The Municipality of Anchorage Development Services Department (DSD) Issues Certificates of Onsite Systems
Approval (COSA) based only upon the representations given in paragraph 4 by an Independent professional civil
engineer registered in the State of Alaska. Certificates of Onsite Systems Approval are required for the transfer of
title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or water
supply system. DSD also Issues.COSAs _upon request to homeowners._ Certificates of On -Site Systems Approval
are valid for 90 days from the date of issue for properties served by a private or Class C well and may be reissued
with new water sample results. (Certificates may be reissued for a period of up to one year with valid water
samples.) Certificates are valid for one year for properties served by Class A or B wells or a public water system.
The Municipality of Anchorage is not responsible for errors or omissions in the professional engineer's work.

As certified by my seal affixed hereto and as of the validation date shown below, I verify that my investigation,
based on procedures outlined in the Certificate of Onsite Systems Approval Guidelines for this application,
shows that the on-site water supply and/or wastewater disposal system is (are) safe, functional and adequate
for the number of bedrooms and type of structure indicated herein. I further verify that based on the information
obtained from the Municipality of Anchorage files and from my investigation and inspection, the on-site water
supply and/or wastewater disposal system is(are) in compliance with all applicable Municipal and State codes,
ordinances, and regulations In effect at the time of installation.

Engineer's Comments: This investigation was completed in compliance with ADEC and MOA regulations. The
assessment of the condition of the well and septic applies only to the conditions as of the day tested. The flow
and absorption rates may change due to subsurface conditions that may not be observed from the surface,
changes Inland use, local soil characteristics, groundwater levels that may fluctuate during the year and the
water usage of the family being served by the system. The operational life of all well and septic systems are
subject to these various and dynamic characteristics and are outside the control of the

Dear Wilma,
Per your request, we have inspected the on-site water and wastewater systems of the
above referenced property. This investigation was based on observations of the day in
which the septic adequacy was performed, information from others and available
Municipality of Anchorage (MOA) record documents. Based on the above information
it appears that the property was developed between 1958/59 and no record documents
of the existing systems were. found at ADEC or the Municipality of Anchorage
(MOA). This is not uncommon for this area within the time this property was
developed.
The property has an existing single-family 5 -bedroom I -story structure with a walk out
basement. The property is served with a 1500 -gallon septic tank with crib disposal field
and a spring fed concrete cistern (see attached photos). Discussions with DSD Staff
indicate that since it is not a well they can not certify the system even though the water
meets current water quality standards. Others performed the water quality testing, with a
sample collected on September 21, 2009, and attached to this report.
On October 13, 2009 the septic adequacy test was performed. Prior to testing JRs
Pumping pumped the 1,500 -gallon septic tank. The absorption field monitoring tube
was dry at the start of the test and 750 gallons of water were added at the monitoring
tube. The septic field readily absorbed the water and was dry at the end of the test. The
system absorbed the required amount of 150 gallons per bedroom per day for a 5 -
bedroom residence. It is my opinion that the system's absorption capacity exceeds that
of a 5 -bedroom house.
It appears that the existing, undocumented septic system is sized as a 5 -bedroom
system. Feld measurements were taken and a testhole performed by Doug Kenley, PE
documents that the system does not encroach bedrock or ground water. Please see
attached drawings. The septic tank is approximately 95'+/- to surface water (see
attached as -built) and conforms to separation distances required at the time of
installation from the information obtained from municipal records. Presently the tank is
less than 5 feet from the building foundation, but met the current separation distances

the mainline in the house to the tank. All other separation distances conform to present
code requirements. From available data and visual observations, it appears that this
system poses no health or safety threat to adjoining properties or other individuals.

RE: HAMANN S/D ADDN #1, BLOCK 1, LOT 5 — ON-SITE WATER &
WASTEWATER SYSTEM
The owner has requested we proceed forward to obtain a COSA on the subject lot. As
part of the process of reviewing the files and investigating the property it was determined
that the house was constructed in 1968/69 and no record documents of the existing
system were found at ADEC or the Municipality of Anchorage (MOA).
The existing structure is a single-family 5 -bedroom 1 -story structure with a walk out
basement. The property is served with a 1500 -gallon septic tank with crib disposal field
and a spring fed concrete cistern. We tested and passed the septic system. Discussions
with DSD Staff indicate that since the water supply is not a well, they could not certify
the system even though the water meets current quality standards. Testing for the water
quality was performed by others and is attached to this letter.

We were unable to locate any public or private wells within 200' of our proposed system
location except as noted. There is no surface water within 50' of the existing system as
required at the time of construction or any known curtain drains within 50'. From
available data and visual observations, it appears that this system poses no health or
safety threat to adjoining properties or other individuals.

RE: HAMANN S/D ADDN $1, BLOCK 1, LOT 5 -- ON-SITE WATER &
WASTEWATER SYSTEM
The owner has requested we proceed forward to obtain a HAA and COSA on the subject
lots. As part of the process of reviewing the files and investigating the property it was
determined that the house was constructed in 1958/59 and no record documents of the
existing system were found at ADEC or the Municipality of Anchorage (MOA).
The existing structure is a single-family 5 -bedroom 1 -story structure with a walk out
basement. The property is served with a 1500 -gallon septic tank with crib disposal field
and a spring fed concrete cistern. We tested and passed the septic system. Discussions
with DIIHS Staff indicate that since it was not a well they could not certify the system
even though the water met current quality standards. Testing for the water quality was
performed by others and is attached to this letter.
In addition to the above information we are providing the following:

• Septic as built documenting the system and separation distances.
• Testhole verifying depth to water table and bedrock
• Current water samples
• Inspection Report
• COSA Form
• HAA Form
We were unable to locate any public or private wells within 200' of our proposed system
location except as noted. There is no surface water within 50' of the existing system as
