This is a revised submittal that addresses the comments and concerr~, brought up by Jay Crewd~on in
his review of the original submittal dated dune 20, 2009
The purpose of this letter is to provide the required design narrative in support of our application for a
permit to install water supply and wastewater disposal facilities to serve a 6-bedroom residence on the
NW ¼, NW ¼, NE ¼, Section 14, Ti IN, R3W, S.M., located on Finland Street in South Anchorage.
Soils logs, perc test results, a site plan, design drawings and construction specifications are enclosed for
your review.
Due to the large lot size (10 acres) DSD issued a land usc permit allowing construction work to start
early this spring prior to completion of soils testing and well and septic permitting. The building is now
under construction.
With the exception of the southeast comer, much ofthe lot is poorly drained. The only viable location
for construction of on-site wastewater disposal facilities is in this southeast comer oftbe lot, which is
also the area where the home is being built. On April 13, 2009 we conducted soils tests in what
appeared to be the most viable portion oft the remaining undeveloped area immediately north of the
building pad and just west ora 50-foot wide public use easement that parallels the eastern lot boundary.
The log for test hole #1 illustrates the soils that were encountered and percolation test results for two
different percolation tests are reported on separate copies ofthe test hole log. As can be seen from the
soils test information, there is a 2 ½ foot thick lens of sandy gravel in this location extending from
immediately below the surficial peat and loam at 1.5 feet down to the top ora damp gravelly silt lens
starting at a depth of 4 feet. The measured percolation rates for this sandy gravel lens were one or less
minutes per inch.
Although no groundwater was encountered at the time the test hole was excavated, subsequent
monitoring conducted on several occasions during the spring breakup period established a seasonal high
groundwater level at 3.8 feet below ground level. Surface water puddles were also observed on and
adjacent to a filled area located 50 feet west of the test hole.
Based on these test results and due to the extremely limited suitable area available, the only option is to
install a Category III advanced treatment wastewater system (ATS). Using a somewhat conservative
soil application rate of 4.0 gpd/square feet for the gravel stratum the required residual disposal area for a
six-bedroom residence is (6 x 150)/4 = 225 square feet, which can be achieved with a 10-foot wide by
22.5 foot long soil absorption bed. Due to the limited available area and the desire to maximize the
longevity of the system, we propose to construct the original and replacement systems at this time in the
form ora I O-foot wide by 45-foot long soil absorption bed.
To achieve the required 4-foot vertical separation from seasonal high groundwater we propose to
excavate down to a depth of 2.0 feet on the uphill side of the bed to expose the top of the grovel stratum
under the entire bed area. We will then place 2.2 + feet (as necessary) oflSF filter sand in the
excavation to achieve a level sand surface 0.5 feet above original ground level and build the residual
disposal field on top ofthat.
We are also specifying that the contractor construct a curtain drain located 50+ feet west of the residual
disposal bed that will eliminate all surface water within 100 feet of the system.
The topography in the area of proposed construction slopes downwards towards the northwest at 15%
until it meets the relatively flat filled area with scattered surface water puddles.
Because this is an advanced treatment system I am requesting the property owner to come in to your
office at 4700 Elmore Road in order to execute the required maintenance agreement. Please give me a

REPAIR AGREEMENT
MEMORANDUM OF UNDERSTANDING
BETWEEN MUNICIPALITY OF ANCIIORAGE
AND
THIS MEMORANDUM OF UNDERSTANDING made and entered into as of this

"MUNICIPALITY." In consideration of the mutual covenants contained herein, the
parties to this Memorandum of Llnderstanding agree as follows:
1. ADVANCED WASTEWATER TREATMENT SYSTEMS. Municipality
grants permission to O~vner to utilize and operate an Advanced Wastewater
Treatment System (AWWTS), described as /0r-)l---0' *X_ h -I/e .~ located

