Electrical installation of power and control for sewage lift station.
The lift station was installed to the latest guidelines and the manufactures recommendations. Physical install used two
runs of UF cable for pump power and high limit controls. A remote high limit alarm was installed on a separate circuit.
Field terminations were in a j-box provided ,using silicone wire nuts. Cables are protected from damage above ground
using liquid tight flex conduit,appropriately grounded at the controller with grounding bushing.The function test of the
hi limit and pump were successfully completed.

Foundation to Septic Tank Waiver
Dear On Site Engineer:
The existing STEP tank on Lot 6, Block 2, Glen Eagle Subdivision was recently replaced with a
new tank. The decommissioning of the existing tank was difficult and construction challenges
resulted in placement of the septic tank 4.25' from the edge of the foundation wall. The vast
majority of the tank is more than 5' from the foundation and only northeastern edge of the tank
encroaches into the 5' setback. Since the separation distance is less than 5' a waiver is required
to meet Municipal Code. The house was originally constructed in 1977 and has stabilized on the
site since that time.
The attached drawing illustrates the septic tank and it location in relation to the house foundation.
The influence line of the footing and foundation when projected down at a 1:1 slope does not
encroach into the septic tank. We hereby request a waiver be issued allowing the septic tank to
be 4.25' from the edge of the foundation.

Septic System Design and Permit Application
Impacts to Adjacent Properties
Dear Onsite Services Engineer:
The 1,250 Gallon STEP Tank on Lot 6, Block 2, Glen Eagle Subdivision has failed and
must be replaced. We are proposing to replace the tank and lift station with a new
1 ,500-gallon STEP tank to serve the four-bedroom home on the lot. The existing 1 ,000
gallon concrete septic tank will also be decommissioned in accordance with Municipal
Code. We are therefore requesting a permit be issued for the construction of a new
tank to serve the home. The attached Site Plan and backup documentation identify the
location and configuration of the existing septic system and the location of the new
STEP tank. Also identified on the plans are the wells on adjacent lots. No conflicts
exist between the well and septic system on this lot and those on adjacent lots. The
existing tanks will be decommissioned in accordance with Municipal Code. The
drainage patterns on the property will be maintained after construction.
The ground surface on the lot is basically flat in the area of the new tank. The tank will
be placed near the same location as the existing tank at nearly the same elevation. It
will then be tied in to the existing service line. The tank will be constructed in
conformance with Municipal requirements.
If the tank is constructed in accordance with our design the following statements apply:
1. The tank, if constructed as designed, will have no adverse impact on the wells in
the area or those to be constructed in the future.
2. The tank, if constructed as designed, will have no adverse impact on existing
septic systems in the area or those to be constructed in the future.

3. The tank, if constructed as designed, will have no adverse impact on reserve
space, either surface or subsurface, on any lots located in the area.
4. The tank, if constructed as designed, will have no adverse impact on drainage

must be submitted within 30 days of construction completion.
When applying for a new permit, the fees are: $320.00 for an
on-site wastewater permit; $120.00 for a well permit and
$440.00 for a combined on-site wastewater and well permit.

Request you issue a permi% to upgrade the septic system
serving the five bedroom house on the referenced property.
A test hole was excavated and a percolation test performed
in the area of the proposed upgrade. The approximate
location of the test hole is located on the attached site
plan.
At the time of excavation water was encountered at 12 feet
in the test hole and after seven day ground water
monitoring, water was found at 12 feet.
We do not anticipate any adverse effects on neighboring
wells, septic systems or drainage patterns by the
installation of the proposed septic system.
Please expedite the issuance of this permit due to effluent
surfacing on the ground and presenting a health hazard.

GENERAL:
The scope of this project includes the installation of
a 1250 gallon wastewater S.T.E.P. system (septic tank)
and two pressurized leachfield trenches to serve the
five bedroom residence located on the referenced
property.
Construction shall be in accordance with the approved
site plan and design drawings, Municipal permit with
any special provisions or conditions, and all
applicable State and Municipal Wastewater Disposal
Regulations.
The contractor shall be responsible for obtaining any
necessary underground utility locates.
Unless specifically agreed otherwise, the property
owner shall be responsible for final grading areas
subsequently depressed from soil settling.
Contractors installing wastewater disposal systems
must be certified by the Municipal Health Department
for system installations. Owners installing their own
systems must also receive prior approval from the
Municipal Health Department.
SEPTIC TAN]( INSTALLATION:
A septic tank is to be constructed by a certified
septic tank manufacturer. Construction shall include
two 4" cleanouts for pumping access.
The septic tank shall be sufficiently bedded to
prevent settling or shifting of the tank.
Ail standpipes on the septic tank shall extend a
minimum of 12 inches above final grade.

Septic tanks installed with less than 4 ft. of cover shall
be insulated.
A foundation cleanout shall be installed one to four feet
from the building foundation. In the line between the tank
and the leachfield there shall be two adjacent cleanouts
(unless an effluent pumping system exists within the septic
tank). These cleanouts shall be located on undisturbed
soil not more than 10 ft. from .the tank. The first
cleanout, in line, shall be to clean toward the leachfield.
The second cleanout shall be to clean toward the septic
tank.
Final grading over the septic tank shall be such that a
positive slope exists away from the septic tank.
ABSORPTION TRENCH/DRAINFIELD INSTALLATION:
Excavate the proposed trench to the dimensions shown on the
design. The bottom of the excavation shall be within 2
inches of level. If the sidewalls of the excavation become
smeared, they must be raked or scratched (ruffed-up) before
gravel (sewer rock) placement.

