Dear Mr. Spurkland:
This office has completed its evaluation of the subject request
for waiver of the required 100 foot separation distance between~
and existing private well and the septic tank on the subject
lot. The request was flor a separation distance of 90 feet.
The request for waiver was evaluated in accordance with the
Alaska Department of Environmental Conservation (ADEC) waiver
review guidelines. Our evaluation of the supporting
documentation, which accompanied the request, yielded a total of
14.7 points. In accordance with ADEC waiver guidelines, a
minimum of 16 points is required for issuance of a waiver unless
the presence of special construction techniques and/or
mitigative factors is documented. Therefore the request for
waiver is denied.
Resolution of the separation distance deficiency may be pursued
by any of the following three alternatives:
1. You may prepare and submit additional supporting
documentation which supports the assignment of ].6 or
more points in accordance with ADEC waiver review
guidelines.

2. You may prepare an engineering report that documents
special construction techniques and/or the presence of
mitigative measures or factors which support the
issuance of the waiver. Such a report must conclude
that there is negligible possibility or probability of
wastewater system serving as a point source of
contamination to the well. The conclusion must be
supported by an appropriate engineering analysis.
3. You may relocate the offending we~L1 or septic tank.

The Municipality of Anchorage Development Services Department (DSD) Issues Certificates of On -Site Systems
Approval (COSA) based only upon the representations given in paragraph 4 by an independent professional civil
engineer registered in the State of Alaska. Certificates of On -Site Systems Approval are required for the transfer
of title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or
water supply system. DSD also Issues COSAs upon request to homeowners. Certificates of Onsite Systems
Approval are valid for 90 days from the date of issue for properties served by a private or Class C well and may
be reissued with new water samples. (Certificates may be reissued for a period of up to one year with valid water
samples.) Certificates are valid for one year for properties served by Class A or B wells or a public water system.
The Municipality of Anchorage is not responsible for errors or omissions in the professional engineer's work.

In conducting this evaluation, GEG, VD. attempted to provide a thorough,
conscientious engineering analysis of the system In accordance with ADEC and MOA
DSD Guidelines B Regulations. The reported results described the performance of the
system under the conditions encountered at the time of the test, and separation
distances measured to readily Identifiable features. The operational life of all wells and
septic systems depend on the local soils condition, groundwater levels that may
fluctuate during the year, and the water usage of the family being served by the system.
These conditions are outside the control of the evaluator of the system. Satisfactory test
results do not guarantee future performance of the system, nor do they guarantee that
there are no hidden defects or encroachments. GEG, LTD. can therefore not provide
any warranty or future estimate of how long the system will continue to meet the
operational requirements of the ADEC or MOA DSD. The content of this report is for
the sole benefit of the owner listed above. Any reliance upon or use of this report by any
other person or party is not authorized, nor will it confer any legal right whatsoever.

Ref: COSA for T12N R3W, SEC 27, L57A, Waiver amendment request
To whom it may concern:
It was observed during our COSA site visit that the original waiver granted to 90' from the well to the
septic tank is not sufficient. It was observed that the distance from the edge of the well casing to the clean
out stand pipe of the first compartment of the septic tank onsite is exactly 90'. Given the fact that the tank
is sitting perpendicular to the well, the Northeast corner of the septic tank onsite is encroaching upon the
well by approximately 86'. We request that the existing waiver issued for the addressed property be
amended to a distance of 85'.
Drainfield notes
It was observed during our septic adequacy test that the measurements conducted in the field to not
match -up with the criteria notes on the original inspection report from 1982. The inspection report shows
that the original design was modified to accommodate a "huge rock" using a twin trench spill over type
system. The inspection report shows the effective depths of 108" for the north trench, and 36" in the south
trench.
Per our field measurements, the effective depth of the north trench is roughly 105" and the south trench
an effective depth of 95.5", measured through a sump type monitoring tube.
The north trench was found to be surcharged. The system absorbed over 826 gallons in the South trench,
thus proving that the system can absorb 450+ gallons in a 24 hour period. The original test hole shows no
water or impermeable soils encountered to a depth of 20'.
