REFERENCE: Ginami Hills Subdivision;. Lot 7B
Request you issue a permit to upgrade the septic system serving the
four bedroomhouseonthe referenced property.
Test holes were excavated and percolation tests performed in the area
of the proposed upgrade. The approximate locations of the test holes
are located on the attached site plan. At the time of excavation no
water was encountered and after seven day ground water monitoring water
was found at 10' in test holes #1 and #2.
We do not anticipate any adverse effects on neighboring properties by
the installation of the proposed septic system.

The scope of this project includes the installation of a 500 gallon
septic tank effluent pumping (S.T.E.P.) system and two absorption beds
to serve the four bedroom residence located on the referenced property.
The existing septic tank is to be excavated to verify its integrity. If
of poor integrity, the tank is to be excavated, pumped, crushed and
abandoned in place and a new 1500 gallon septic tank effluent pumping
(S.T.E.P.) system installed. The existing trench is to be abandoned in
place.
Construction shall be in accordance with the approved site plan and
design drawings, Municipal permit with any special provisions or
conditions, and all applicable State and Municipal Wastewater Disposal
Regulations.
The contractor shall be responsible for obtaining any necessary
underground utility locates.
Unless specifically agreed otherwise, the property owner shall be
responsible for final grading areas subsequently depressed from soil
settling. On all leachfield mound systems, the property owner shall be
responsible for ensuring a satisfactory vegetation growth over the
mounded area.
Contractors installing wastewater disposal systems must be certified by
the Municipal Health Department for system installations. Owners
installing their own systems must also receive prior approval from the
Municipal Health Department.
1. A septic tank is to be constructed by a certified septic tank
manufacturer. Construction shall include two 4" cleanouts for pumping
access.
The septic tank shall be sufficiently bedded to prevent settling or
shifting of the tank.
Ail standpipes on the septic tank shall extend a minimum of 12 inches
above final grade.
4. Septic tanks installed with less than 4' of cover shall be insulated.

A foundation cleanout shall be installed one to four feet from the
building foundation. In the line between the tank and the leachfield
there shall be two adjacent cleanouts (unless an effluent pumping system
exists within the septic tank). These cleanouts shall be located on
undisturbed soil not more than 10' from the tank. The first cleanout,
in line, shall be to clean toward the leachfield. The second cleanout
shall be to clean toward the septic tank.
Final grading over the septic tank shall be such that a positive slope
exists away from the septic tank.
P~F. qSURIZED M(]~ND ~"fSTEM INSTALLATION:
1. Any peat or organic matter must be removed from the elevated mound site.
2. The bottom of the basal bed area as well as the top of the sand filter
is to be within two inches of level.
3. The distribution piping is to be of P¥C (ASTM D3034 or equal). Ail
joints are to be solvent cemented.
4. The side slopes of the top layer of the mound system must not be steeper
than 33% (3:1).
5. The top of the mound shall be covered with a minimum of 6 inches of
topsoil and vegetated sufficiently to prevent erosion.

The distribution pipes are to be embedded in sewer rock. Care should be
taken to backfill in such a way as to prevent damage to the piping
system.

Silt barrier material must be installed between the final gravel layer
and the native soil backfill. Ensure the silt barrier covers the entire
gravel surface before placing backfill.
Backfill over the final gravel layer must not be less than twenty-four
(24) inches. Insulation must be installed when the backfill depth is
less than thirty-six (36) inches.
MINI34UM MATERIAL SPECIFICATIONS:
1. Any septic tank proposed for installation must be constructed by a
Municipally approved septic tank manufacturer.

2. The following pipe materials are approved for use in septic system
installations in the Municipality of Anchorage:

Use of a type of pipe other than listed above must be approved
the inspecting engineer.
Insulation shall be at least 2" thick extruded direct burial polystyrene
(Dow Chemical Company Styrofoam HI or equal).
Septic tank inlets and outlets shall be fitted with watertight couplings
(Caulder, Fernco, or equal).
A permeable nontoxic silt barrier (Typar 3401, Mirafi 140/N, or equal)
must be installed between the final leachfield gravel layer and the
native soil backfill.
Ail leachfield gravel (sewer rock) shall be 0.5"-2.5" screened gravel
with less than 3% passing the ~200 sieve.
When sand is being used as a filter material, its gradation
specifications must conform to current M.O.A. or D.E.C. requirements.
INSPECTIONS:
Typically there will be a minimum of three (3) inspections required during the
installation of the wastewater disposal system. These inspections will occur as
follows:
The first inspection must be conducted after the excavation of ditches,
pits, trenches, or beds and before the installation of any gravel. A
septic tank may be set in place, but may not be backfilled before this
inspection.

The second inspection must be conducted after the placement of the silt
barrier, gravel, distribution lines, standpipes, cleanouts, and
insulation, but before the placement of any other backfill.

3. The final inspection is to occur upon final grading of the property.
Often there will be more than these 3 inspections required, especially with the
installation of multiple trenches, sand filters, pressurized distribution
systems, etc. Thus, the inspecting engineer is to be contacted at least 24
hours prior to the start of construction. If necessary, a pre-construction
meeting will take place on-site. The inspecting engineer will not coordinate,
direct or control in any way the contractor's activities.
The owner shall contract with the contractor to perform the work outlined in
these specifications and plans and in accordance with the attached M.O.A.
permit. There will be no contractual arrangement existing between the
contractor and S & S Engineering. S & S Engineering shall be the owner's
representative and will inspect the work as stated above to document the
contractor's activities. Final acceptance of the contractor's work rests with
the owner and the M.O.A.
S & S Engineering shall have no liability to the owner or to others for acts or
omissions of the contractor or any other persons performing work on this project
or the failure of the contractor to carry out the work in accordance with these
construction documents. S & S Engineering's inspecting engineer will not be
responsible for the construction means, methods, techniques, sequence,
procedures or the safety precautions incident to this project.

