Mr. Crewdson:
The permitted septic system was installed on the referenced property in 2010. Since that time, the
drainfield and Quanics Aerocell have been performing well. This year it was noticed that there is no
continuity to the Jandy Valve wires. During our last few site visits, we have observed that the drainfields
are still dry. In order to simplify the operation and maintenance of the system, we are proposing that this
unit be permanently configured as a CAT II system. Our justifications are as follows:
The drainfields both received at least 4' of filter sand during construction.
The receiving soil perked at less than 1 minute per inch and so the CAT II soil application rate is
4.0 GPD/Sq. Ft.
The home has 4 bedrooms, so 150 sq./ft. of absorption area is required. The existing drainfields have 215
sq/ft. of absorption area.
The following information should supersede the application rates outlined in GEG's original design dated
7/20/10:
Allowable application rate: 4.0
-Proposed application: 3.0
At this time, we request that your department approve a CAT II configuration to be used at the referenced
property. Upon your concurrence, we will insure the jandy valve is in the open position, and will place a
CAT II sticker on the control panel.

Ref: Proposed Septic System for Greenbrook Subdivision; Lot 3, and T12N, R3W,
Section 26, E2, NE4, NE4, NW4.
To whom it may concern:
The proposed 4 bedroom house to be located on Greenbrook S/D; lot 3 is to be served by the
existing Class "A" water system and a private septic system. Multiple test holes were excavated
on lot 3 and no suitable soils were found on-site. The property to the west of lot 3 (T12N, R3W,
Section 26, E2, NE4, NE4, NW4) is owned by the same property owner and two test holes were
excavated on this property. Test Hole #4 was found to be suitable for a septic system, but will
require the use of an advanced wastewater system. We are proposing that the house be located
on lot 3 and the septic system to be located on T12N, R3W, Section 26, E2, NE4, NE4, NW4.
The owner will be required to grant an ingress/egress sewer easement for the proposed design.
We are proposing to utilize the Quanics Aerocell system for this property which will require the
installation of a 1500 gallon STEP tank and dual 5 -wide trench type drainfield with a MOA
approved sand filter. The drainfield will be designed around the 30 foot radius of this test hole
#4. Upon your approval of the proposed design, the owner will be required to have the
ingress/egress easement recorded at the State of Alaska Recorders Office. Comments
regarding the design are summarized as follows:
1. SOILS: See the attached logs which shows the soil classifications, groundwater monitoring,
and the percolation test results. Test holes #1, #2, and #3 were found to be unsuitable;
however, test hole #4 had suitable soils for an on-site absorption system. A percolation test was
performed between 1.5 to 2.5 feet and had a percolation rate of <1 minute/inch. The
groundwater was found to be at 3'-10" below the ground surface. We are proposing that the
drainfields be excavated to a depth no more than 1.5 feet deep, removing all organics, and
having four feet plus (4'+) of MOA approved sand filter installed. This will give us a 6.3 feet
separation distance to the groundwater reading taken 7/25/2006. Based upon this information,
the proposed design meets the MOA policy regarding groundwater separation distances.
2. DRAINFIELD DESIGN: See attached design drawing for drainfield specifications.
3. SURFACE WATERS: There are no surface waters within 100 feet of the proposed septic
system; however, both properties have classified wetlands on property. Attached is a
jurisdictional delineation map which shows the wetland areas. Prior to any construction, the
owner or contractor shall have the wetland area flagged/staked by the Corp of Engineers or the

MOA wetlands department to insure the septic system or house is not placed or effect the
wetlands. The owner will be responsible for any wetland permits required for this installation.
4. TOPOGRAPHY: As can be seen on the attached design drawing, the average topography
in the proposed drainfield area is a 1 to 10 percent slope running approximately east to west.
There are no cutbanks or slopes greater than 50 feet downhill from the proposed drainfields. In
short, there are no slope concerns.
We are unaware of any adverse impacts this installation would have on adjacent wells or septic

Ref: Response to Comments for Greenbrook Subdivision; Lot 3
To whom it may concern:
The following is our response to your comments for the referenced property:
1. The Ingress/egress agreement is to be filed with the state before the permit is issued. — A
copy of the recorded ingress/egress easement has been supplied to your department.
2. Please submit the wetland permit. — A copy of the wetlands permit is attached along with
a delineation map supplied by the COE.
3. Letters ofnon-objection required for the sewer line going through the utility easements. -
Letters of non -objection have been obtained and supplied to the right-of-way
department.
4. There is not 2' of unsaturated acceptingsoils. — As of 8/10/2010, there was 2.5 feet of
unsaturated accepting soils. However, based upon the MOA's new interpretation of their
own policy, we are now required to account for a 2 foot rise in groundwater levels from
this reading which would result in all the insitu receiving soils being saturated. We are
proposing to remove the organic layer and build up the drainfield with 4 feet of MOA
approved sand in order to meet the separation distance to the anticipated high season
groundwater. We request a waiver to the MOA policy that requires there be 2 feet of
unsaturated insitu receiving soils, and request the MOA allow the system to be placed
with classified fill (MOA sand filter). We are unaware of any adverse effects this waiver
would have on adjacent wells or septic systems. We are also using an advancod
wastewater sy tem for this system. Based upon these mitigating factors, it is our opinion
that the waive should be granted.

