Box 19-6650 Anchorage, AK 99519 Dear Sirs: Per our request you recently issued permit # SW090105 to upgrade an existing ISF on Lot 9, Block 8, Trails End S/D, located at 11440 Browder Avenue. We are now requesting reissuance of that permit to allow installation of a holding tank system instead. In preparation for the upgrade work the contractor excavated down inside the bottomless ISF to allow surcharged effluent to be pumped. Instead of there being a biomat formed on the top of the sand as anticipated, no biomat was in evidence, but rather the entire column of ISF sand was saturated with effluent. This indicates that the reason the system was failing was that the native soils underlying the bottomless ISF are not able to accept water at the design rate. Apparently the soils test results used in the design of the original system did not conform to the actual conditions encountered on the lot. To the extent that it was able to accept effluent at all, the ISF appeared to be functioning primarily by effluent migrating over the top of the plywood ISF box into the imported cover material. During the winter time when the cover material froze, untreated effluent tended to daylight and flow towards the house on the surface of the ground. That being the case, the proposed upgrade to an Advantex system would not work either, because it could not change the underlying problem of impermeable native soils. At this point the only viable option seems to be to install a holding tank to serve this property. Accordingly, I have drawn up the enclosed plans and specifications for installation of a 2000 -gallon holding tank to be located downstream of the existing 1300 -gallon polyethylene septic tank, giving an effective total holding tank capacity of 3300 gallons. 1 am requesting that you re -issue the permit to reflect the proposed change. T- attop?echnica[Services 14530 Echo Street, Anchorage, A X 99516 Thone (907) 345-13SS Lot 9, Block 8, Trails End S/D 11440 Browder Avenue Ilolding tank installation Specifications 1.0 General: 1.1 The scope of the project consists of abandonment of an existing lift station and bottomless ISF and installation of a 2000 -gallon holding tank with a high-level alarm downstream of an existing 1300 -gallon polyethylene septic tank. 1.2 Construction shall be as depicted on the approved site plan. Minor deviations may be allowed or required by the engineer conducting the inspections. All construction procedures and material specifications shall conform to Municipal and State requirements. All separation distances shall be in conformance with Municipal requirements, unless specifically waived. 1.3 The contractor shall be responsible to obtain any necessary utility locates, and to work around any buried utilities. 1.4 The contractor shall provide adequate cover material and rough grading over all system components to ensure that proper drainage is achieved after settlement and that there are no residual depressions. Insofar as possible the contractor shall minimize damage to trees and existing lawn areas. 1.5 Unless specifically agreed otherwise, the homeowner shall be responsible for finish grading after the soil is compacted, as well as placement of topsoil and reseeding all areas disturbed by the construction. 2.0 Holding Tank: 2.1 The existing "Fat Albert" lift station must be properly abandoned by thoroughly pumping, removing the top and backfilling with soil, or by removing, crushing and burying elsewhere on the lot or disposal at the Municipal landfill. 2.2 The new 2000 -gallon Municipally approved holding tank shall be installed in the approximate location shown on the site plan. The tank shall be set level on undisturbed soil and buried a minimum of 51eet below final ground surface to prevent flotation. 2.3 All pipe connections to the tank shall be equipped with waterproof mechanical couplings. 2.4 The holding tank shall be equipped with a low voltage, high level float that triggers an approved visible and audible alarm mounted inside the residence. The float shall be positioned to trigger the alarm when the fluid depth is approximately 500 gallons of capacity remaining in the tank at the time the alarm first sounds. 2.5 The existing ISF shall be abandoned in place with excess soil cover over the ISF plywood box removed and used to provide additional fill over the holding tank 3.0 Inspections: 3.1 A total of 3 engineering inspections will be required during the course of the project: (1) initial stakeout with the contractor to establish the location of the system and to discuss the plans, specifications and construction procedures, (2) after the tank has been set level in the excavation and the piping connected and alarm installed, but prior to backfill, and (3) after final backfill and grading is complete. 