Septic System Conditional Approval Request
This letter is intended to request conditional approval for the septic system serving the subject lot. This system consists
of a 1500-gallon MoA approved epoxy-coated steel tank utilizing an Advantex AX-20 treatment unit and auxiliary pump
vault –to discharge into a mounded wide-trench. This lot and surrounding lots are served by private wells. This system was
approved on 08/14/2024 and installed by Precision General Contractors, LLC. within the period from 09/03/2024 to
09/16/2024.
Ground cover was observed by PES during final grade inspection conducted on 11/21/2024 to a magnitude of only 5 to 16-
inches. Slopes were observed on this date to not meet the listed requirements under AMC 15.65.210(E).
Per Paul Douthit (Owner), the existing system has been in operation without issues since its commissioning by Anchorage
Tank on 11/14/2024. Per Precision General Contractors, LLC. (contractor), the mound has in its existing state –has 4
inches of rigid insulation. Paired with snow cover, the field should operate throughout the winter given that it utilizes
pressure distribution and water does not remain in the lateral distribution pipe. The side slopes of the mound are not at
risk of erosion given that the top layer is frozen. The drain field mound and tank manhole depressions shall be
completed to MOA standards by the contractor as soon as construction is feasible in spring 2025. The owner of the
property assumes all responsibility and cost associated with completing the mound along with any and all maintenance
costs associated with the system in its entirety if the system is to freeze. This is to say that the owner shall be financially
responsible for all costs associated with the system and shall not seek any monies from Pannone Engineering Services,
LLC associated with the system in its entirety. The owner shall notify PES prior to completion of the mound so we can
ensure that the finished mound complies with MOA regulations. Failure of the contractor & owner to complete the mound
in a timely manner shall be their own fault and PES shall not be held liable by the MOA, contractor, or owner –for any
penalties imposed on the property or the owner by the Municipality of Anchorage. By signing below the owner agr ees to

Initial Septic System Permit Request
This is a design narrative for a permit to install an initial well and septic system on the subject property. The proposed
system will serve an existing two (2) bedroom dry cabin. The wastewater treatment system will be sized for three (3)
bedrooms to allow for potential future expansion of the structure. This lot and surrounding lots are served by private
wells. Currently there are no wells within 100ft of the proposed installation. A permit to install a Privy on this lot was
granted in May 2010. It is our understanding that the privy was never constructed. The proposed location for the privy is on
the northern property line. If the privy was constructed it will be decommissioned per MOA code.
1. Soils: A test hole was performed on this lot by Pannone Engineering Services, LLC on 6/13/2024. Groundwater was
observed during the excavation of the test hole, seeps were encountered at a depth of 1.0’ BGS with groundwater
encountered at a depth of 5.0’ BGS. Bedrock was not encountered. Groundwater was observed in the test hole
monitor tube after 7-days at a depth of 1.0’ BGS, the groundwater elevation was adjusted by 1.0ft per moa code.
Based on the results of the percolation test and overall soils appearance an application rate of 6.0 gpd/SF was used
for the design of an advanced treatment vertical conveyance ERS wastewater treatment system in the area of the test
hole.
2. Soil Absorption System Design. See Sheet 1 of the design package.
3. Surface Water: There is no surface water within 100 feet of the proposed septic tank and drain field. There are
mapped wetlands on this and surrounding properties. ABR, Inc performed a wetland determination on this property. The
report is attached.
4. Topography: See attached site plan for area topography.
5. Drawing Markings: The Drawings are marked “For MOA Review Only”. When written notification that the review is
complete and no further comments are received from MoA On-Site Department, the note will be removed and “For
Construction” drawings will be issued.
The proposed installation will not affect the future development of this or the surrounding lots.

MAINTENANCF. AND REPAIR AGRLEMENT
'PHIS MAINTENANCE AND REPAIR AGREEMENT, herein the "AGREEMENT" made and

Anchorage, herein the "MUNICIPALITY", in accordance with Anchorage Municipal Code
(AMC) 15.65.365. In consideration of the mutual covenants contained herein, the parties to this
Agreement agree as follows:
1. Advanced Wastewater Treatment Systems. The Municipality grants permission to the
Owner to utilize and operate an Advanced Wastewater Treatment System (AWWTS),
described as Advantex

2. Maintenance, Repairs and Alterations.
(Owner is required to read, understand and initial each section)
__fW Throughout the term of this Agreement, the Owner shall enter into a service agreement
with an AWWTS service and maintenance provider approved by the Municipality or the
manufacturer's representative. The AWWTS shall be maintained in a satisfactory
condition capable of performing as designed and producing treated septic effluent in
accordance with the cquipment's approval for operation in the Municipality.
10D It shall be the responsibility of the Owner during the term of this Agreement to pay for all
repair(s), maintenance, adjustment(s), replacement costs, and inspection costs. This
includes an annual maintenance fee (typically $400 to $600).
.Q Owner agrees that only maintenance and repair personnel approved by the Municipality
or the manufacturer's representative will inspect and make any necessary maintenance,
repairs or permitted alterations to the system.
POD Owner acknowledges that regular maintenance of an AWWTS reduces the potential
failure of the system, which could include sewage backup and costly repairs or drainfield
replacement.

repairs from the manufacturer's representative or maintenance provider.

guidelines for the construction, maintenance and repair of the Owner's AWWTS.

3. Term. The term of this Agreement shall begin on the date of approval by the
Municipality to operate the installed system, or upon transfer of title, and shall continue
while the AWWTS is operational or until title is transferred.
4. Nonwaiver. The failure of the Municipality at any time to enforce a provision of this
Agreement shall in no way constitute a waiver of the provisions, nor in any way affect
the validity of the Agreement or any part hereof, or the right of the Municipality
thereafter to enforce every provision hereof.
5. Amendment. This Agreement shall only be amended by authorized representatives of
the Owner and Municipality. Any attempt to amend this agreement by either an
unauthorized representative or unauthorized means shall be void.
6. Jurisdiction: Choice of Law. Any civil action arising from this Agreement shall be
brought in the Superior Court for the Third Judicial District of the State of Alaska at
Anchorage. The laws of the State of Alaska shall govern the rights and obligations of the
parties under this Agreement.
7. Severability. Any provisions oil' this Agreement decreed invalid by a court of competent
jurisdiction shall not invalidate the remaining provisions of the Agreement.
