Ref: Proposed Septic System Upgrade for Granite View #1; Block 10, Lot 3
To whom it may concern:
The existing 3 -bedroom house is served by a private well and septic system. Per GEG inspection, the existing drainfield is in a state
of failure and must be upgraded. We are proposing to decommission the existing steel septic tank (34 years old), install a new 1500 -
gallon HDPE Greer tank and deep trench type drainfield. Installation of a 1500 -gallon tank will allow for an easier upgrade in the
future if an AWWTS is necessary. One testhole was excavated on the property and the drainfield is designed around its 30 -foot
radius. Comments regarding the design are summarized as follows:
1. SOILS: See attached 2019 testhole by Garness Engineering Group, Ltd. which shows, soil classification, percolation test and
groundwater monitoring. As can be seen on the attached soil log, the soils perked at approximately 1.2 min/inch, however due to the
existing soils information in MOA records, it is our opinion that a more conservative 0.8 gpd/FT2 application rating should be used.
2. DRAINFIELD DESIGN: See attached design drawing for drainfield specifications.
3. SURFACE WATERS: There are no surface water concerns.
4. TOPOGRAPHY: There are no topography concerns
5. WAIVERS: The proposed tank is approximately 80 feet from the existing well. Water samples were pulled on 10/1/2019 and we
have yet to receive the results. Once the results are provided to us, we will forward you a copy. We also request that your
department issue a variance from the drainfield to existing drainfield down to 10 feet (instead of twice the effective depth).
Additionally, we are requesting that your department issue a variance from the proposed drainfield to the drainfield to the south on
Lot 4. Per MOA records, the drainfield on Lot 4 is 30'+ from the lot line, however, it appears to be about 10 feet from the property
line. We also request your department issue a variance of 90' from the well on property to the proposed drainfield. Justifications for
granting these waviers are as follows:
• The existing well is uphill from the proposed septic tank. If effluent were to overflow it would not travel towards the well
head
• The existing house is between the proposed tank and drainfield; effluent would not travel towards the well head.
• The proposed tank will be HDPE. This is an improvement on the existing steel septic tank.
• Per the water samples in MOA records from the 2014 COSA, the nitrates were non-detectable.
• Only one drainfield will be in use at a time.
• The existing drainfield appears to enchroach to 80 feet from the existing well.
• Nitrate samples that were pulled on 10/1/2019 came back as non-detectable. The results are attached.
We are unaware of any adverse impacts this installation would have on adjacent wells or septic systems. If you have any questions,

Ref: Waiver Amendment Request for Granite View #1; Block 10, Lot 3
To whom it may concern:
Per the design, the contractor was required to have the south lot line flagged prior to
construction. Upon GEG arrival, the south lot line was not flagged. The contractor continued
installation at his own risk. As can be seen on the attached as -built survey by Elizabeth
Walatka, PLS, the drainfield is on property, however, it is closer than 1 foot to the property line
(see attached record drawings, survey and email). It should be noted that during construction
GEG confirmed that the cleanout shown on the south portion.of the trench is located at the very
end of the drainfield. In short, while it is extremely close, the drainfield appears to be on
property. We request that your department issue an amendment to the existing waiver
WR#OSV191089 from 1' to 0'. During construction GEG employee David Garness, P.E.
measured the separation distance from the cleanout for the new drainfield to the cleanout on the
southern lot (Granite View #1; Block 10, Lot 4 & 5N). It was greater than 12' which is the
required separation distance for drainfield to drainfield (6 feet and 5 feet of effective).
We are unaware of any adverse impacts this installation would have on adjacent wells or septic

Dear Susan,
We performed an adequacy test on the system located on the
referenced property in August of this year. At that time the
system completely led he_adequacy test and the septic tank
was exposed and ound to be deteriorates:` This was documented
in our letter to yo -ed—Au , 1985. In this letter
we had indicated that there was no suitable area on this property
that would permit a complete upgrade and therefore we requested
that you issue a permit for a holding tank (850551). After
the permit was issued, investigation found that a drain valve
had malfunctioned allowing a water softener to continue to
discharge water at the rate of 2 gallons per minute into the
septic system. This continuous flow was found to be the cause
of the drainfield saturation. The valve was replaced and the
system allowed to rest for approximately six weeks. An adequacy
test was again performed and it was found that the system can
be considered adequate for approximately two bedrooms. We
have determined that there is adequate room to upgrade the
system for one bedroom as shown on the attached sketch, therefore,
request you cancel the holding tank permit 850551 and issue
a permit to replace the existing septic tank and upgrade the
absorption area for one additional bedroom in accordance with
the attached application.