3.2 The installer shall coordinate the timing of the inspections with the engineer sufficiently far in advance to ensure the availability of the engineer. Box 19-6650 Anchorage, AK 99519 Dear Sirs: June 18, 2009 14530 ECHO CANYON ROAD ANCHORAGE, AK 99516 The purpose of this letter is to provide the required design narrative in support of our application for a permit to upgrade the wastewater disposal facilities on Lot 9, Block 8, Trails End S/D, located at 11440 Browder Avenue. The existing bottomless ISF is no longer able to accept the wastewater flow coming from the residence at certain times of the year, and there is a problem with surfacing effluent flowing towards the adjacent house. We are proposing to replace the ISF with an Advantex ATS — and to construct the residual disposal bed within a portion of the existing ISF. As discussed with Jay Crewdson on 5/12/09, because the proposed residual disposal system will be constructed within the footprint of the existing ISF the original soils tests should apply and no additional waiver should be needed. A site plan, design drawings and construction specifications are enclosed for your review. The original soils test conducted by Dee high in 2001 showed a native soil percolation rate of 11 minutes per inch, and groundwater monitoring in August of 2002 after installation of a curtain drain showed groundwater at 5 feet. Based on this percolation rate a Category III ATS has an allowable soil application rate of 5 gpd/sq. ft. Using a somewhat more conservative application rate of 3 gpd/sq. ft. this 4 -bedroom residence would require (4 x 150)/3 = 200 square feet. To achieve this we propose to construct a 12 -foot wide by 19 -foot long residual disposal drainfield in the northern portion of the existing bottomless ISF enclosure, which has a basal area of 228 square feet. We propose to leave the existing 1300 -gallon polyethylene septic tank in place to provide additional primary pre-treatment prior to the Advantex system. The existing "Fat Albert" lift station will be abandoned in accordance with code. The Advantex system will consist of a standard 1500 -gallon steel Advantex tank paired with a single AX -20 treatment pod. Treated effluent will then flow into an "Orenco" pump basin from which it will be pumped up into the residual disposal bed. To construct the residual disposal bed we will remove the soil cover material, filter fabric, insulation, distribution piping, sewer gravel and top 12 inches of ISF sand from the northernmost 12 feet of the 19' x 19' bottomless ISF box. This should expose existing ISF sand free from any contamination. We will then place 18 inches of new carefully screened and washed ISF sand in the excavation to create a new sand surface 6 inches higher than the present sand surface. The washed sewer gravel and distribution piping will be constructed on that surface. This configuration will ensure that the effluent is distributed as far as possible away from the house foundation, while recognizing that the entire original ISF basal area will function for ultimate dispersal of effluent into the underlying soil. To provide additional protection against lateral migration of effluent towards the residence we propose to install a 4 -foot high by 50 -foot long impermeable "Miradrain" or equivalent barrier on the outside of the south side of the existing ISF box and extending further to the north and south as appropriate. The original natural topography in the area of proposed construction slopes down towards the west at approximately 15%. Soils testing and curtain drain construction on Lot 2 to the west of the subject lot has created some temporary surface water ponding. It is anticipated that this temporary ponding will be eliminated as that lot is developed, so there should not be a need for a surface water separation distance waiver. Because this is an advanced treatment system I am requesting the property owner to come in to your office in order to execute the required maintenance agreement. The filtered effluent is pumped up into a 12 -foot wide by 19 -foot long residual disposal bed located within the northern portion of an existing bottomless intermittent sand filter box. The contractor shall also arrange separately with the homeowner to install an impervious moisture barrier between the disposal bed and the residence. 