Approval (COSA) based only upon the representations given in paragraph 4 by an independent professional civil
engineer registered in the State of Alaska. Certificates of On-Site Systems Approval are required for the transfer
of title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or
water supply system. DSD also issues COSAs upon request to homeowners. Certificates of On-Site Systems
Approval are valid for 90 days from the date of issue for properties served by a private or Class C well and may
be reissued with new water samples. (Certificates may be reissued for a period of up to one year with valid water

In conducting this evaluation, GEG, LtD. attempted to provide a thorough,
conscientious engineering analysis of the system in accordance with ADEC and MOA
DSD Guidelines & Regulations. The reported results described the performance of the
system under the conditions encountered at the time of the test, and separation
distances measured to readily identifiable features. The operational life of all wells and
septic systems depend on the local soils condition, groundwater levels that may
fluctuate during the year, and the water usage of the family being served by the system.
These conditions are outside the control of the evaluator of the system. Satisfactory test
results do not guarantee future performance of the system, nor do they guarantee that
there are no hidden defects or encroachments° GEG, LTD. can therefore not provide
any warranty or future estimate dhow long the system will continue to meet the
operational requirements of the ADEC or MOA DSD. The content of this report is for
the sole benefit of the owner listed above. Any reliance upon or use of this report by any

Ref: Valli Vue S/D #1: Block 3, Lot 2; Total Depth and Adequacy Test;
Mr. Poet,
The existing house is served by a community well and a private septic system. Per the MOA
records, the trench has an effective depth of 7 feet. During our inspection (6/7/2011) the
monitoring tube was found to be a 6 inch sump (from the original installation) with a 2 inch steel
pipe driven inside it. It is assumed that the steel tube was driven at some point as a repair.
However, the steel tube only extends 32 inches into the effective depth. Therefore, the total depth
listed on the C.O.S.A. sheet is calculated.
We contacted you during our adequacy test. Per our conversation with you (6/7/2011 Jeff Poet),
we introduced 2000 gallons into the drainfield over a period of 420 minutes. The liquid level
never rose high enough to be observed in the monitoring tube. Based upon this data, we believe
this system is functioning adequately for a 4 bedroom household.
Based upon the aforementioned facts, we request that you approve a C.O.S.A. for the property in
its current configuration.

conscientious engineering analysis of the system in accordance with ADEC and MOA
DSD Guidelines & Regulations. The reported results desc#bed the performance of the
system under the conditions encountered at the time cf the test, and separation
distances measured to readily identifiable featuraa. The operational life of ali wefts and
septic systems depend on the local soils condition, groundwater levels that may
fluctuate during the year, and the water usage of the family being sen, ed by the system.

TEST PROCEDURE: System wab inspected and measured.
Tank was
found with 2.5 feet of cover and ~ith a liquid level of 47 inch-
es. Trench clean out was 3 feet deep and dry. Trench 6--inch sump
was 5 feet deep and dry, bottom of sump was 27 inches below
trench cleanout.
1000 gallons of clean water was added to the trench cleanout. No
water showed up at the sump, and no water backed up into clean-
out. An additional 200 gallons were added directly to the sump.
No water accumulated.
TEST RESULT: This system meets the code requirements of
the Health and Social Services Department of the Municipality of
Anchorage.
NOTE The operational life of att septic systems depends on the
local soil conditions~ groundwater levels that may fluctuate
during the year, and the water usage e~ the ~amily being served
by the system. These conditions are eutside the control of the
evaluator o~ this septic system. We can therefore not give any
estimate of how long this system will ~unction saris-Factory ~or