INSPECTION An adequacy test was performed on the existing
on-site
waste water disposal system. The results of this
adequacy test were unsatisfactory.
SYSTEM DESIGN
In accordance with Municipality of Anchorage records
-
the existing system was installed in 1971. Due
to the age of the system, the septic tank was exposed
to verify its integrity. The tank was found to
be full of holes and will require replacing. A
WELL INSPECTION soil test hole was to be excavated in the only area
&FLOW TEST
left on the property suitable for upgrading the
absorption area. During test hole excavation an
old abandoned absorption area was encountered.
SITE PLANS There is no suitable area on this property that
will permit upgrading of the on-site waste water
disposal system, therefore it is requested that
you issue a permit for a holding tank that can be

The nearest public sewer is now in construction
on the corner of South Circle and Little Creek Drive
SOIL TEST approximately 600 feet away. Therefore it is possible
that public sewer will be available for this property
in the near future.

In conducting this evaluation, GEG provided an engineering evaluation of the well and/or septic system
in accordance with the guidelines and regulations established by the Municipality of Anchorage and
industry practices. The reported results describe the condition of the system/s on the date/s of the
evaluation. Separation distances were measured to readily identifiable features. Hidden defects or
encroachments may exist that were not identified during the evaluation. The operational life of all wells
and septic systems depend upon a variety of variables, including but not limited to, soil conditions,
groundwater levels (that may fluctuate during the year), quality of construction (materials and
workmanship), and the water usage of the family utilizing the system/s. These conditions can vary, and
are outside the control of GEG. Satisfactory test results do not guarantee future performance of the
system/s; therefore, GEG makes no warranty (express or implied) regarding the future performance of
the well or septic system. GEG makes no representation whether an alternative well or septic system
can be installed on the property in the event either of the current systems fail to perform adequately in
the future. The content of this report is for the sole benefit of the person/party that retained GEG to

The Anchorage Water & Wastewater Utility received a request from your Representative
Anderson Engineering on December 10, 2009 in regards to an existing absorption trench
encroaching into the utility easement running north to south along the east property line of
GRANITE VIEW # 1 BLK 10 LT 3.

AWWU is currently not using this easement, and has no objection to the encroachment. AWWU
hereby issues this letter with stipulations to allow the encroachment of an existing absorption
trench, and by using this easement the property owner(s) agrees to the following:

\`• 3. Permit. Chugach hereby authorizes Permittee to continue the existence of the
Encroachment in its existing location subject to the covenants set forth in this permit.
4. Indemnification. Permittee shall indemnify and hold Chugach harmless from any
claims for personal injury, property damage or other loss arising in any way from the continuation of
the Encroachment.
5. No Expansion or Increase. Permittee shall not in any way expand or increase the
extent to which the Encroachment occupies any of the real property encumbered by the Easement
without the prior written consent of Chugach. Chugach has no obligation to provide such consent.
6. No Interest in Real Estate Acquired. Permittee hereby acknowledges that the
Encroachment and any additions thereto, even if in violation of this permit' has not and shall not in
the future give rise to any interest or estate inthe real property subject to the Easement or any other
real property. Without in any way limiting the foregoing, Permittee acknowledges that the
Encroachment is not adverse to any interest of Chugach and its continuation is entirely with the
permission of Chugach for purposes ofthe common law doctrine of adverse possession. This permit
creates no interest in the real property subject to the easement.
7. Modification of Uti! tv Facilities. If the electric utility facilities located within the
Easement are upgraded, added to, replaced or reconstructed, Permittee shall pay that portion of
Chugach's total costs for such modification which is reasonably attributable to accommodating or
preserving the Encroachment. If such payment is not made within thirty (30) days of written demand
for same, Chugach may terminate this permit in accordance with paragraph 9, below.
S. Covenants Running with the Land The obligations ofthe Permittee arising from this
permit shall be covenants running with the land which shall burden the Parcel and the Nrinittee's
interest in the land subject to the Easement and shall benefit the Easement.
9, Termination. Chugach may terminate this permit after the giving of thirty (30) days
written notice of such termination if.-