1.2 Construction shall be as depicted on the approved site plan and design drawings. Minor deviations from these drawings may be allowed or required by the engineer conducting the inspections. All construction procedures and material specifications shall conform to Municipal and State requirements. All separation distances shall be in conformance with Municipal requirements, unless specifically waived. 1.3 The contractor shall provide adequate cover material and rough grading over all system components to ensure that proper drainage is achieved after settlement and that there are no residual depressions. Insofar as possible the contractor shall minimize damage to trees. 1.4 The homeowner shall be responsible for finish grading after the soil is compacted, as well as placement of 6 inches of topsoil and reseeding all areas disturbed by the construction. 2.0 "Advantex" AX -20 Advanced Treatment System: 2.1 The "Advantex" treatment unit major components include (1) a 1500 -gallon, 2 -compartment epoxy - coated steel tank, (2) an "Orenco" AX -20 filter pod, and (3) an "Orenco" fiberglass pump discharge vault. All components shall be Municipally approved and shall be set level on undisturbed soil and backfilled in accordance with Municipal requirements as well as Orenco's and the supplier's instructions. The existing 1300 -gallon polyethylene septic tank shall be left in place as part of the system upstream of the Advantex tank. The 550 gallon "Fat Albert" lift station tank shall be properly abandoned in accordance with code. 2.2 All pipe connections to the tank shall be equipped with waterproof mechanical couplings. A double cleanout shall be installed upstream of the Advantex tank. 2.3 The contractor shall be responsible to obtain the necessary Municipal electric permit and inspection, and shall provide a copy of the electrical inspector's report to the engineer overseeing the construction of the wastewater disposal system. 2.4 The second compartment of the tank shall be equipped with a watertight, insulated manhole riser and cover extending a minimum of 6" above final grade with positive surface drainage away from the manhole. The Advantex AX -20 treatment pod shall be installed over the tank with the cover extending a minimum of 3 inches a.g.l. The contractor shall be responsible for determining the necessary length of the manhole riser, and for properly positioning the Recirculating Splitter Valve (RSV) in the riser. 2.5 An "Orenco" package lift station shall be installed downstream of the treatment tank. The lift station manhole riser shall extend a minimum of 6 inches above final grade. 2.5 The control and alarm floats shall be set at the elevations specified by the supplier, and their proper operations shall be verified by the contractor. 2.6 The treatment unit shall be set up with a "VeriComm" remote monitoring system as supplied by the distributor, and the control panel for the unit shall be mounted at an approved location. 3.0 Residual soil absorption system: 3.1 The 12'x 19' residual soil absorption bed shall be constructed within the confines of the existing ISF plywood box - located in the northern portion of the box. Prior to the start of reconstruction the existing soil cover, insulation, filter fabric, distribution piping, gravel and 12 inches of sand shall be removed from the 12' by 19' northern portion of the ISF box. The exposed level sand surface shall be inspected to verify that it appears free of wastewater contaminants. 3.2 Next, 18 inches of M.O.A. approved ISF filter sand (Q% passing the # 100 sieve, <1% passing the #200 sieve) shall be placed in the excavation (as illustrated on the cross-section) to create a 12' by 19' level surface that is 6 inches higher than the existing ISF sand. The filter sand shall be compacted by foot as it is being placed. Next, a total of 0.8 feet of sewer gravel shall be placed on top of the sand filter material with the perforated distribution pipe laid level such that the pipe invert is no less than 0.5 feet above the bottom of the sewer gravel. Sewer gravel shall be 0.5" - 2.5" washed gravel. The distribution pips shall be fabricated from 1.25" diameter Schedule 40 PVC with 3/16" diameter holes drilled in the bottom at 18 -inch intervals. The three pipe runs shall be spaced 4 feet apart. 3.3 The monitor tube shall be of 4" diameter and installed in the location shown on the design drawings. The portion of the monitor tube extending through the sewer gravel shall be perforated. 3.4 Two inches of rigid insulation, filter fabric and 2 feet of soil cover shall be placed over the entire top surface of the sewer gravel. Fill slopes shall be no steeper than 3:1. The finish grading shall include 6 inches of topsoil and seeding to promote rapid revegetation of all areas disturbed by the construction. 