my investigation and
inspection, the on-site water supply and/or wastewater disposal system is (are) in compliance with all applicable Municipal and State
codes, ordinances, and regulations in effect at the time of installation.
In conducting an adequacy test, I attempt to provide a thorough, conscientious engineering analysis of the system in accordance with MoA
COSA guidelines and regulations. The reported results describe the performance of the system under the conditions encountered at the time
of the test, and separation distances measured to readily identifiable features. The operational life of all wells and septic systems depend on
the local soil condition, ground water levels that may fluctuate during the year, and the water usage of the family being served by the system.
These conditions are outside the control of the evaluator of this system. All systems eventually fail and satisfactory test results do not
guarantee future performance of the system, nor do they guarantee that there are no hidden defects or encroachments. Therefore we cannot
provide any warranty for future performance, nor can we estimate remaining life of the system. The content of this report is for the sole
benefit of the owner listed above. Reliance on this report by another person is at their own risk. Pannone Engineering Services LLC highly
recommends buyers hire their own engineer to evaluate this report.

My apologies for the late response, I went home sick yesterday.
1. Per the technicians field notes, the water level rose roughly 3/8 of an inch. We count anything under a %" as an
insignificant enough amount of water to denote it as zero. After discussion with multiple other Engineers, the
practice of stating zero for less than a % of an inch in rise is standard practice and has been for 30 years. In
talking with the various engineers performing COSA's, they have indicated that a zero inch rise is fairly routine
and have never been called out on a zero inch rise. We are assuming that this code reference and indicating a
minimal rise is going to be uniformly and consistently applied to all engineers from here on out? This is the first
time we have experience the callout of this code reference.
2. 1 have reviewed the eplans submittal and I believe I was clear that it was 5 % Inches and not Feet.
3. The map you referred to in the original comments, is accurate and the correct map. The street is no longer
named Terrace Wood Lane, but is in fact, South Park Loop per the most recent AWWU 300 scale record grid map
(attached.) The keybox is accurately represented on the AWWU record drawing (Record Drawing 13894),
making the sketch that was added to a -plans in response to the comment, the logical representation of the
water line routing. I have attached to this email, the connect card from AWWU showing the location of the
properties key -box in the southeast corner of the lot, and approximately 3 FEET from the east property
line. Unfortunately, that card does not show the actual water line alignment (AWWU purchased the South park
Water system in 1994.) 1 have also included on the sketch the 10ft separation distance from each septic pipe to
prove that even with the worst possible case of logical water line installation, the 10ft distance would not be
exceeded. We assert that the information provided is adequate to prove there will be greater than 10ft
separation between the septic system and the water line.

I Just listen to your message.
1. The code says to add sufficient amount of water till you get a rise. Adding 600 gallons in 70 min should have
resulted in a rise in the liquid level of at least a half inch rise if that is what you are measuring to.. Please check
the field notes.
2. Please clarify that 5.5 is not 5 foot 5 inches.
3. You will need to supply either correct mapping along with connect cards with enough information that a logical
assumed path can be made now and in the future or Steve can do a quick site plan with the location of the key
box and the assumed path of the public water line. This needs to be stamped and signed. Or the last option is to
have it professionally located and so noted on a drawing.
The other comments are addressed.
