Dear Mr. Chervan,
Our records indicate that you own an Intermittent Dosing Sand Filter Advanced Wastewater Treatment
System (AWWTS). An AWWTS is a wastewater disposal system that uses advanced technology to
provide a higher quality effluent than a conventional septic system.
In accordance with Anchorage Municipal Code (AMC) 15.65.360, owners of AWWTS are required to
sign an AWWTS Maintenance and Repair Agreement with the Municipality of Anchorage. A copy of
the agreement is attached for your review. Please sign the maintenance agreement and have notarized.
For your convenience, free notary service is available at our office located at 4700 Bragaw Street.
Please return the signed and notarized agreement to our office within 30 days of receipt of this letter.

Reference: Glacier View Heights Subdivision #4; Lot 1, Block 4,
The septic system on the subject property was designed by our firm and installed by the
homeowner (Paul Chervan) in 1997. Final documentation was not completed pending the
completion of the house. Unfortunately, the property owner did not contact us when the house
was completed and we forgot that the documentation had yet to be finalized and submitted to the
MOA. This oversight became apparent to us recently when we looked at the file in association
with another project. Following this discovery we immediately took action to as -built the system
and submit the records to your department. The final inspection was performed on 1/11/2005 at
which time the ISF was found to be dry and the compressor was running at normal pressure
levels. Attached is the final documentation for the well and septic system for the referenced
luestions, please contact us at 337-6179.

The subject permit, issued August 28, 1997 by this office for a
single family well and/or on-site wastewater system, has
expired as of August 28, 1998.
A new permit must be obtained from this office for a well
and/or on-site wastewater system NOT installed by the
expiration date.
If you have drilled the well, a well log must be sent to
this office for documentation of the installation and to
close the permit.
If a licensed Professional Engineer has inspected the
installation of the on-site wastewater system, the original
as -built inspection report must be sent to this office for
review, approval and documentation. All inspection reports
must be submitted within 30 days of construction completion.
When applying for a new permit, the fees are: $320.00 for an
on-site wastewater permit; $120.00 for a well permit and
$440.00 for a combined on-site wastewater and well permit.

To whom it may concern:
A sewer permit (SW970283) was issued on 9/2/97 to install the subject septic system. The
bottomless ISF, and the STEP tank were installed on 10/4/97 & 10/5/97. Currently there is no
well, or structure on the property. The owner intends to begin construction on the house next
spring. All that remains to be done is running the sewer service line, the air line (for the ISF),
install the compressor, and drill the well. The as -built drawings will be submitted next spring after

The subject property is currently undeveloped. The property owner is proposing to build a 4
bedroom house on the site, which will require the installation of a well and a septic system. This
lot had been previously evaluated by S&S Engineering, and it was determined that is was
unsuitable for an onsite septic system. The reason for this assessment was the fact that the
groundwater table was so high, that there was not 2 feet of unsaturated accepting soil. S&S went
on to recommend that a curtain drain be installeA, the soils re-evaluated. The property owner
installed a curtain drain along the north, east, and west property lines, which daylights near the
southeast corner of the property. It was constructed by the property owner, and Carl's
Excavating and is almost 20 feet deep, along the north property line. The location of the curtain
drain is shown on the site plan for this design. Several weeks after construction of the drain, I
performed percolation tests, and groundwater monitoring. Comments regarding soils evaluation,
and the proposed design are summarized as follows:
1. SOILS: Attached is a copy of the soils logs. The soil is primarily silty gravel overlain vyith
1.5 feet to 2.5 feet of clayey gravel, and 5 feet of peat/organics. On 5/25/97 no groundwater was
encountered to a depth of 11.2 feet in the west TH, and to a depth of 13.5 feet in the east TH. In
short, the curtain drain appears to have lowered the water table enough so that there is now
almost 7 feet of unsaturated accepting soil. The accepting soil stratum, GM soils at a depth of
approximately 6.5 to 7.5 feet, had a percolation rate of 12 to 48 minutes per inch.
2. SYSTEM DESIGN: Do to the topography of the lot, and the proposed house location, a lift
station will be necessary. Rather than install a conventional bed or trench system, we are
proposing to install a Bottomless Intermittent Sand Filter (BISF). The design criteria are
summarized as follows:
a. Percolation Rate: 12 to 48 minutes/inch
b. Allowable Application Rate for BSF: 2 gallons/day/ft2
c. Number of Bedrooms: 4
d. Design Flow: 600 gallons per day
e. Minimum Absorption Area: 300 ft2
f. Effective Depth of the pea gravel: 3 inches below the pipe
g. Width: 18 feet
h. Length: 20 feet.
i. Effective absorption area = 360 ft2 (>300 ft2 OK)
j. Air Supply: Thomas Industries, Model 5070, "Anchorage Tank".
k. Air Supply Line: "Wasteflow" emitterline, 1/2 inch I.D, "Anchorage Tank".
1. Sand Material: Central Paving Products "Winter Road Sand"
m. Pea Gravel: 100% passing 3/8" sieve, less than 20% passing the 1/4" sieve, and
less than 1% passing the #8 sieve.
We are proposing to excavate down to a depth of approximately 7 feet, place 6 inches of sand,
install the air supply line, and cover it with 1.5 feet of sand. On top of the sand, we will place 6
inches of 3/8 inch pea gravel, with the pressure laterals midway in the layer. We will use a
conventional STEP tank (Anchorage Tank), equipped with a programmable timer so that flow
will be intermittently dosed to the IST'.
3. SURFACE WATERS: There are no surface waters within 100 feet of the proposed septic
system, or the reserve site.
4. TOPOGRAPHY: The lot slopes downward, at approximately 10-15%, heading from
northeast to the southwest, and is generally flat from northwest heading southeast. There is a
mound of fill (peat/organics) in the north portion of the lot, which parallels Eagle Glacier Loop
Road. The slope coming off of this fill, down to the original grade is fairly steep (approximately
100% slope). Since the bottom of the new ISF will be excavated down into the original soils
(depth of 7 feet to 9 feet below top of fill), the slope created by the fill will not be a concern.
Based upon my site assessment, there does not appear to be any slope concerns.
