Box 196650 Anchorage, AK 99519 REFERENCE: Lot], Marvin SID It is requested that you issue a permit to install a new shallow trench and 1300 gallon IIDPF, septic tank septic system to serve the existing four bedroom house on the referenced property. A flow diverter valve will be installed to allow the existing trench to be used at a later date. A new monitoring tube will also be installed in the existing trench to replace the six inch sump. A test hole was excavated and a percolation test performed by Pannone Engineering Services. The approximate location of the test hole is located on the attached site plan. At the time of excavation (7/12/2004) water was not found to 13 feet. After seven days of monitoring, ground water was found at 12.2 feet. We do not anticipate any adverse effects on neighboring wells, septic systems, reserve areas or drainage patterns by the installation of the proposed septic system. The scope of this project includes the installation of a new 1300 gallon 11DPE septic tank and two shallow septic trenches to serve the existing four bedroom ETKs'NEERw051LOES residence located on the referenced property. A flow diverter valve will be MIDREPORTS installed to allow the existing trench to be used at a later date. A new monitoring tube will also be installed in the existing trench to replace the six inch sump. NELLMSPECRON SFLOWTEST 2. Construction shall be in accordance with the approved site plan and design drawings, Municipal permit with any special provisions or conditions, and all applicable State and Municipal Wastewater Disposal Regulations. &TEPL44 3. Unless specifically agreed otherwise, the property owner shall be responsible for final grading areas subsequently depressed from soil settling. SOn TEST 5. Contractors installing wastewater disposal systems must be certified by the Municipal Development Services Department or ADEC if required, for system installations. Owners installing their own systems must also receive PMOL Tom,, prior approval from the respective agency. TEST SEPTIC TANK INSTALLATION: STR, %AAL& 1. A septic tank is to be constructed by a certified septic tank manufacturer. ONS Construction shall include two 4" cleanouts for pumping access. All 1lDPE ONSTE WASTEWATER XPOM SYSTEM OESGN 15861 S. Birchwood Loop Road - Chugiak, Alaska 99567 Page 2 Lot 1; Block 1; Marvin SID March 7, 2010 septic tanks are required to have a 4" monitoring pipe installed next to the tank extending to the bottom of the tank. 2. The septic tank shall be sufficiently bedded to prevent settling or shifting of the tank. 3. All standpipes on the septic tank shall extend a minimum of 12 inches above final grade. 4. Septic tanks installed with less than 4 ft. of cover shall be insulated. All IIDPE septic tanks are required to be covered with a minimum of 2" direct burial insulation, and a maximum of 4 feet of cover. 5. A foundation cleanout shall be installed one to four feet from the building foundation. In the line between the tank and the leachfield there shall be two adjacent cleanouts (unless an effluent pumping system exists within the septic tank). These cleanouts shall be located on undisturbed soil not more than 10 ft. from the tank. The first cleanout, in line, shall be to clean toward the leachfield. The second cleanout shall be to clean toward the septic tank. 6. Final grading over the septic tank shall be such that a positive slope exists away from the septic tank. ABSORPTION TRENCIUDRAINFIELD INSTALLATION: Excavate the proposed trench to the dimensions shown on the design. The bottom of the excavation shall be within 2 inches of level. If the sidewalls of the excavation become smeared, they must be raked or scratched (ruffed -up) before gravel (sewer rock) placement. 2. Once the gravel is installed, the distribution pipe is to be installed level with the perforations faced downward. Gravel is then to be placed over the distribution pipe to provide a minimum of 2 inches of cover over the pipe. 3. A silt barrier must be installed between the final gravel layer and the native soil backfill. Ensure the silt barrier covers the entire gravel surface before placing backfill. 4. Monitor tubes shall be of four (4) inch diameter, installed approximately in the locations shown on the design, and extend a minimum of 12 inches above final grade. The portion of the monitoring tube extending through the gravel shall be perforated from the bottom of the trench to the invert of the distribution pipe. This is equivalent to the effective depth of the gravel as noted on the design. Page 3 Lot 1; Block 1; Marvin S!D March T, 2010 5. Backfill over the final gravel layer must not be less than twenty-four (24) inches. Insulation must be installed when the backfill depth is less than thirty-six (36) inches. The finish grade over the trench must be mounded to prevent the formation of a depression alter settling. Page 4 Lot 1; Block 1; Marvin SID March 7, 2010 NIININIUM MATERIAL SPECIFICATIONS: Any septic tank proposed for installation must be constructed by a Municipal approved septic tank manufacturer. 2. Insulation shall be at least 2" thick extruded direct burial polystyrene (Dow Chemical Company Styrofoam III or equal). 4. Septic tank inlets and outlets shall be fitted with watertight couplings (Caulder, Femco, or equal). 5. A permeable nontoxic silt barrier (Typar 3401, Mirafi 140N, or equal) must be installed between the final leachfield gravel layer and the native soil backfill. 