1. An owner install letter needs to be submitted.
2. An upgrade due to an increase in bedrooms needs a reserve site located on the site plan. The
Engineer needs to do an additional percolation test prior to the construction of the septic field.
Please submit stamped and signed results with the As -built Inspection Report. If the results
require a design change, construction of the system will stop pending On -Site review and
approval.
3. AMC 15.65.060.A.d A subsurface disposal field may not be located less than two times
the depth of gravel below the level of the horizontal perforated pipe or ten feet, whichever
Is greater, from any existing or abandoned subsurface disposal field.

System Permit Application, 5 Bedroom Single Family Dwelling
In accordance with AMC 15.65.030, the following information is provided in support of an On-
site Wastewater Disposal System Permit Application for the subject property:
1. The subject property currently has a 3 bedroom single family dwelling with a well and a
documented on-site wastewater disposal system. The existing septic tank is a 1,000 -
gallon double compartment tank. The soils absorption system is a bed. The owner is re-
modeling and adding two bedrooms. This application is for a 5 bedroom single-family
dwelling.
2. The owner wishes to keep the existing system operational. This system will serve the
garage and a portion of a daylight basement. It was tested 3 years ago and was found to
be adequate.
3. The property lies is on Great Land Street in Chugiak, Alaska. The lot is in the shape of a
square, 300 ft by 300ft. It is Lots 2,3, & 4, Block 1, to the north, Lot 4, Block 2 to the east,
Lot 2, Block 2 to the west and Lots 10, 11, and 12, Block 2, to the south. All adjoining
lots, if developed, have onsite water and wastewater systems. While several of the lots
are developed, they do not appear to have documented systems. The engineer has
verified that no wells on adjoining lots are within 100 ft of any portion of the proposed
system on.this lot. See Figure 1.
4. The property generally slopes from south to north. It appears some leveling was
accomplished when the lot was developed. There is a 6 ft cut bank to the north, but does
not affect the proposed onsite system. The area further slopes to the north at a 3-8%
slope to Greatland Street. There are some areas where the slope exceeds 15% on the
front of the lot near the street, but these slopes do not affect the proposed soils
absorption area. The location of the soils test and percolation tests were in native soils.
See Figure 2.
5. The area, where the replacement soils absorption system is proposed is located along the
east side of the lot. There appears to be sufficient area for at least two replacement
systems between the parking area along the side of the dwelling and lot line on the east
site of the lot.

6. A new 1,500 -gallon septic tank will be installed and the existing septic tank and soils
absorption field will be left in-place.
7. The soils found during the soils test were silt, sandy gravel, and silty sandy gravel.
Ground water was not observed in the test holes, and in subsequent monitoring.
8. Percolation tests were performed in the silty sandy gravel. The silty sandy gravel
percolated at 4.39 and 5.96 minutes per inch, for an average of 5.18 minutes per inch.
Rounding up, the design will be based upon 6.0 minutes per inch. Per MOA guidance, the
silty sandy gravel will be loaded at 0.8 gal/ft2/day.
9. The silty sandy gravel layer extends from 3 feet below grade to the bottom of the test hole
at 15.0 feet and is 12.0 ft thick. The top of the soils absorption system will be at 4.0 feet
below grade. Since the bottom of the system must be 4.0 feet above ground water and
6.0 feet above bedrock or an impermeable layer, the bottom of the system will be at 9.0
feet below grade. This provides a usable depth of 5.0 feet. See Soils Log.
10.The system will be a trench. The bottom of the trench will be 5.0 feet below the
distribution pipe. 5 br x 150 gpd/br = 750 gal/day, 750 gpd/0.8 gpd/sq ft 937.5 sq ft. Then
937.5 sq ft / (2 x 5) = 93.75 ft of trench. Using two legs, 93.75/2 = 46.8 ft.
11.There will be two trenches and will be at least 47.0 feet long. This will provide for a total
of at least 940 sq ft of absorption area. This will allow the bottom of the trench to be at
least 6 feet above an impermeable layer or bedrock and at least 4 feet above seasonally
high ground water.
12.There are wells and on-site absorption systems on some of the adjacent lots. There are
no wells, or surface waters within 100 feet of the proposed system on the subject lot or
adjacent lots. Due to the size of the lots in this subdivision, it does not appear the system
on this property will prevent development of the adjacent lots or the installation of
replacement systems on the adjacent lots.
The following are the percolation test results for two percolation tests in the vicinity of Test
Hole # 1. These percolation tests are referenced on the MOA Soils Log/Percolation Test
Form, dated 05/11/06. The test was performed between 4.0 and 4.5 feet below grade. The
test hole was pre-soaked for 4 hours. The test was performed in accordance with all state
and municipal guidelines in effect on the date of the test.
