All construction shall be in accordance with:
1. The attached approved design.
2. All requirements specified in Anchorage Municipal code Chapters 15.55 and 15.65 and the State of Alaska
Wastewater Disposal Regulations (18AAC72) and Drinking Water Regulations (18AAC80)
3. The wastewater code requires inspections during the installation. The engineer shall notify the Development
Services Department per AMC 15.65. Provide notification by calling (907) 343-7904 (24/7).
4. From October 15 to April 15, a subsurface soil absorption system under construction during freezing weather
shall be either:
a. Opened and Closed on the same day, or
b. Covered, sealed, and heated to prevent freezing

Special Provisions:
• Locate the end of the trench to ensure that the required separation to the new tank is met. Per the permit,
install a cleanout.
• Install boulders, bollards or similar to protect the tank from being driven over.

Subject: Addenda Lot 2 - 7511 Addenda Circle
Septic Tank Replacement
Dear On-Site Services Engineer:
The owner of the above lot has a septic tank that has reached its end of useful life, so we are
submitting this permit application for its replacement. The attached site plan identifies the location
of the home as well as the wells and septic location. No conflicts exist between this proposed
system and any other wells or septic system, whether on this lot or adjacent lots. We are replacing
the septic tank with the same size designed for 3 bedrooms.
The new septic tank will be a minimum of 100’ from all wells and surface water. The space is very
tight in the area where the septic tank will be located, but there appears to be enough room to
maintain 10’ separation from the foundation and 5’ separation from the existing trench. The
existing trench does not have any cleanouts, and only one monitoring tube. As part of this permit,
the near end of the trench will be uncovered and a cleanout added.
Please refer to the attached plan for the septic design. If this design is followed, there will be no
adverse impacts to adjacent properties.

Tank to field waiver request
Dear On-Site Services Engineer:
During the installation of this new septic tank the near end of the septic trench was uncovered and
a cleanout added. Only the top portion of the trench was exposed for the installation. However, the
location of the trench made it impossible to provide 10’ separation from the building foundation
without putting the tank under the driveway in front of the garage. The tank was therefore placed
outside of the driving area, 10’ from the foundation, but within just a few feet of the trench.
The new tank is further from the foundation than the old tank, and does not cause any issues with
the trench. The tank is off the end of the trench and effluent reaching the tank via subsurface flow
will not inhibit treatment. We would like to request a waiver be issued for a 1.5’ separation from
the tank to the leach field.

During a recent Health Authority Approval on-site inspection and test of the
potable water supply well on Block , Lot 2 of Addenda subdivision,
the well's productivity was determined to be 0.8 gallons per minute. The
minimum well productivity required by this Department (AMC 15.55) for a
3 -bedroom residence is 0.31 gallons per minute. Although the subject well
currently exceeds this minimum requirement, all parties concerned are
advised that the production capacity of the well may fluctuate. Restriction

Subject: Lot 2, Addenda Subdivision
Well and Septic System Inspection and Testing
Dear Carol:
The septic system serving Lot 2, Addenda Subdivision was originally constructed in July
of 1983 and has remained in service since that time. It is composed of a steel septic
tank with a capacity of 1,050 gallons and a 5' wide absorption trench approximately 27'
in length with an effective depth of 4'. This information is taken from records found in
the Municipal files and completed at the time of construction in 1983. The records
further indicate the trench was constructed in soil materials rated with an absorption
requirement of 85 square feet of absorption area per bedroom. The components of the
septic system appear to meet all setback requirements as mandated by the
Municipality. The end of the trench is very near the edge of the driveway and may
encroach under the driveway for a short distance. The aboveground cleanout/monitor
tube shown on the as -built at the end of the trench could not be found. A new tube was
installed in the trench to allow inspection and testing of the trench.
Our initial probing of the absorption trench during placement of the monitor tube
showed no water. Very little biomat material was noted on the drain field rock indicating
the absorption trench is in good condition. We then injected 450 gallons of water into
the trench through the new monitor tube. No water developed in the trench and was
apparently immediately absorbed into the surrounding soil lense. It is obvious based
on our test the absorption trench will absorb more than 450 gallons of water in a 24
hour period and is functioning in compliance with Municipal requirements which state an
absorption trench must be capable of dispersing a minimum of 150 gallons of water per
day per bedroom.
Arrow Pump and Well Service of Anchorage completed a flow test on the well on
February 20, 2006. Their results indicate the static water level in the well was
measured at 52' below the top of the casing. The well flow rate was determined to be
.8 gallons per minute. This production rate exceeds the requirements of .324 gallons
per minute for a three-bedroom home. The pump in the well is the original unit installed
more than 20 years ago. It is apparently showing signs of wear and, in the opinion of
Brian Wille of Arrow Pump and Well Service, may be require replacement to improve

the production rate of the well. The pump currently in place, however, will deliver
sufficient water to meet the requirements of the Municipality.
Water from the well was tested for coliform, bacteria, arsenic and nitrate content and
found to be within acceptable tolerances for human consumption. Copies of the results
are attached for your review.
