Subject: Hideaway Forest Block 1 Lot 3
Drain Field Location Justification
This is a justification letter for the location of a newly installed reserve drain field installed on the subject property
on (12/27/2021). The drain field was installed outside of the thirty-foot (30') radius of all existing test hole locations
on the subject lot.
Three test holes were performed on this lot, west of the drain field location by Pannone Engineering Services, LLC on
O8/11/07, TH-1, TH-2 and TH-3. Bedrock was not encountered in these test holes. Groundwater was observed in the
test hole monitor tubes of TH-1 and TH-2 after 7-days at a depth of 6.5' and 6.0' respectively. A test hole was also
performed on this lot by Garness Engineering Group, Ltd. on 4/28/06 east of the new drain field installation. Bedrock
was not encountered in this test hole, and ground water was observed after 7-days in this test hole monitor tube at
a depth of 4.0' below grade on 05/04/2006. Soil logs for the test holes described above have been attached to this
letter for your reference.
The design of the new drain field used the slowest application rate and highest groundwater reading of all the
documented test holes performed on the property. It is the opinion of this office that the installation of this drain
field meets the requirements of AMC 15.65.210.C.Exception. Substantial information has been provided to the
department to document that the soil surrounding the drain field and groundwater elevations are similar.

Ladies and Gentlemen:
I am writing in rcsponsc to your comments dated September 18, 2008. I will respond in the same order they are
presented.
Comment: Please address Iht surface water located between the cut area and the driveway.
Response: Surface water is storm water caught in disturbed ground by the construction activity. This disturbed ground will
be graded to smooth to prevent storm water from accumulating.
Comment: The driveway must meet code. Please show the driveway and how it will meet thc 10% requirement.
Response: Driveway will be graded to meet grade. Fill will be added as necessary to meet code.
Comment: Per our previous conversation, please note the amount of fill required for thc septic area.
Response: I do not recall a conversation discussing the amount of fill required. Thc plans call for fill to bc placed. The
amount of fill required is to bc calculated by the contractor to mcct code.
Comment: After the "Review Only" drawings have been reviewed and you are notified that there arc no additional
comments, the "For Construction" drawing must be submitted before the permit is i~ucd. The permit won't be issued until
the two sets of drawings have been compared.
Response: Issued for Construction Drawings attached.
Comment: Thc entire subsurface disposal field and alternate is not within a 30 foot radius of a percolation test. The
Engineer needs to do an additional percolation test prior to the construction of the septic field. Please submit stamped
and signed results with the As-built Inspection Report. If the results require a design change, construction of the
system will stop pending On-Site review and approval.

Ladies and Gentlemen:
I am writing to request a modification to the existing well and septic permit be allowed. The original permit
called for the installation of an altemate system to be installed near the proposed house. The soils
encountered did not match those reflected on the soils log and the ground water was significantly higher that
indicated. Four additional test holes were excavated at the base of the hill. A single percolation test was
performed. The proposed systems will serve a new four-bedroom house. Currently the lot is developed. The
surrounding lots are served by private water systems and are over 100 feet from the proposed soil absorption
system. The proposed well will be located in the eastern portion of the lot, greater than 100 feet from the
proposed system and from the neighboring septic systems.
1. Soils. The soils were investigated during August of 2007. Ground water was verified on August 1 !,
2007. Ground water was found at 6.5 feet below ground level in the test holes. Bedrock was encountered in
test holes to a depth ofg.0 feet. It is my opinion that the overall soils appearance of the soils, an application
rate of 3 gallons/day/square feet should be used using advanced treatment system and gravity flow to the
drain field.

3. · Surface Water: There is no surface water within 100 feet of the proposed system. The proposed
systems will maintain at least 100 feet from all surface water and drainage ditches.
4. Topography: Lot 1 slopes to the north at approximately 10-15 percent based on a survey conducted
in 2007. The proposed installation will be located in the northern portion of the lot, greater than 50 feet
from slopes greater than 25%.
The proposed installation will not affect the future development of the surrounding or existing lots. There
are no wells or septic systems within 100 feet of the proposed septic location.

THIS MEMORANDUM OF UNDERSTANDING made and entered into as of this
herein the "OWNER;' and the Municipality of Anchorage, herein the
"MUNICIPALITY." In consideration of the mutual covenants contained herein, the
parties to this Memorandum of Understanding agree as follows:
1. ADVANCED WASTEWATER TREATMENT SYSTEMS. Municipality
grants permission to Owner to utilize and operate an Advanced Wastewater
Treatment System (AWWTS), described as ]~\l bd~ xl"~'~ located
2. Definitions.
A. Alteration. Any change to the design or function oran AWWTS that
Includes the installation or removal of any paris, components or pieces not
included in the original construction permit and design. Prior to
performing any alterations to an AWWTS the owner must obtain a
Wastewater Disposal System Construction Permit from the Municipal!ty
pursuant to Anchorage Municipal Code (hereinafter, "AMC") i 5.65.
B. Certificate of On-Site Systems Approval. An approval by the
Municipality of existing water and wastewater disposal systems given at
the time of property sale and title transfer in accordance with AMC 15.65.
These approvals certify that the systems are adequate for the homes that
they support and meet the codes that were in place at the time of system