6. All leachfield gravel (sewer rock) shall be 0.5"-2.5" screened gravel with less than 3% passing the 11200 sieve. 7. When sand is being used as a filter material, its gradation specifications must conform to current M.O.A. or D.E.C. requirements, which ever requirement applies. Page 5 Lot 1; Block 1; Marvin SID March 7, 2010 INSPECTIONS: Typically there will be a minimum of three (3) inspections required during the installation of the wastewater disposal system. These inspections will occur as follows: The first inspection must be conducted after the excavation of ditches, pits, trenches, or beds and before the installation of any gravel. A septic tank may be set in place, but may not be backfilled before this inspection. 2. The second inspection must be conducted after the placement of the silt barrier, gravel, distribution lines, standpipes, cleanouts, and insulation, but before the placement of any other backfill. 3. The final inspection is to occur upon final grading of the property. Often there will be more than these 3 inspections required. Especially with the installation of multiple trenches, sand filters, pressurized distribution systems, etc. Thus, the inspecting engineer is to be contacted at least 24 hours prior to the start of construction. If necessary, a pre - construction meeting will take place on-site. The inspecting engineer will not coordinate, direct or control in any way the contractors activities. The owner shall contract with the contractor to perform the work outlined in these specifications and plans and in accordance with the attached M.O.A. permit. There will be no contractual arrangement existing between the contractor and S R S Engineering. S R S Engineering shall be the owner's representative and will inspect the work as stated above to document the contractors activities. '522-6779 (FAX) _ August 25, 2002 Ms. Linda Brame 15238 Husky Street Eagle River, AK 99567 Subject: Lot 1, Marvin Subdivision Well and Septic System Inspection, Test and Certification Certificate of Health Authority Approval Dear Linda: We recently completed the inspection and testing of the well and septic system on your property. Prior to the inspection we reviewed the records pertaining to the systems on the property from the files of the Municipality. These records indicate the septic system was originally constructed in July of 1976 and consisted of a two compartment steel septic tank with a capacity of 1,000 gallons. Effluent from the tank is currently absorbed in a 50' long by 3' wide by 6' effective depth absorption trench with a 6" sump at the end. Sanitary Pumpers pumped the septic tank on August 6, 2002. Our initial probe of the trench indicated a standing water depth of 56" with a total effective depth of the trench of 72". Water was then injected into the crib to determine the absorption rate over a 24-hour period. The water level rose to 59" after 450 gallons of water had been added. Twenty-three hours later the water had receded to the 56" level. It appears therefore, the absorption trench is currently capable of absorbing more than 450 gallons of water per day which is the minimum required for certification by the Municipality. The normal life of a septic system is from 8 to 12 years and can be less depending on the type of system, quality of soils in which it is constructed, maintenance and type of use. Your system is now over 26 years old and well past an average useful life of a typical system. We are unable to estimate the future life of the system or to make any guarantees or warrantees based on our limited testing and inspection. The system may be subject to groundwater intrusion during periods of heavy runoff, which may affect its performance. At this particular time, however, the system appears to meet the requirements for which it was originally designed. Simultaneously with the adequacy test we performed a flow test on the well and water system on the property. The log for the well states it was completed on March 11, 1976. It has a total depth of 325' with casing to 147'. The log further states the well produces 1 gallon of water per minute. For this reason we Lot 1, Marvin Subdivision August 25, 2002 Page 2 of 2 performed the test with a 1 gallon per minute flow restrictor on the hose connected to the exterior hose bib. We noted no low flow protection on the well and attempted to prevent total draw down of the well. The well was run constantly for a total of 7.5 hours at an average rate of slightly more than 1 gallon per minute. During that time the water level in the well receded from 110' to more than 240'. Based on our test we conclude the well is producing approximately 1 gallon per minute or more than 450 gallons per day which is required for certification. We must note, however, the well will not sustain higher flows and may draw down completely to at or below the pump level if allowed to flow freely for long periods of time. A low flow shutoff, though not required, should be considered to prevent damage to the pump. A water sample was taken from the system and tested for coliform, nitrates and other bacteria. No coliform or bacteria was detected and the nitrate content was undetectable and well below the maximum allowable of 10 milligrams per liter. The water is therefore well within acceptable parameters and suitable for human consumption. The well is currently situated barely 100' from the septic tank and more than 100' from the absorption trench. State law requires a minimum of 100' of separation be provided. Sincerely, // Michael E.