ADVANCED WASTEWATER TREATMENT SYSTEM MAINTENANCE AND
REPAIR AGREEMENT
MEMORANDUM OF UNDERSTANDING
BETWEEN MUNICIPALITY OF ANCHORAGE

THIS MEMORANDUM OJJJNDFftSTANDTNG made and entered into as of this

herein the "OWNER," and the Municipality of Anchorage, herein the
"MUNICIPALITY." In consideration of the mutual covenants contained herein, the
parties to this Memorandum of Understanding agree as follows:
1. ADVANCED WASTEWATER TREATMENT SYSTEMS. Municipality
grants permission to Owner to utilize and operate an Advanced Wastewater
Treatment System (AWWTS), described as (1 -CAM J�L S Ar-,-t.7t e- ? I located

2. Definitions.
A. Alteration. Any change to the design or function of an AW WTS that
Includes the installation or removal of any parts, components or piece,,; not
included in the original construction pen -nit and design. Prior to
perfonning any alterations to an AWWTS the owner must obtain a
Wastewater Disposal System Construction Permit from the Municipality
pursuant to Anchorage Municipal Code (hereinafter, "AMC;") 15.65.
B. Certificate of On -Site Systems Approval. An approval by the
Municipality ot"existing water and wastewater disposal systems given at
the time of property sale and title transfer in accordance with AMC 15,65.

These approvals certify that the systems are adequate for the home-, that
they support and meet the codes that were in place at the time of'systern
construction.
C. Damage. Any man-made or natural change in a system that would inhibit
the system from performing as designed,
D. Maintenance and Repair. The scheduled and as needed replacement of
existing parts, components and pieces of an AWWTS' that were included in
the original design which would allow the AWWTS to continue to
perform as designed,
E, Permit (Construction) An On -Site Wastewater Disposal System
Construction Pen -nit as defined by AMC 15.65.
K Permit (Operating) An Advanced Wastewater Treatment System
Operating Permit. An annual permit, issued by the Municipality, that
allows the Owner to operate an AWWTS, upon meeting all the
requirements of this agreement, the conditions of Operating Pen -nit, the
requirements of the On -Site Wastewater System Construction Permit and
all relevant provisions ot'AMC 15.65
I Fee. Owner shall pay to Municipality an annual fee of
($ Q .00), payable on or before the issuance of the operating permit and annually
thereallcr. The annual fbc is due on or before the anniversary date ofthc approval by the
Municipality of installed system.
4. Terin. The term of this Memorandum of Understanding, shall be for the life of the
AW'W'I"S.

The term begins on the date of approval by the Municipality of the installed system and
shall continue while the AWWTS system is in use or operational or until the property is
sold or title is transferred by owner and a new certificate of On -Site approval is issued to
the new owner or transferee of the property,
5. Alterations, Installation and Removal of Additional Equipment. Owner agrees
not to make any alterations, removal of parts or additions to the AW WTS without a
Construction Permit from the Municipality.
6. Maintenance and Repid .
A. Throughout the term of this Memorandum of Understanding, the Owner shall
maintain AWWTS in good repair. In addition, it shall be the responsibility of' the
Owner during the teen of this Memorandum of Understanding, and any extensions or
renewals thereof.. at the owner's sole expense, to pay for any and all: (1) repair(s), (2)
maintenance, (3) adjustment(s), (4) replacement costs., and (5) inspection costs,
Further, Owner agrees to comply will all applicable ordinance, laws, regulations,
rules and orders for the A W WTS'.
B. Owner agrees to provide the Municipality a written schedule of routine
maintenance and repairs which have been performed on the systern pursuant to the
terms and conditions contained in the Owner's AWWTS Operating Permit. This
schedule shall be submitted to the Municipality annually upon the renewal of the
permit. The schedule of maintenance and repair contained in the Owner's AWWTS

D. Owner agrees that only maintenance, repair personnel certified by the

F. Owner agrees that any sale or transfer of title of the property will not occur
without a new Certificate of'On-Site Systems Approval.
G. Owner agrees that the relevant provisions of the standard specification guidebook
for AWWTS is the governing professional guidelines for the construction,
maintenance and repair of the Owner's AWWTS.
7. Nonwaiver. The failure of either party at any time to enforce a provision of this
Memorandum of Understanding shall in no way constitute a waiver of the provisions,
nor in any way effect the validity of'thc Memorandum of Understanding or any part
hereof, or the right of such party thereafter to enforce each and every provision
hereof.
8. Amendment.
A. This Memorandum of I Jnderstanding shall only be amended, modified or changed
by a writing, executed by authorized representatives of the parties, with the same
formality of this Memorandum of Understanding was executed and such writing shall
be attached to this Memorandum of Understanding as an amendment.
B. For the purposes of any amendment modification or change to the terms and
conditions of this contract, the only authorized representatives of the parties are.,

boundaries of said septic leach field, drain pipes, or other below -surface installation required by
any applicable state or local reg',•la ions regarding residential septic systems snail be Iixed and
define the boundaries of this grant of easement.

touches, concerns and runs with said lands., This is not intended to be an exclusive easement:
Grantor herein expressly reserves unto itself equal rights of ingress and egress and all other

In the event any successor party other than the hereinabove -named Grantee or Grantor owns
either parcel of land described in this document, and a dispute or disagreement arises under or in
any way concerning this instrument, it shall be submitted for resolution to the process of binding
arbitration in accordance with the Alaska statutes made and provided for said arbitration. The
jurisdiction of the Arbitrator shall commence 60 days from the onset of the dispute and shall
operate to toll all applicable Statutes of Limitation.
