Septic System Design
Dear On-Site Services Engineer:
The septic tank on the subject property has passed its useful life and must be replaced. We are
submitting this application for a permit to construct a new septic tank. The attached site plan
identifies the location of the home and the proposed and existing septic tank sites. No conflicts
exist between this proposed tank location and any other well or septic system, whether on this lot
or adj acent lots.
The new septic tank will be placed near the same location as the existing tank. Storm water
drainage will not impact this site.
Wells on this and adj acent lots are shown. The new tank will be a minimum of 100’ from all wells
and surface water, 10’ from the house foundation and more than 5’ away from the absorption
trench. No surface water is within 100’.
Please refer to the attached plan sheet for the septic design. If this design is followed, there will be
no adverse impacts to adj acent properties.

Approval (HAA) based only upon the representations given in paragraph 4 by an independent professional
civil engineer registered in the State of Alaska. Certificates of Health Authority Approval are required for the
transfer of title (except between spouses) for properties served by a single-family on-site wastewater disposal
and/or water supply system. DSD also issues HAAs upon request to homeowners. Certificates of Health
Authority Approval are valid for 90 days from the date of issue for properties served by a private or Class C well
and may be reissued with new water sample results. (Certificates may be reissued for a period of up to one
year with valid water samples.) Certificates are valid for one year for properties served by Class A or B wells or
a public water system. The Municipality of Anchorage is not responsible for errors or omissions in the
professional engineer's work.

Engineer's Comments:
This investigation was completed in compliance with
ADEC and MOA regulations. The assessment of the
condition of the well and septic applies only to the
conditions as of the day tested. The flow and absorption
rates may change due to subsurface conditions that
may not be observed from the surface, changes in
land use, local soil characteristics, groundwater levels
that may fluctuate during the year and the water usage
of the family being served by the system. The operational
life of all well and septic systems are subject to these
various and dynamic characteristics and are outside the
control of the evaluator of the well and septic system.
Therefore, KND can not give any estimate of how long a
system will function satisfactory for current or future
occupants or can KND guarantee that no unseen
encroachments, deficiencies or discrepancies exist.

Gentlemen:
At a pre-test site visit, only two septic standpipes were present (one septic tank cleanout
and one cleanout at the end of the drainfield). On April 12, 2003 prior to testing the
septic system, monitoring tubes were installed in the drainfield. The effluent was found
at the bottom of the effective depth. Subsequently on April 15, 2003, we performed the
septic adequacy and well flow test. The system appears to be functioning properly.
Also during the site investigation, it was discovered that the house has a grinder
pump/basin on the first level and the foundation cleanout is located within the
foundation. The owner stated that he has the septic tank pumped semiannually. Given
the fact that the tank is only a single compartment, we would recommend that the next
property owner continue this maintenance schedule. Once we have received the as -
built survey of the property, we will submit it to you.

not going to have his yard torn up with backhoes, dozens, etc. and that

though at this point in time I pointed out to him what appeared to be
