Attachments u 0 � � m FA � } \ } ON-SITE WASTEWATER DISPOSAL SYSTEM CONSTRUCTION AND MATERIAL SPECIFICATIONS SUBJECT: LOT 1, BLOCK 1, GLEN EAGLE SUBDIVISION GENERAL: 1. The scope of this project includes the pumping, crushing and abandonment of the existing septic tank on the lot and ithe procurement and placement of a new 1,250 -gallon septic tank at the location shown on the site plan. Work also includes the placement of 2 post tank cleanouts and a new foundation cleanout if one does ;not currently exist. The existing septic tank must be disposed in accordance with Municipal and ADEC requirements. The new septic tank must be constructed a minimum of 100' from any surface water and 100' from the existing well on this and adjacent lots. A minimum of 4' of cover is required over the septic tank. In lieu of cover 2' of direct bury insulation and 2' of cover may be placed. 2. Construction shall be in accordance with the approved site plan, design drawings, Municipal Permit with any special provisions' or conditions, and all applicable State and Municipal Wastewater Disposal Regulations. 3. The Contractor shall be responsible for obtaining all underground utility locates and for the layout of the septic system and verification of the location of all lot lines. 4. Unless specifically agreed otherwise, the contractor shall be responsible for final grading areas subsequently depressed from soil settling. Property owner shall be responsible for revegetation' of affected areas unless specifically agreed otherwise. 5. Contractors installing wastewater disposal systems must be certified by the Municipal Department of Health and Human Services Ifor system installations. Owners installing their own systems must receive prior approval from D.H.H.S. before beginning system installation. SEPTIC TANK INSTALLATION 1. A new 1,250 -gallon septic tank must be procured from a certified septic tank manufacturer if the existing tank is found to be defective: 2. A septic tank is to be constructed by a certified septic tlank manufacturer. Construction shall include two 4" cleanouts for pumping access. 3. All standpipes on the septic tank shall extend a minimum of 12 inches above final grade. 5. Tanks installed without 4' of cover shall have a minimum of 2" of direct burial insulation. 6. A foundation cleanout shall be installed one to four feet from the building foundation. Two cleanouts are required between the tank and the drainfield. 7. Final grading over the tank shall be such that a positive slope exists away from the septic tank. DRAINFIELD CONSTRUCTION: - NOT REQUIRED 1. The drainfield shall be constructed to the dimensions shown on the design. The bottom of the trench shall be within 2" of level. 2. Distribution piping must be placed level with perforations down atop a level bed of drainfield rock. Rock should then be placed over the pipe to provide a minimum of 2" of cover. 3. A silt barrier or geotextile fabric must be placed between the drainfield rock and the natural soil backfill. 4. Monitor tubes must be 4" in diameter and installed at the locations shown on the design. The portion below ground must be perforated. 5. Contractor shall verify the septic tank and drainfield are a minimum 100' away from any private water wells in the area, 150' from a Class "C" Well or 200' from any community well. 6. Direct bury insulation must be placed over the distribution system if less than 3' of backfill depth is available. Finish grade over the trench must be mounded to prevent settlement or depressions. 7. Grade area surrounding the absorption trench to drain away. 8. A minimum 2' of accepting soil is required below the drainfield rock for a 5' wide trench. Contractor shall verify this condition prior; to placement of the rock. Septic tanks must be constructed by a municipally approved septic tank manufacturer. 2. The following pipe materials are approved for use in septic system installations in the Municipality of Anchorage: Cast Iron (perforated and solid), ASTM D3034 or P.V.C. (perforated and solid), ASTM F810 or H.D.P.E. (perforated, but not solid) and ASTM D2662 or A. B.S. (perforated and solid). 3. Insulation shall be at least 2" thick extruded direct burial polystyrene (Dow Chemical Co. Styrofoam HI or equal). 4. Septic tank inlets and outlets shall be fitted with watertight couplings (Caulder, Fernco, or equal). 5. A permeable geotextile fabric (Typar, Mirafi or equal) must be installed between the final drain rock layer and the native soil layer. 6. All drain rock shall be .5" to 2.5 in diameter with less than 3% passing the #200 sieve. INSPECTIONS: Municipal Ordinance requires a minimum of two inspections. These inspections must be conducted under the supervision of a professional engineer registered in the State of Alaska. The first inspection must be conducted after the excavation of trenches, beds or pits and before the installation of any gravel. A septic tank may be set in place, but may not be backfilled. The second inspection must be conducted after the placement of the geotextile fabric, gravel, distribution piping, standpipes, cleanouts and insulation. No backfill should be in place at the time of inspection. This situation is a violation of Anchorage Municipal Code (AMC) 15.65.020.A and 15.65.020.0 which read: A. No person may cause or permit any wastewater to be discharged or disposed of except into an on-site wastewater disposal system conforming to the standards of this Chapter and 18 AAC 72 or into a public sewer in a manner conforming to this Chapter and to the tariff and laws governing the utility operating the public sewer. C. A person may not cause or permit any wastewater to be discharged or disposed of on the surface of the ground or in such a manner that it may gain access to surface or ground water except in accordance with provisions of this Chapter or of Alaska Department of Environmental Conservation. This property must meet the requirements of AMC Wastewater Disposal Regulations. Chapter 15.65 by July 1, 1998 or legal proceedings may be initiated as provided by law. We went to the back door to inform the occupants of our presence and identity. We then proceeded to the location of the absorption field indicated on our septic systetn inspection report from 1986. The septic tank with lift station is located immediately adjacent to the driveway and the absorption field is just up a small incline. At the Southeast standpipe of the absorption field there was a pool of effluent leading to a steady trickle down the hill to the driveway and road ditch along The Sun Loft Drive. We then looked under the lid of the lift station and observed a trickle of effluent returning to the lift station. From these observations we concluded that the absorption field has failed and a portion of the effluent is surfacing and a portion draining back to the lift station. We infornied the tenant, Mr. Bill Fryers, that the septic tank needed to be pumped immediately and as frequently as necessary to keep effluent from surfacing at the drainfield. We returned to the site the afternoon of May 27 to take photographs and observe a lift station pump cycle. We removed the broken portion of standpipe and observed effluent approximately a foot below the surface. As the lift station began pumping the level of effluent in the field rose to the surface and spilled out of the hole. Effluent was constantly flowing back to the lift station. In conducting an adequacy test, I attempt to provide a thorough, conscientious engineering analysis of the system in accordance with MOA COSA guidelines and regulations. The operational life of all wells and septic systems depend on the local soil condition, ground water levels that may fluctuate during the year, and the water usage of the family being served by the system. These conditions are outside the control of the evaluator of this system.