2. Definitions. A/ iL/Lic //./~' ~
A. Alteration. Any change to the design or function of an AWWTS that
Includes the installation or removal ofany parts, components or pieces not
included in the original 6onstruction permit and design. Prior to
performing any alterations to an AWWTS the owner must obtain a
Wastewater Disposal System Construction Permit from the Municipality
pursuant to Anchorage Municipal Code (hereinafter, "AMC") 15.65.
B. Certificate of On-Site Systems Approval. An approval by the
Municipality of existing water and waste~vater disposal systems given at
the time &property sale and title transfer in accordance with AMC 15.65.
These approvals certify that the systems are adequate for the homes that
they support and meet the codes that were in place at the time of system
construction.
C. Damage. Any man-made or natural change in a system that ~vould inhibit
the system from performing as designed.
D. Maintenance and Repair. The scheduled and as needed replacement of
existing parts, components and pieces of an AWWTS that were included in
the original design ~vhich ~vould allow the AWWTS to continue to
perform as designed.
E. Permit (Construction) An On-Site Wastewater Disposal System
Construction Permit as defined by AMC 15.65.
F. Permit (Operating) An Advanced WastewaterTreatment System
Operating Permit. An annual permit, issued by the Municipality, that
allows the O~vner to operate an AWWTS, upon meeting all the
requirements of this agreement, the conditions of Operating Permit, the
requirements of the On-Site Wastewater System Construction Permit and
all relevant provisions of AMC 15.65
3. Fee. Owner shall pay to Municipality an annual fee of
($ O.00), payable on or before the issuance of the operating permit and annually
thereafter. The annual fee is due on or before the anniversary date ofthe approval by the
Municipality of installed system.
4. Term. The term ofthis Memorandum of Understanding shall be for the life ofthe
AWWTS.
The term begins on the date ofapproval by the Municipality ofthe installed system and
shall continue while the AWWTS system is in use or operational or until the property is
sold or title is transferred by owner and a new certificate of On-Site approval is issued to
the new owner or transferee of the property.
5. Alterations, Installation and Removal of Additional Eqnipment. Owner agrees
not to make any alterations, removal of parts or additions to the AWWTS w~thout a
Construction Permit from the Municipality.
6. Maintenance and Repairs.
A. Throughout the term of this Memorandum of Understanding, the Owner shall
maintain AWWTS in good repair. In addition, it shall be the responsibility ofthe
Owner during the term of this Memorandum of Understanding, and any extensions or
renewals thereof, at the owner's sole expense, to pay for any and all: (l) repair(s), (2)
maintenance, (3) adjustment(s), (4) replacement costs, and (5) inspection costs.
Further, O~vner agrees to comply will all applicable ordinance, laws, regulations,
rules and orders for the AWWTS.
B. Owner agrees to provide the Municipality a ~vritten schedule of routine
maintenance and repairs which have been performed on the system pursuant to the
terms and conditions contained in the Owner's AWWTS Operating Permit. This
schedule shall be submitted to the Municipality annually upon the renewal ofthe
permit. The schedule ofmaintenance and repair contained in the Owner's AWWTS

C. Owner acknowledges that the fine schedule for failing to maintain and repair an
AWWTS are codified in AMC 14.60.
D. Owner agrees that only maintenance, repair personnel certified by the
Municipality will inspect and make any necessary maintenance, repairs or permitted
alterations to the system.
E. O~vner agrees to grant the Municipality reasonable access to test and inspect the
AWWTS upon 24 hours written notice.
F. Owner agrees that any sale or transfer of title ofthe property will not occur
without a new Certificate of On-Site Systems Approval.
G. Owner agrees that the relevant provisions ofthe standard specification guidebook
for AWWTS is the governing professional guidelines for the construction,
maintenance and repair ofthe Owner's AWWTS.
7. Nonwaiver. The failure of either party at any time to enforce a provision of this
Memorandum of Understanding shall in no way constitute a waiver of the provisions,
nor in any way effect the validity of the Memorandum of Understanding or any part
hereof, or the right of such party thereafter to enforce each and every provision
hereof.
8. Amendment.
A. This Memorandum of Understanding shall only be amended, modified or changed
by a writing, executed by authorized representatives of the parties, ~vith the same
formality of this Memorandum of Understanding ~vas executed and such ~vriting shall
be attached to this Memorandum of Understanding as an amendment.
B. For the purposes of any amendment modification or change to the terms and
conditions of this contract, the only authorized representatives of the parties are:

C. Any attempt to amend, modify, or change this contract by either an
unauthorized representative or unauthorized means shall be void.
9. Jurisdiction: Choice of l,aw. Any civil action arising from this Memorandum of
Understanding shall be brought in the Superior Court for the Third Judicial District of the
State of Alaska at Anchorage. The laws ofth¢ State of Alaska shall govern the fights and
obligations of the parties under this Memorandum of Understanding.
10. Severahilit¥. Any provisions of this Memorandum of Understanding &creed
invalid by a court of competent jurisdiction shall not invalidate the remaining provisions

I understand that Justin Green is in the process of appl)ing for a building permit to construct a residence
on the NW sA, lqW ¼, NE ~, Section 14, TI 1 N, R3W, S.M., located adjacent tO Finland Street in South
Anchorage. This property will need to be served by on-site water supply and wastewater disposal
systems, however it is not f-~asible to complete the neces:~a'y soils analysis and septic design ~vork until
spring breakup conditions can be observed. It is rccogui.:ed that there are some conccms in this general
ama regarding shallow groundwater, soil percolation rates and proximity to surface ~ter that will need
to be addressed. Given the large lot size (10.0 acres) and the option ofadvanccd waslewater treatment
systems (ATS) for lots with poor soil conditions, I am reasonably confident that it will be possible to
identify a suitable and sufficiently large area in the southwestern portion of the lot to allow the design of
an original and a replaccment wastewater disposal system. Of course, no guarantee can be provided as
to the soil suitability prior to completion of sobs testing work, so Mr. Green will necessarily be
proceeding at his own risk. Thc fact that Mr. Green also owns the adjoining Lot 8, Block 2 of Potter
Heights S/D, provides another potential site that could b~ used.