3.5 The contractor shall also arrange separately with the homeowner to install a 50 -foot long by 4 -foot high impervious moisture barrier between the disposal bed and the residence. The moisture barrier shall be Miradrain or equivalent and shall be located along the south outside of the ISF plywood box and extending east and west as appropriate to best protect the north side of the house. 4.0 Inspections: 4.1 A minimum of 5 engineering inspections will be required during the course of the project: (1) initial stakeout with the contractor to establish the location of the system and to discuss the plans, specifications and construction procedures, (2) after the existing ISF material has been excavated to expose the infiltrative surface to ensure that it is level and at the right elevation, and conforms with the soil test information, (3) after the filter material has been placed, (4) after the sewer gravel is in place and the distribution pipe has been laid and connected up to the septic tank, but prior to placement of insulation or filter fabric, and (5) after rough backfill and grading is complete. The "Advantex" treatment unit requires one inspection after the tank is set level and the piping connected, but prior to backfill. This inspection may be incorporated with any of the above inspections. ADVANCED WASTEWATER TREATMENT SYSTEM MAINTENANCE AND REPAIR AGREEMENT MEMORANDUM OF UNDERSTANDING BETWEEN MUNICIPALITY OF ANCHORAGE AND THIS MEMORANDUM OF UNDERSTANDING made and entered into as of this Day of of 20_, by and between herein the "OWNER," and the Municipality of Anchorage, herein the "MUNICIPALITY." In consideration of the mutual covenants contained herein, the parties to this Memorandum of Understanding agree as follows: 1. ADVANCED WASTEWATER TREATMENT SYSTEMS. A. Alteration. Any change to the design or function of an AW WTS that Includes the installation or removal of any parts, components or pieces not included in the original construction permit and design. Prior to performing any alterations to an AWWTS the owner must obtain a Wastewater Disposal System Construction Permit from the Municipality pursuant to Anchorage Municipal Code (hereinafter, "AMC") 15.65. B. Certificate of On -Site Systems Approval. An approval by the Municipality of existing water and wastewater disposal systems given at the time of property sale and title transfer in accordance with AMC 15.65. These approvals certify that the systems are adequate for the homes that they support and meet the codes that were in place at the time of system construction. C. Damage. Any man-made or natural change in a system that would inhibit the system from performing as designed. D. Maintenance and Repair. The scheduled and as needed replacement of existing parts, components and pieces of an AWWTS that were included in the original design which would allow the AWWTS to continue to perform as designed. E. Permit (Construction) An On -Site Wastewater Disposal System Construction Permit as defined by AMC 15.65. F. Permit (Operating) An Advanced Wastewater Treatment System Operating Permit. An annual permit, issued by the Municipality, that allows the Owner to operate an AWWTS, upon meeting all the requirements of this agreement, the conditions of Operating Permit, the requirements of the On -Site Wastewater System Construction Permit and all relevant provisions of AMC 15.65 3. Fee. Owner shall pay to Municipality an annual fee of Z Gro ($_Q.00), payable on or before the issuance of the operating permit and annually thereafter. The annual fee is due on or before the anniversary date of the approval by the Municipality of installed system. 4. Term. The term of this Memorandum of Understanding shall be for the life of the nWIN&I The term begins on the date of approval by the Municipality of the installed system and shall continue while the AWWTS system is in use or operational or until the property is sold or title is transferred by owner and a new certificate of On -Site approval is issued to the new owner or transferee of the property. 5. Alterations, Installation and Removal of Additional Equipment. Owner agrees not to make any alterations, removal of parts or additions to the AWWTS without a Construction Permit from the Municipality. 