agreements, oral or written, betweenthe parties. No modifications, amendments, deletions, additions

Certificate of On -Site Systems Approval # 090447
A Certificate of On -Site Systems Approval inspection and test of potable
water was recently conducted on the well water supply on Block 10, Lot 3 of
Granite View #1 Subdivision. This inspection revealed an arsenic
concentration of 16 micrograms per liter (ug/L) for the property's well water
sample. The Environmental Protection Agency (EPA) has established a
maximum contaminant level (MCL) of 10.0 ug/L for public drinking water
systems. While private wells are not subject to this regulation, EPA
standards are based on existing health information and can therefore be used
to gauge the relative quality of water from private wells. Information on
arsenic is available from the On -Site Water and Wastewater Program
website (www.muni.ort'/onsite) or at 343-7904.
This advisory must be attached to all copies of the subject Certificate of On -
Site Systems Approval.

The Municipality of Anchorage Development Services Department (DSD) issues Certificates of OnSfte Systems
Approval (COSA) based only upon the representations given in paragraph 4 by an independent professional civil
engineer registered in the State of Alaska. Certificates of Onsite Systems Approval are required for the transfer of
title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or water
supply system. DSD also issues COSAs upon request to homeowners. Certificates of OnSfte Systems Approval
are valid for 90 days from the date of issue for properties served by a private or Class C well and may be reissued
with new water sample results. (Certificates may be reissued for a period of up to one year with valid water
samples.) Certificates are valid for one year for properties served by Class A or B wells or a public water system.
The Municipality of Anchorage is not responsible for errors or omissions in the professional engineers work.

based on procedures outlined in the Certificate of Onsite Systems Approval Guidelines for this application,
shows that the on-site water supply and/or wastewater disposal system is (are) safe, functional and adequate
for the number of bedrooms and type of structure indicated herein. I further verify that based on the information
obtained from the Municipality of Anchorage files and from my investigation and inspection, the on-site water

Utility Easement Encroachment
Dear Mr. Roth:
The septic system on Lot 3, Block 10, Granite View Subdivision No. 1 was originally
constructed in 1971 with a log crib located approximately 15' from the east property line. The
system was upgraded in 1984 with an undocumented trench which extended approximately 4'
into the utility easement at the east side of the lot. A new trench was then constructed in 1985
which connected to the undocumented trench at a location outside the easement. It appears the
only encroachment into the trench is by the undocumented absorption trench. The encroachment
is 5' wide by 5' deep. The monitor tube marking the end of the trench is no longer in place and
no aboveground encroachment is evident.
The attached drawing shows the approximate layout of the absorption system. A steel monitor
tube was placed in the system to allow testing of the system. The system includes a total of 853
square feet of effective absorption area. The elimination of the section which encroaches into the
easement would reduce the total to 833 square feet. The adequacy test performed on the
absorption system indicated it is capable of absorbing more than 450 gallons of water per day.
We are preparing requests for an encroachment permit into the utility easement. Unfortunately
receipt of the final permit will take more than a month. We therefore request a conditional
COSA be issued so the property can be transferred to the new owner while the encroachment
permit process continues.

Water was introduced into,the set,tic tank at a measured rate
and the water depth in the tank monitored. By adjusting the
rate of inflow a rate was found at which the welter level
remained stationary. This rate of flow was founf to be 2.5 gal.
per min. which is much greater than the required absorption
rate.