Once the gravel is installed, the distribution pipe is to
be installed level with the holes faced downward. The
distribution piping is to be of PVC (ASTM D3034 or equal).
All joints are to be solvent cemented. Gravel is then to
be placed over the distribution pipe 'to provide a minimum
of 2 inches of cover over the pipe.
A silt barrier must be installed between the final gravel
layer and the native soil backfill. Ensure the silt
barrier covers the entire gravel surface before placing
backfill.
Monitor tubes shall be of four (4) inch diameter,
installed approximately in the locations shown on the
design, and extend a minimum of 12 inches above final
grade. The portion of the monitoring tube extending
through the gravel shall be perforated from the bottom of
the trench to the invert of the distribution pipe. This is
equivalent to the effective depth of the gravel as noted on
the design.

Backfill over the final gravel layer must not be less 'than
twenty-four (24) inches. Insulation must be installed when
the backfill depth is less than thirty-six (36) inches.
The finish grade over the trench must be mounded to prevent
the formation of a depression after settling.
MINIMUM MATERIAL SPECIFICATIONS:
Any septic tank proposed for installation must be
constructed by a Municipally approved septic tank
manufacturer.

Use of a type of pipe other than listed above must be

Insulation shall be at least 2" thick extruded direct
burial polystyrene (Dow Chemical Company Styrofoam HI or
equal)·
Septic tank inlets and outlets shall be fitted with
watertight couplings (Caulder, Fernco, or equal).
A permeable nontoxic silt barrier (Typar 3401, Mirafi 140N,
or equal) must be installed between the final leachfield
gravel layer and the native soil backfill.
Ail[ leachfield gravel (sewer rock) shall be 0.5"-2.5"
screened gravel with less than 3% passing the #200 sieve.
When sand is being used as a filter material, its gradation
specifications must conform to current M.O.A. or D.E.C.
requirements.

INSPECTIONS:
Typically there will be a minimum of three (3) inspections
required during the installation of 'the wastewater disposal
system. These inspections will occur as follows:
The first inspection must be conducted after the
excavation of ditches, pits, trenches, or beds and
before the installation of any gravel. A septic tank
may be set in place, but may not be backfilled before
this inspection.
The second inspection must be conducted after the
placement of the silt barrier, gravel, distribution
lines, standpipes, cleanouts, and insulation, but
before the placement of any other backfill.
The final inspection is to occur upon final grading of
the property.
Often there will be more than these 3 inspections required.
Especially with the installation of multiple trenches, sand
filters, pressurized distribution systems, etc. Thus, 'the
inspecting engineer is to be contacted at least 24 hours prior
to the start of construction. If necessary, a pre-construction
meeting will take place on-site. The inspecting engineer will
not coordinate, direct or control in any way the contractors
activities.
The owner shall contract with the contractor to perform the work
outlined J.n these specifications and plans and in accordance
with the attached M.O.A. permit. There will be no contractual
arrangement existing between the contractor and S & S
Engineering. S & S Engineering shall be the owner's
representative and will inspect the work as stated above to
document the contractors activities. Final acceptance of the
contractors work rests with the owner and the M.O.A.

S & S Engineering shall have no liability to the owner or to
others for acts or omissions of the contractor or any other
persons performing work on this project or the failure of the
contractor to carry out the work in accordance with these
construction documents. S & S Engineering's inspecting engineer
will not be responsible for the construction means, methods,
techniques, sequence, procedures or the safety precautions
incident to this project.

Reference: Lot 6~ Block 2; Glen Eagle Subdivision
This letter reports the findings and determinations of an inspection
of the individual on-site well and waste water disposal system
serving the single family dwelling located on 'the referenced property.
This inspection involves the checking of horizontal separations
between the existing well and the on-site waste water disposal system.
At the same time the well was checked to determine if grade around the
casing and the wiring from the casing was in compliance with existing
codes. A water sample was also collected from within the residence
at this time.
The water system consists of a six inch steel case wall drilled to a
depth of 135 feet with a static water level at sixty feet. The well
wires are in conduit and a sanitary sea]_ is ins-tailed on the top of
the casing. The grade around the casing'is sloped so water drains
away from the well. A representative water sample was collected
from the kitchen and submitted 'to Chem Lab of Alaska. The test
results fer total coliform bacteria were reported as sa'tisfactory.
A copy of this report is attached.
The waste water disposal system has clean outs installed on the
septic tank and seepage pit and are adequately sealed~ Records
provided by the occupant indicates that the septic tank has been
pumped annually over 'the last few years.

The Municipality of Anchorage Development Services Department (DSD) issues Certificates of On-Site Systems
Approval (COSA) based only upon the representations given in paragraph 4 by an independent professional civil
engineer registered in the State of Alaska. Certificates of On-Site Systems Approval are required for the transfer of
title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or water
supply system. DSD also issues COSAs upon request to homeowners. 'Certificates of On-Site Systems Approval
are valid for 90 days from the date of issue for properties served by a private or Class C well and may be reissued
with new water sample results, (Certificates may be reissued for a period of up to one year with valid water
samples.) Certificates are valid for one year for propedies served by Class A or B wells or a public water system.
The Municipality of Anchorage is not responsible for errors or omissions in the professional engineer's work.