C. Damage. Any man-made or natural change in a system that would inhibit
the system from performing as designed.
D. Mmntenance and Repair. The schedUled and as needed replacement of _.
existing parts, components and pieces of an AWWTS that were included in
the original design which would allow the AWWTS to continue to
perform as designed.
E. Permit (Construction) An On-Site Wastewater Disposal System
Construction Permit as defined by AMC 15.65.
F. Permit (Operating)' An Advanced Wastewater Treatment System
Operating Permit. -An annual peimit, issued by the Municipality,.that
allows the Owner to operate an AWWTS, upon meeting all the
requirements of this agreement, the conditions of Operating Permit, the
requirements of the On-Site Wastewater System Construction Permit and
all relevant provisions of AMC 15.65
3. Fee. Owner shall pay to Municipality an annual fee of
($_~.00), payable on or before the issuance of the operating permit and annually
thereafter. The annual fee is due on or before the anniversary date of the approval by the
Municipality of installed system.
4. Term. The term of this Memorandum of Understanding shall be for the life of the
AW WTS.
The term begins on the date of approval by the Municipality of the installed system and
shall continue while the AWWTS system is in use or operational or until the property is
sold or title is transferred by owner and a new certificate of On-Site approval is issued to
the new owner or transferee of the property.
5. AIterations~ Installation and Removal of Additional Equipment. Owner agrees
not to make any alterations, removal of parts or additions to the AWWTS without a
Construction Permit from the Municipality.
6. Maintenance and Repairs.
A. Throughout the term of this Memorandum of Understanding, the O~vner shall
maintain AWWTS in good repair. In addition, it shall be the responsibility of the
Owner during the term of this Memorandum of IJnderstanding, and any extensions or
renewals thereof, at the owner's sole expense, to pay for any and all: (1) repair(s), (2)
maintenance, (3) adjustment(s), (4) replacement costs, and (5) inspection costs.
Further, Owner agrees to comply will all applicable ordinance, laws, regulations,
rules and orders for the AWWTS.
B. Owner agrees to provide the Municipality a written schedule of routine
maintenance and repairs which have been performed on the system pursuant to the
terms and conditions contained in the Owner's AWWTS Operating Permit. This
schedule shall be submitted to the Municipality annually upon the renewal of the
permit. The schedule of maintenance and repair contained in the Owner's AWWTS
Operating Permit is: %t' ~:~t~fX.O, tt-~
C. Owner acknowledges that the fine schedule for failing to maintain and repair an
AWWTS are codified in AMC 14.60.
D. Owner agrees that only maintenance, repair personnel certified by the
Municipality will inspect and make any necessary maintenance, repairs or permitted
alterations to the system.
E. Owner agrees to grant the Municipality reasonable access to test and inspect the
AWWTS upon 24 hours written notice.
F. Owner agrees that any sale or transfer of title of the property will not occur
without a new Certificate of On-Site Systems Approval.
G. Owner agrees that the relevant provisions ofthe standard specification guidebook
for AWWTS is the governing professional guidelines for the construction,
maintenance and repair of the Owner's AWWTS.
7. Nonwalver. The failure ofeither party at any time to enforce a provision of this
Memorandum of Understanding shall in no way constitute a waiver of the provisions,
nor in any way effect the validity of the Memorandum of Understanding or any part
hereof, or the right of such party thereafter to enforce each and every provision
hereof.
8. Amendment.
A. This Memorandum of Understanding shall only be amended, modified or changed
by a writing, executed by authorized representatives of the parties, with the same
formality of this Memorandum of Understanding was executed and such writing shall
be attached to this Memorandum of Understanding as an amendment.
B. For the purposes .of any amendment modification or change to the terms and
conditions of this contract, the only authorized representatives of the parties are:

C. Any attempt to amend, modify, or change this contract by either an
unauthorized representative or unauthorized means shall be void.
9. ,Jurisdiction: Choice of Law. Any civil action arising from this Memorandum of
Understanding shall be brought in the Superior Court for the Third Judicial District of the
State of Alaska at Anchorage. The laws of the State of Alaska shall govern the fights and
obligations of the parties under this Memorandum of Understanding.
10. Severabilitv. Any provisions of this Memorandum of Understanding decreed
invalid by a court of competent jurisdiction shall not invalidate the remaining provisions
of the Memorandum of Understanding.

Anna McCormick has asked me to email you regarding some concerns you have with septic
system installation and/or foundation drain.
· The foundation drain will not be discharged in the wetland boundaries.
· Excavation will not occur in the wetland area. We have anticipated the fill over the septic
system and have allowed for slopes to not encroach upon the wetland area. It will be the
responsibility of the contractor to install the system in a fashion that the equipment will
not travel over the wetland area.
· Owner will address this issue.
· It will be the responsibility of the contractor to have the wetland boundary marked by a
registered land surveyor or the proper authority.
· All equipment will be stored/serviced outside of the wetland areas.
Please issue the wetland permit so we may proceed with the well and septic permit.

Sonnet,
Mrs. McCormick asked if I would look over their home plans for the
building permit and address the zoning note. She and her husband have
consulted with Thede and I on the best use of this site to avoid wetlands
and streams, and it appears they will achieve that. From the site plans I
was given (Plot Plan from Lantech. dated 3/15/07; and Well and Septic
plan from Garness, dated 2/26/07) the proposed Improvements can be
approved for the building permit provided the following:
· : The foundation drain and its outlet are not located within wetlands or the
stream setback.
· Toe of fill slope for home and septic are sufficiently separated from the
wetlands boundary and stream setback to prevent equipment from encroaching.
· No improvements to the existing driveway are proposed. Should additional fill
beyond driveway's existing footprint or replacement of culverts be needed, a Corps of
Engineers permit would be required.
· Contractor is to mark the stream setback with conspicuous fencing material or
flagging prior to construction.
· No equipment shall be serviced or stored within wetlands or the stream
