During the construction a new sewer service line was installed to from the new construction to the existing private sewer system. It is assumed now that the sewer service line has an approximate separation distance of 15 feet to the existing well. According to the owner (Nathan Gubser), he had a meeting with MOA employees; Jay Crewdson, Jeff Poet, & Deb Wockenfuss in which they stated that a possible waiver could be issued if he had; • A letter from the installer/contractor stating that the sewer service line was glued at all joints (see attached letter from Curt Whitters). • The pitless adapter needs to be sealed with bentonite (see attach quote to perform work). At this time we request that your department issue a 15' waiver from the well to the sewer service line. The new property owner would like to install a conventional septic system and is willing to abandon a portion of the round -about driveway in order to achieve this. We are now proposing to install a 1250 gallon septic tank and a dual 5 -wide trench type drainfields. One test hole was excavated on the property previously and a second confirmation test hole was recently excavated which we were not present for. The excavator did however install a monitoring tube to a depth of 13.5 feet and left the hole benched to a depth of 6 feet. The stockpile was inspected and it appeared to reflect the same as TH #1. The drainfield will be designed around the 30 foot radii of these test holes. Comments regarding the design are summarized as follows: 1. SOILS: See the attached logs which show the soil classifications, groundwater monitoring, and the percolation test results. 2. DRAINFIELD DESIGN: See attached design drawing for drainfield specifications. 3. SURFACE WATERS: There are no surface waters within 100 feet of the proposed septic system. 4. TOPOGRAPHY: As can be seen on the attached design drawing the average topography in the area below the drainfield is approximately 5-10% running south to north. In short, there are no slope concerns with this installation. 5. LOT LINE WAIVER: Due to site constraints, we are requesting a 01+ lot line waiver from the north lot line to the proposed drainfield. There are no neighboring drainfields with in 10 feet of the proposed waiver. We are unaware of any adverse impacts this installation would have on adjacent wells or septic systems. We are proposing to install a private well. The existing septic system is undocumented and needs to be upgraded. We are proposing to install a 1500 gallon S.T.E.P. tank with Quanics controls/floats, a 650 Aerocell unit, and a 5 -wide trench type drainfield. One test hole was excavated on the property. The drainfield will be designed around the 30 foot radius of the test hole. Comments regarding the design are summarized as fol lows: 1. SOILS: Sec the attached logs which show the soil classifications, groundwater monitoring, and the percolation test results. 2. DRAINFIELD DESIGN: See attached design drawing for drainfield specifications. 3. SURFACE WATERS: There are no surface waters within 100 feet of the proposed septic system. 4. TOPOGRAPIIY: As can be seen on the attached design drawing the average topography in the area below the drainfield is approximately 5-10% running south to north. Owner agrees not to make any alterations, removal of pans or additions to the AWWTS without a Construction Permit from the Municipality. 6. Maintenance and Rerrairs. A. Throughout the term of this Memorandum of Understanding. the Owner shall maintain AWWTS in good repair. In addition, it shall be the responsibility of'the Owner during the term of this Memorandum of Understanding, and any extensions or renewals thereof, at the owner's sole expense, to pay for any and all: (J) repair(s), (2) maintenance, (3) adjustment(s). (4) replacement costs, and (5) inspection costs. Further, Owner agrees to comply will all applicable ordinance, laws, regulations. rules and orders for the AWWTS. B. Owner agrees to provide the Municipality a written schedule of routine maintenance and repairs which have been performed on the system pursuant to the terms and conditions contained in the Owner's AWWTS Operating Permit This schedule shall he submitted to the Municipality annually upon the renewal of the permit. (GEG) waiver request letter, addressed to your department, and dated 6/26/13, the homeowner worked with a contractor to expose the pitless adapter on the existing well to inspect the presence/absence of a bentonite seal. During the excavation around the well, the owner and contractor experienced dangerous wet sloughing soils that began to slough towards the house. When the sloughing had come within approximately 5' of the house foundation, the owner and contractor made a decision to backfill the excavation to protect the house foundation. It is reasonable to believe that without major excavation which may include