A permit issued by this Department for an individual well
and/or on-site sewer system has expired as of December 31,
1986.
Permits are issued on a calendar year basis by authority
of Municipal Ordinance. A new permit must be obtained from
this Department for any well and/or on-site sewer system not
installed by the expiration date.
If you have drilled the well, a well log needs to be sent to
this Department for documentatiofl of the installation and to
close the permit. ~
If a private engineer inspected the installation of the on-site
sewer system the original as-built inspection report (three part
form) must be sent to this office for review and approval, and
for documentation.

Dear Ms. Oswalt,
This letter is written to notify your department that
Frontier Engineers Inc. has examined the above referenced lot and
finds that this lot is not capable of supporting an on-site
septic system due to the soil conditions and other property
constraints. We would therefore request a permit for a 3000
gallon holding tank for this property.
The soil strata consists primarily of moist impervious silt
with some trace of sand. Two open hole investigations were
performed and a perc test was attempted with the result as shown
on the ~attached test log. The blue gray silty soil underlying
the topsoil and fill material was consistent In texture and
nature. Some moisture was evident at varying locations withln
the silt in both holes. The density and moisture of the silty
soil precludes any septic system in this soil lens.
The fill material overlying the blue gray silty soil is
inconsistent ranging from peat and stumps to cobbly silt and
varying in thickness. The inconsistency of this fill material
and the topography of the area around this lot precludes use of a
mound system.
The septic system that had been serving this lot and house
has recently been found to be inadequate and In violation of DHEP
requirements. In order to bring this residence back into
compliance we are herewith making permit application for a 3000
gallon holdlng tank. Attached is a plot plan showing the
residence, lot dimensions and wells on adjacent properties. We
would request a permit to Install this holding tank as soon as
possible in order to minimize any future problems with the
existing system.

Dear Mr. Strode,
On May 17, 1986 the septic system on Lot 8 Blk. 2 Grecian
Hills was determined to be in noncompliance with A~C
15.65.020. Following notification of this you obtained a
permit for the installation of a 3,000 gallon holding tank.
The permit was issued on August 5, 1986; on January 9, 1987
a letter was sent to you stating that the permit had
expired as of December 31, 1986.
To date this office has received no confirmation of the
holding tank having been installed or that the other
requirements of the permit have been met. As you are aware
the requirements of the permit must be met and an as-built
stamped and signed by a registered civil engineer must be
submitted to our office.
since this office has not received such confirmation and a
visit to the subject lot on July 28, 1987 indicated that
some work had occurred the following shall apply:
1) You have ten (10) days from the date of this
letter to comply with the requirements of the
permit. A new permit is required for any work
performed in 1987.
2) The as-built submitted be completed by a civil
engineer registered in the State of Alaska.
3) Failure to comply with the above will result in
the issuance of a citation to AMC 15.05.080;
15.65.020; 15.65.040; and 15.65.090.

Please be advised that this firm should no longer be contacted
regarding the health violations on Lot 8, Block 2, Grecian Hills
Subdivision. This firm designed an acceptable system for waste-
water control on this lot approximately August 1, 1986. The
design was approved and a permit for construction was issued.
~S~bseqUent to that time, a system was installed by the owner.
However, this firm cannot verify construction compliance with
the approved design. If the owners' ability at construction is
as poor as their ability to pay for engineering services, I can
only surmise that the installed system is questionable.
We bring this system to your attention inasmuchas the Grecian
Hills Subdivision is a problem area within Anchorage, and this
lot in particular had been a violation for some time prior to our
work for the owners. We have notified the owners numerous times
in reference to the completion of the system, but we have received
no contact from them regarding their obligations.
We do not wish to burden you with a problem lot, but inasmuchas
we have no enforcement ability ourselves, we must at this time
remove our firm from this system design and request that the
Department of Health & Human Services finalize this system.
The owners' names and phone number are listed on the permit.

5. STATEMENT OF INSPECTION BY ENGINEER
As certified by my seal affixed hereto and as of the validation date shown below, I verify that my investigation, based on procedures outlined
in the Certificate of On -Site Systems Approval Guidelines for this application, shows that the on-site water supply and/or wastewater
disposal system is (are) safe, functional and adequate for the number of bedrooms and type of structure indicated herein. I further verify that
based on the information obtained from the Municipality of Anchorage files and from my investigation and inspection, the on-site water supply
and/or wastewater disposal system is(are) in compliance with all applicable Municipal and State codes, ordinances, and regulations in effect at
the time of installation.
In conducting an adequacy test, I attempt to provide a thorough, conscientious engineering analysis of the system in accordance with MoA COSA
guidelines and regulations. The reported results describe the performance of the system under the conditions encountered at the time of the test,
and separation distances measured to readily identifiable features. The operational life of all wells and septic systems depend on the local soil
condition, ground water levels that may fluctuate during the year, and the water usage of the family being served by the system. These conditions
are outside the control of the evaluator of this system. All systems eventually fail and satisfactory test results do not guarantee future
performance of the system, nor do they guarantee that there are no hidden defects or encroachments. Therefore we cannot provide any warranty
for future performance, nor can we estimate remaining life of the system. The content of this report is for the sole benefit of the owner listed
above.