6. Maintenance and Repairs. A. Throughout the term of this Memorandum of Understanding, the Owner shall maintain AWWTS in good repair. In addition, it shall be the responsibility of the Owner during the term of this Memorandum of Understanding, and any extensions or renewals thereof, at the owner's sole expense, to pay for any and all: (1) repair(s), (2) maintenance, (3) adjustment(s), (4) replacement costs, and (5) inspection costs. Further, Owner agrees to comply will all applicable ordinance, laws, regulations, rules and orders for the AWWTS. B. Owner agrees to provide the Municipality a written schedule of routine maintenance and repairs which have been performed on the system pursuant to the terms and conditions contained in the Owner's AWWTS Operating Permit. This schedule shall be submitted to the Municipality annually upon the renewal of the permit. The schedule of maintenance and repair contained in the Owner's AWWTS Operating Permit is: SC. r)I % — h u4s.� C. Owner acknowledges that the fine schedule for failing to maintain and repair an AWWTS are codified in AMC 14.60. D. Owner agrees that only maintenance, repair personnel certified by the Municipality will inspect and make any necessary maintenance, repairs or permitted alterations to the system. G. Owner agrees to grant the Municipality reasonable access to test and inspect the AW WTS upon 24 hours written notice. F. Owner agrees that any sale or transfer of title of the property will not occur without a new Certificate of On -Site Systems Approval. G. Owner agrees that the relevant provisions of the standard specification guidebook for AWWTS is the governing professional guidelines for the construction, maintenance and repair of the Owner's AW WTS. 7. Nonwaiver. The failure of either party at any time to enforce a provision of this Memorandum of Understanding shall in no way constitute a waiver of the provisions, nor in any way effect the validity of the Memorandum of Understanding or any part hereof, or the right of such party thereafter to enforce each and every provision hereof. 8. Amendment. A. This Memorandum of Understanding shall only be amended, modified or changed by a ATiting, executed by authorized representatives of the parties, with the same formality of this Memorandum of Understanding was executed and such writing shall be attached to this Memorandum of Understanding as an amendment. B. For the purposes of any amendment modification or change to the terms and conditions of this contract, the only authorized representatives of the parties are: Owner: Anchorage: Purchasing Officer C. Any attempt to amend, modify, or change this contract by either an unauthorized representative or unauthorized means shall be void. 9. Jurisdiction: Choice of Law. Any civil action arising from this Memorandum of Understanding shall be brought in the Superior Court for the Third Judicial District of the State of Alaska at Anchorage. The laws of the State of Alaska shall govern the rights and obligations of the parties under this Memorandum of Understanding. 10. Severability. Any provisions of this Memorandum of Understanding decreed invalid by a court of competent jurisdiction shall not invalidate the remaining provisions of the Memorandum of Understanding. General The work under this section consists of the performance of all operations pertaining to furnishing and installing the septic tank/lift station, air compressor and piping to the sand filter. B. Materials 1. Tank a. Septic tank shall be a two compartment tank of at least the minimum size indicated on the drawings and MOA approved. b. First compartment shall have a separate 4" pump -out riser. The second compartment shall have a separate 4" pump -out riser. 2. Manhole Riser or Lift Station Tani: •o a. An additional second compartment manhole riser shall be capable of being equipped with the following: 1. A junction (Nems 4x) box or equal, bonded or attached to the pump basin. 2. UL listed electrical cord grips, installed in the J -box. b. An additional tank (M.O.A. approved) may be provided for the effluent pumping assembly and the items in (a.) above. 3. Effluent Pumping Assembly, Controls, Alarm & Piping a. Effluent pumping assembly shall be of approved materials, including the following: 1. A 110 volt or equal U.L. Listed effluent pump. 2. Piping from the septic tank to the sand filter is to be 1 1/4" PVC schedule 40 solvent weld piping properly bedded as described in Materials Specifications and sloped to drain back to the lift station. Insulation board 2" thick and 2' wide centered over pipe is to be placed where soil cover is less than 4' depth over pipe. d. The air line from the house to the filter is to be HDPE arctic insulated pipe 3/4"-2" SDR - 11. The compressor will have a gauge (0-10 psi) and be installed in a heated area, crawl space, or garage. CONSTRUCTION SEQUENCE In order to insure proper installation, the system is recommended to be installed in the following order: 1. Sewer line from house to tank 11. Septic tank/lift station 111. Sand filter IV. Leachfield bed (if required) V. Final grading CONSTRUCTION 1. Construction Instruments Contractor shall provide instruments such as transits, or levels, for establishing proper elevations and levelness of pipe or materials. 