To whom it may concern:
This letter details the inspections performed on the septic system being installed on Lot 21 Block 2 of
Glenn View Estates West Phase 2 by Guaranteed Services. As required by this on-site permit an
additional percolation test was performed within the southern region of the absorption area prior to the
construction of the system. The October 15, 2009 test yielded a percolation rate of 26 min/in. The
associated application rate of 0.6 gpd/sf (Table 1 of Chapter 15.65) is the same value uscd in our original
design and confirmed the system was sized correctly.
On November 6, 2009 F. Robert Bell and Associates conducted two inspections and established the
southwest comer of the building foundation footing as a benchmark. On the first visit our inspector
confirmed a 1,250 gallon tank was installed level at a depth of 10.4 feet below the foundation benchmark.
The absorption trench had been excavated to a depth of 13 feet below the footing and was measured to be
100 feet in length. The trenching configuration was noted to be slightly modified from the design; 40 feet
south and 60 feet north of the central manifold was measured and parallel to the length of the house. The
floor of the trench had been raked for ensuring the infiltration surface had not been smeared. In the
evening a second inspection was performed. The piping in the absorption trenches were measured at 7.7
feet below the footing benchmark. Installed monitoring tubes were measured to be 12.1 feet below the
top of the footing; within 1 foot of the bottom of the excavation previously inspected. The slope of the
piping from the tank outlet to the absorption area was also verified. Visual marks were made on the
standpipes for the cleanouts and monitoring tubes at the required minimum depths of cover.

Lot 21 Block 2 Glenn View Estates West Subdivision, Phase 2 — Rev. 1
To Whom It May Concern:
A proposed 4 -bedroom house is planned to be constructed on the 0.92 acre parcel described as Lot 21
Block 2 of Glenn View Estates West Subdivision. Phase 2. The property is located in the Peters Creek
off of Polly Circle and is surrounded by Tots which are developed with similar single-family homes. The
property is relatively flat with the topography generally sloping away from Polly Circle in a westerly
direction.
Domestic Wacte Water Calculations & Septic Desizn:
We are using a soil log- percolation test prepared by S&S Engineering for sizing our absorption area.
Their test hole was in the vicinity of the proposed infiltration trench and shows 2 feet of topsoil followed
by silty sand and gravel (SM/GM) down to a depth of 16 feet. No groundwater was noted. Their
percolation test found soils 5 to 6 feet below the surface to have a percolation rate of 7.7 minutes per inch.
Because an additional percolation test has yet to be performed we reviewed historic data for on-site
systems on adjacent properties. These records show similar soil conditions. Using Table 1 from Chapter
15.65 of the MOA Codes we conservatively estimated an application rate of 0.6 gpd/s.f. for our design.
A second percolation test shall be completed prior to the construction of the septic system to confirm the
size of the absorption area is adequate. Field changes may be required dependant on the results of the
second percolation test.
With the logged soil characteristics, moderate slopes, and the limited space found on this property we
recommend using a "5 -wide" shallow trench absorption system. Our design maintains 4 feet of ground
cover over the entire septic system. Because the S&S Engineering soil log was only performed to a depth
of 16 feet we have been constrained to a maximum depth of cover of 5.5 feet.

The following calculations were used for sizing all components of the on-site septic system:
Tank Size = 1,250 gal (1,000 gal for 3 bedroom + 250 gal / additional bedroom)
Daily Flow = 600 gpd (4 bedrooms x 150 gpd / bedroom)
Application Rate = 0.6 gpd/s.f. (per Chapt. 15.65 of MOA Code)
Required Absorption Area = 600 gpd + 0.6 gpd/s.f. = 1000 s.f.
Required Min. Length (5 ft wide) = 1000 s.f. _ (5ft) = 200 ft
Designed Depth of Shallow Trench = 4 ft
Reduction Factor = (W+2)/(W+1+2D) _ (5 ft+2)/(5ft+1+(2x4 ft)) = 7/14 = 0.5
Min. Length for "5 -Wide" Shallow Trench = 200 ft x 0.5 = 100 ft
Constriction of New Septic System:
A new 1,250 gallon septic tank will be installed and connected to a 4 -inch service line from the building.
The septic tank will be connected to a "5 -wide" shallow trench system at central manifold. The trench
will extend equal distances, 50 feet, to the north and south of the manifold. The absorption area will
consist of perforated distribution lines placed on top of 4 feet of filtering rock (3/4" to 1-1/2" washed
rock). Monitoring pipes will be installed at the end of each trench, and cleanouts will be installed on the
ends of each distribution line. The bed will be filled with additional washed rock to ensure 2 inches of
cover is maintained above the lines. Permeable geotextile fabric will then be installed over the washed
rock to prevent fines in the backfill material from infiltrating and damaging the absorption system. At
least 3 feet of native soils shall be placed over the entire system to obtain the minimum effective ground
cover. Backfill over absorption trench shall not be compacted. Backfill shall be mounded a minimum of
6 inches above existing grade to ensure positive drainage after settlement. Cleanouts shall be installed on
the service lines going in and out of the septic tank. All pipes going in and out of the septic tank shall be
installed on undisturbed native soils, or in trenches that have been adequately compacted.
Maintenance of Septic System:
Routine maintenance shall be performed on the system, including pumping of the septic tank at least
every two years. Using garbage disposals and flushing of non -degradable items (coffee grounds, cigarette
butts, tampons, paper towels, etc.) should be avoided. The owner should limit heavy surface loads in the
area of the septic system to maintain the structural integrity of the system. Trees and other large plants
should be removed and maintained at an adequate distance to prevent roots from growing into the system.

Approval (COSA) based only upon the representations given in paragraph 4 by an independent professional civil
engineer registered in the State of Alaska. Certificates of On -Site Systems Approval are required for the transfer of
title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or water
supply system. DSD also issues COSAs upon request to homeowners. Certificates of On -Site Systems Approval
are valid for 90 days from the date of issue for properties served by a private or Class C well and may be reissued
with new water sample results. '(Certificates may be reissued for a period of up to one year with valid water
samples.) Certificates am valid for one year for properties served by Class A or B wells or a public water system.
The Municipality of Anchorage Is not responsible for errors or omissions in the professional engineer's work.