excessive trenching, sheet piling, and site drainage work, the pitless cannot be inspected using conventional methods (see attachment from ARM Services). It was then our recommendation that the pitless be inspected from inside the well, to inspect for any leaks. Being that the pitless is currently sitting in groundwater, an inspection of the pitless (if found to be dry) would prove that the seal is free of leaks. After GEG discussed the project with Jay Crewdson, the owner hired Cole Sullivan who pumped down the static water level, and inspected the pitless via camera. The inspection reflected that the pitless is in fact not leaking (see attachment from Sullivan Water Wells). The house has been occupied for several months, utilizing the existing sewer line. It should be noted that GEG pulled water samples on 6/12/13, and the Coliform sample results reflected 0 colonies/100 ml. We request that w.,'ver requested on 6/26/13 be granted at this time. Tudor Road Anchorage AK, 99507 Reference: 8182 Stewart Mountain Drive — Well Pitless To Whom It May Concern, On 6.29.13, ARM Services, LLC (ARM) attempted to expose the pitless adapter on the existing well located at the referenced property, to inspect the bentonite seal, or install new bentonite if required. During the attempt, the closest edge of our excavation (9.5' deep) did not extend past the South side of the well casing (towards the house), when wet sloughing soils were encountered. Over the course of the next 45 minutes, soils began to slough off moving rapidly towards the house foundation, intensifying as time went on. When ARM and the home owner became uncomfortable with the sloughing soils near the foundation, a decision was made to quickly backfill the excavation to stop further sloughing. At this time it is our understanding that the pitless on the well cannot be safely exposed or inspected, based upon the location of the well, without the completion of extensive drainage work on the property ($15,000-$20,000 rough estimate), or the installation of sheet piling to protect the house foundation (price unknown). It is our recommendation that a Municipal certified pump and well company (such as Sullivan Water Wells) is hired to inspect the pitless from the inside of the well with a camera. Since we know that the pitless is currently sitting in groundwater (at a depth of approximately 11'+), an inspection of the pitless from the inside of the well should negate the need for inspection via excavation. These items shall be stockpiled onsite. • Exporting of excess soils • Replacing of items such as sidewalks, fences, decks, sheds and other similar items removed as part of this work • Replacement or repair of unmarked private residential utilities (such as buried power/gas lines between buildings, not located by the locate services) • Prep work required prior to topsoil and seeding and or paving Typically, there is excess soil left over that was displaced by items such as the septic tank and drainrock. This soil will be spread out onsite as required to achieve the proper burial depth of all components, and to achieve positive drainage (rain, melting snow) away from the septic system. This will likely change the original landscape of the yard. The staging location for all imported soils, sewer rock or other materials required for this job will be chosen at the discretion of the contractor unless specified otherwise by the owner in writing. Every attempt shall be made to choose a location that will minimize damage to existing landscaping or driveways and parking surfaces; however in some cases it may be necessary to stage the imported materials (i.e. sewer rock, septic sand or other materials) on paved/unpaved driveway surfaces or landscaped areas. This contract does not include any site cleanup or restoration of the areas damaged by material staging. Large dump trucks and other heavy equipment can have a detrimental effect on paved surfaces. It is possible through the process of moving materials and equipment on site, that graveled or asphalt surfaces may be damaged. The majority of the time, septic systems are installed without any issues or design changes; however, it's important to note that unforeseen issues can arise, dictating the need for a design change or a change order. Such unforeseen issues include but are not limited to: • Negative or insufficient grade from the structure to the septic system, which may require excessive excavation to acquire grade, dictating the need for a lift station. • Changes in groundwater or soil conditions that dictate design changes. • Unforeseen subsurface conditions such as large boulders, shallow bedrock or excessive sloughing of soils (which require use of excess quantities of drain rock, or the use of trench boxes to protect workers). • Differing soil types and/or groundwater conditions which dictate changes to the design.
