Disposal Regulations (18AAC72) and Drinking Water Regulations (18AAC80).
3. The wastewater code requires inspections during the installation. The engineer must notify the Development Services
Department at least 2 hours prior to each inspection. Provide notification by calling (907) 343-7904 (24 hours).
4. From October 15 to April 15, a subsurface soil absorption system under construction during freezing weather must either:
A. Open and Close on the same day.
B. Covered, sealed, and heated to prevent freezing.

To Whom it may concern:
This is a request for a new septic system permit on the above referenced lot. The owner has also requested
upgrading to a 4 bedroom system. Therefore two test holes were completed with both of the test holes soils
consisting of silty gravel (GM) with some sand, for the entire 18 foot hole depth. The perc tests were 20 &
24 minutes per inch. No water was observed during the excavation or after the 7 day monitoring. The
trenches are perpendicular to the slope of the lot. The old tank will be pumped and crushed per the MOA
codes.
All of the north neighboring lots are services by a community well located about 400 feet away. There is
neither surface water within 100 feet nor any known curtain drains within 50 feet of the proposed system.
None of this development will have any adverse effect on the adjacent lots.

This letter is response your comments on Sept 1, 20011. The 200 foot clear area around the community
well has been added to the site drawing. This is also our request for a 5 foot lot line waiver, separation
distance between the new/existing system waiver and a perpendicular to slope waiver. Due to existing well
radius', property lines and utility easement we have a very limited work area. The exact location of the crib
is not known but per the MOA records our test hole #1 should have disturbed the system but nothing was
found. The monitoring tube has been shown on the site.

Approval (HAA) based only upon the representations given in paragraph 4 by an independent professional civil
engineer registered in the State of Alaska. Certificates of Health Authority Approval are required for the transfer
of title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or
water supply system. DSD also issues HAAs upon request to homeowners. Certificates of Health Authority
Approval are valid for 90 days from the date of issue for properties served by a private or Class C well and may
be reissued with new water samples. (Certificates may be reissued for a period of up to one year with valid
water samples.) Certificates are valid for one year for properties served by Class A or B wells or a public water
system. The Municipality of Anchorage is not responsible for errors or omissions in the professional engineer's
work.
Note: Alaska Water and Wastewater Consultants, Inc. shall be paid S at, orprior

information obtained from the Municipality of Anchorage riles and from my investigation and inspection, the

and State codes, ordinances, and regulations in effect at the time of installation.

Engineer's Comments:
In conducting this evaluation, AKW WC, Inc. attempted to provide a thorough,
conscientious engineering analysis of the system in accordance with ADEC and MOA
DSD Guidelines & Regulations. The reported results described the performance of the
system under the conditions encountered at the time of the test, and separation
distances measured to readily identifiable features. The operational life of all wells and
septic systems depend on the local soils condition, groundwater levels that may
fluctuate during the year, and the water usage of the family being served by the system.
These conditions are outside the control of the evaluator of the system. Satisfactory test
results do not guarantee future performance of the system, nor do they guarantee that
there are no hidden defects or encroachments. AWWC, Inc. can therefore not provide
any warranty or future estimate of how long the system will continue to meet the
operational requirements of the ADEC or MOA DSD. The content of this report Is for
the sole benerit of the ownerlisted above. Any reliance upon or use of this report by any
other person or party is not authorized• nor will it confer any legal right whatsoever.