2. Pipe Laying All pipe shall be laid with material free of large or sharp rocks and organic matter. Each section of pipe shall be handled carefully and placed accurately. The spigot end shall be fully inserted pointing in the direction of flow and glued in place to make a water tight seal. Each section of pipe shall be properly supported to insure true alignment and an invert which is smooth and free from roughness or irregularity. At all times, when work is not in progress, open ends of pipe and fittings shall be closed so that no undesirable substance will enter the end of the pipe or fittings. Native materials may be used as back fill material above the pipe, provided it is free of large rocks and organic matter. All lift stations shall be installed in accordance with the manufacturer's recommendations and standards. Electrical installation is to be in accordance with the National Electric Code (latest adopted edition) by a licensed electrical contractor, with M.O.A. inspection where required. If MOA inspection is not required, a licensed electrician is to provide a letter to the Engineer stating that the system meets or exceeds all applicable codes. 4. Excavation Excavate to proper depth to allow gravity flow, install tank level, tolerance 0.05 foot, on undisturbed or compacted classified sand fill material. Create a level sand surface over the tank and add 2" burial foam +35 PSI directly over tank. Mound unclassified soil over tank with a minimum of 2 feet of cover and anti -flotation weights that may be required in areas of shallow ground water. 5. Connections All connections to be as per MOA requirements, PVC 3034 piping with Caulder or equivalent couplings for inlet riser pipes. All tank connections are to be completely water tight. SAND FILTER The work under this section consists of the performance of all operations pertaining to famishing and installing the sand filter. A. materials 1. Sand Filter a. Liner (if required - not required for bottomless sand filter) - OSI Model LI or equivalent. 30 mil PVC cut and fabricated to provide water tight enclosure of filter sand volume. Seams to be permanent as per Manufacturer's recommendations. All connections, such as boot flanges, through sand filter to be permanently bonded to the liner. The Contractor shall provide a washed sand to meet ASTM C-33 Concrete Sand Specifications with the additional requirement that all but 1% of the materials passing the 4100 sieve be removed. All aand shall be approved for use by the Municipality of Anchorage (M.O.A.). If the sample does not meet specifications, all sand will be removed and replaced. c. The Contractor shall be responsible for providing pea gravel that shall be a washed material containing no fines (0% passing the #100 sieve) and a maximum size of 1/4" diameter. d. The Contractor shall provide the manifold assembly. The manifold assembly shall be of approved materials with orifice shields and flushing assembly. All drilled holes to be cleaned of burrs and pipes flushed, prior to assembly. Flushing valve to include riser to finish grade. Lateral distribution piping is to be laid level (@ ± 0.05'). e. Side walls and top of the filter box to be insulated with 2" direct burial polystyrene insulation. f. The Contractor shall be responsible for providing filter fabric. g. Air coil in sand filter to be geoflow WF164-24 150 LF placed 6" above bottom of sand. Construction of Sand Filter The sand filter construction is to follow procedures as outlined below, as applicable to this design. Pea gravel and filter sand are to be hand leveled to proper depth (tolerance within 0.05 feet) in filter, taking care not to allow mud or fine soil material into the filter or gravel layer. This would include any soil material picked up by excavation machinery buckets while loading specified material. The Contractor should estimate filter material volumes such that allowance is made for wastage of material in contact with the ground at site. The area over the sand filter to be finish graded with a minimum of 2 feet of cover and to prevent ponding of surface water run off. Side slopes shall not exceed 3:1. The Engineer will reject any sand or pea gravel that does not meet specifications or is contaminated in transit or while handling. If such material is placed in the filter, all soil material may be required to be removed and replaced with new material.
