Ref: Proposed Septic Upgrade for Seavicw Heights Subdivision; Lot 1, Block 2
To whom it may concern:
The existing 5 bedroom house is served by a private well and an undocumented septic system.
Five test holes were dug on the property. The drainfield will be designed around the 30 foot
radius of test hole #5. We are proposing that a new 2000 gallon S.T.E.P. tank and a pressurized
deep trench type drainfield be installed. Comments regarding the design are summarized as
follows:
1. SOILS: See the attached log which shows the soil classifications, groundwater monitoring,
and the percolation test results. It is our opinion that due to the overall appearance of the soils, an
application rate of 1.0 gallons/day/ft2 should be used.
2. TRENCH DESIGN:
a. Percolation Rate: 3.6 minutes/inch
b. Proposed Application Rate: 1.0 gallons/day/ft2
c. Number of Bedrooms: 5
d. Design Flow: 750 gallons per day
e. Minimum Absorption Area: 750 fie
f. Total Depth: 10 feet (max.)
g. Effective Depth: 7.5 feet
h. Width: 2.5 feet
i. Minimum Length: 50 feet long total
j. Effective absorption area = 750 ft2
Note: The distribution line is to be 1.25 inch SCI- 1. 40 PVC with V4 inch diameter
holes spaced 20 inches on center (30 holes total).
3. SURFACE WATERS: There arc no surface waters within 100+ feet of the proposed septic
system.

4. TOPOGRAPHY: The majority of the south half of the property has been filled with soil.
However, Testhole #5 was excavated in a low lying area which is believed to be undisturbed.
The proposed drainfield is to be installed in this "valley" and then filled with material so that
water will not pond over drainfield. In short, there are no slope concerns.

The Municipality of Anchorage Development Services Department (DSD) issues Certificates of On -Site Systems
Approval (COSA) based only upon the representations given in paragraph 4 by an independent professional civil
engineer registered in the State of Alaska. Certificates of On -Site Systems Approval are required for the transfer of
title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or water
supply system. DSD also issues COSAs upon request to homeowners. Certificates of On -Site Systems Approval
are valid for 90 days from the date of issue for properties served by a private or Class C well and may be reissued
with new water sample results. (Certificates may be reissued for a period of up to one year with valid water
samples.) Certificates are valid for one year for properties served by Class A or B wells or a public water system.
The Municipality of Anchorage is not responsible for errors or omissions in the professional engineer's work.

Certificate of On -Site Systems Approval # 090343
A Certificate of On -Site Systems Approval inspection and test of potable
water was recently conducted on the well water supply on Block 2, Lot 1B
of Seaview Heights Subdivision. This inspection revealed an arsenic
concentration of 17 micrograms per liter (ug/L) for the property's well water
sample. The Environmental Protection Agency (EPA) has established a
maximum contaminant level (MCL) of 10.0 ug/L for public drinking water
systems. While private wells are not subject to this regulation, EPA
standards are based on existing health information and can therefore be used
to gauge the relative quality of water from private wells. Information on
arsenic is available from the On -Site Water and Wastewater Program
website (www.muni.or onsite) or at 343-7904.
This advisory must be attached to all copies of the subject Certificate of On -
Site Systems Approval.

Ladies and Gentlemen:
With this submittal we are applying for a waiver of the 100 foot separation distance required between the
septic tank and a private well. During the COSA review process it was discovered that the existing well to
septic tank separation distance (measure from the center of the well to center of the 1" compartment septic
tank standpipe) was 99.2 feet.
A well log for the existing well could not be located. On Sept. 14th, 2009 the static water level was observed
at 113 feet below the top of the well casing. Steady pumping of 1000 gallons of water from the well at a
maximum out put of 5.3 gpm caused a draw down of 3 feet. Water samples collected from the exterior hose
faucet on September l 1", 2009. Water quality analysis for bacteria, nitrate and arsenic showed 0 coliform or
other bacteria per 100 ml, Below detection limits for nitrate -nitrogen, and 17 ug/l for Arsenic.
The existing septic system was installed in September of 2004. It consists of a 2000 gallon steel STEP septic
tank and a 50 foot long subsurface shallow absorption trench with 7.5 feet (effective depth) of sewer rock.
During an adequacy test, 1000 gallons was added to the septic system. Prior to the start of the test, the fluid
depth in the trench monitor was 0 inches and the maximum depth reached was 0 inches.
Soil logs indicate the following site conditions: a layer of organics; poorly graded gravel, silty sand, well
graded gravel and well graded sand. The total depth of the test hole was 16 feet. Ground water was not
observed during the excavation, nor during the monitoring period.
Well logs from several neighboring properties across Dimond Blvd. record the follow soil strata's:Gravely
sandy silts and silty sands. Several of the well logs record layers of clay below a depth of 125 to 150 feet.
The water bearing strata appears to be below 250 feet deep.

Mitigating Factors:
The well head is located up gradient from the septic tank. If the septic tank were to overflow, it would drain
away from the well head, making surface flow contamination from the septic system impossible.
The water bearing strata appears to be below a depth of 250 feet. Layers of clay are identified on some
neighboring well logs starting at a depth of 125 feet. The clay layer should protect the aquifer from
subsurface contamination from the septic system.
A camera was sent down the well casing and no perforations were observed to a depth of 4 a r.
The existing septic tank is only 5 years old and should be water tight. Total coli form and other bacteria were
not detected in the water sample collected from the well on the subject lot. A nitrate result of less than 0.0
mg/I was reported.
Waiver Request: 97 feet from septic tank to well.
Included for your review is a surveyed as -built showing the location of the existing drain field, the septic tank
and the well, soils logs conducted in 2004, well logs for Seaview Heights [ilk 1 Tract 1, lot 5, lot 6, and water
quality result from water samples collected on September I Vh, 2009.