The exploration was conducted using a Nodwell mounted Mobile
Drill model B-50 drill rig with a continuous flight solid
stem auger. The rig is owned and operated by Denali Drilling Inc.
Drilling was supervised and the test holes logged by Mr. Terry
Barber, 'geologist with Alaska Testlab. The percolation test
was run by Mr. R. Mark Hansen~ engineer with Alaska Testlab.
The test hole was placed at the approximate location shown
on Figure 2. The log of this test hole is included as Table A
of this report. In interpreting the log it would be helpful
to utilize the explanatory information contained in sheets
1 to 3 of this report.
When dri. lling was completed a 3/4" slotted PVC pipe was inserted
in the hole to aid in determining the free water level. For
the percolation test, the test hole was filled with water
and left overnight to saturate. On returning the next day
the hole was refilled with water and the drop in the water
level carefully monitored over the next two hours.

This procedure is not a standardized percolation test, however,
we understand that the Anchorage Department of Environmental
Quality prefers tests performed in this manner to evaluate
a site for a p~oposed on-site sewerage system.
using the above test, the observed minimum percolation rate
was 24 minutes per inch. No water table was observed during
drilling, but ~t should be noted that the free water level
normally fluctuates seasonally and with precipitation.

In conducting this evaluation, GEG, LtD. attempted to provide a thorough,
conscientious engineering analysis of the system in accordance with ADEC and MOA
DSD Guidelines & Regulations. The reported results described the performance of the
system under the conditions encountered at the time of the test, and separation
distances measured to readily identifiable features. The operational life of all wells and
septic systems depend on the local soils condition, groundwater levels that may
fluctuate during the year, and the water usage of the family being served by the system.
These conditions are outside the control of the evaluator of the system. Satisfactory test
results do not guarantee future performance of the system, nor do they guarantee that
there are no hidden defects or encroachments. GEG, LTD. can therefore not provide
any warranty or future estimate dhow long the system will continue to meet the
operational requirements of the ADEC or MQA DSD. The content of this report is for
the sole benefit of the owner listed above. Any reliance upon or use of this report by any

During a recent COSA on-site inspection and test of the potable water
supply well on Block , Lot 7B of Ginami Hills subdivision, the well's
productivity was determined to be 0.56 gallons per minute. The minimum
well productivity required by this Department (AMC 15.55) for a 4-bedroom
residence is 0.41 gallons per minute. Although the subject well currently
exceeds this minimum requirement, all parties concerned are advised that the
production capacity of the well may fluctuate. Restriction of non-critical
water uses such as washing cars and watering lawns and gardens may be
required.
This advisory must be attached to all copies of the subject Certificate of On-
Site Systems Approval.

The deficiencies were as follows:
I would recommend you contact A+
1. The high water alarm is present but not functioning. This could caused by a bad float or some other
electrical problem.
2. The first septic tank pipe is cracked and missing a cap. Also, the foundation cleanout is flush with
grade and missing a cap. Both of these caps should be extended and fitted with watertight caps.
Feel free to forward this description to A+ so they can understand the scope of work.
Please notify us when these deficiencies are corrected so when can proceed. Thank you.

Following is a brief summary of the tests performed on your well

A four hour flow test was performed on the well. The test
indicates the well to be producing water at a rate of 3 gallons
per minute.
ABSORPTION FIELD
The type of absorption system is a trench with a total
absorption area o 1074 sqft. The soils rating for the system at
the time of construction was 250 sqft per bedroom. The system
accepted water at approximately this rate during the test.

SERVICE CONTRACT
MULTI-~LO WASTEWATER TR.EAT~NT PK~NT
and between the user of a multt-flo waste water plant and Arctic Chain Plumbing
& Heating Inc. the servicer,
WITN~S:
THIS AGREE~NT is executed concurrently by the user and the dealer service
manager at the time of installation. The dealer covenants and agrees to furnish
twenty-four (24) months service on the user's waste treatment plant fromdate
of installation. The purchaser-user covenants and agrees by the presents to,
and does hereby contract with Arctic Chain Plumbing and Heating Inc to service
the user's waste treatment plant upon expiration of the dealer's aforesaid
free service period of twenty-four (24) months. The user understauds by
signing this agreement that the user accepts responsibility for the service
after the dealer free service period, and also understands the necessity and
responsibility for the service requirements that are advised by the dealer.

This service guarantees to make two (2) service and inspection calls per year
at regular intervals and to respond promptly to any emergency calls for repair
service. Unscheduled emergency or service calls not due to malfunction of or
defective sewage treatment units shall be paid for by the sewage treatment
plant owner~ Although the regular service and inspection calls 'during the
first twenty-four (24) months of installation will be free to the sewage
treatment plant owners, the sewage treatment plant owner shall be required
to pay the Service Contractor's road time for travel on an average scale
computed by averaging road time costs for the number of plant customers in
the area of the service call.
Inspection service calls will include the following:
1. An effluent quality inspection consisting of a visual check for color
and examination for odors.
2. Adjustment and servicing of any mechanical and electrical components
that are out of order·
3. Ba.ckwashi. ng of filter media or replacement of filter media with pre*
vio~ly washed filters.
4° Periodic sampling, twice a year if necessary, of the effluent quality,
and of the settled solids in the aeration tank.
Any ~dditional work necessary will be done upon written authority from the
homeowner and at additional charge.
Service call cost $~OoOq per call plus travel charges, if any.
