As certified by my seal affixed hereto and as of the validation date shown below, I verify that my investigation,
based on procedures outlined in the Certificate of On-Site Systems Approval Guidelines for this application,
shows that the on-site water supply and/or wastewater disposal system is (are) safe, functional and adequate
for the number of bedrooms and type of structure indicated herein. I further verify that based on the information
obtained from the Municipality of Anchorage files and from my investigation and inspection, the on-site water
supply and/or wastewater disposal system is(are) in compliance with all applicable Municipal and State codes,
ordinances, and regulations in effect at the time of installation.

4. From October 15 to April 15, a subsurface soil absorption system under construction during freezing weather

B. Covered, sealed, and heated to prevent freezing.
5. The following special provisions.

Ref: Septic System Upgrade for Lot 3B-1, Block 1, Fischer Subdivision
To whom it may concern:
The existing 3 bedroom house is served by a private well and septic system. The existing septic
system is in a state of failure and needs to be replaced. A test hole was excavated on the north
end of the property. The proposed septic system will be designed around the 30 foot radius of
the test hole. We are proposing that a 1000 gallon septic tank and a five foot wide drainfield be
installed. Comments regarding the proposed design are summarized as follows:
1. SOILS: See the attached log which shows the soil classifications, groundwater monitoring,
and the percolation test results. It is our opinion that due to the overall appearance of the soils, an
application rate of 1.2 gallons/day/f12 should be used.
2. TRENCH DESIGN:
a. Percolation Rate: <1 minutes/inch
b. Allowable Application Rate: 1.2 gallons/day/f12
e. Number of Bedrooms: 3
d. Design Flow: 450 gallons per day
e. Minimum Absorption Area: 375 ft2
f. Total Depth: 9.0 feet (max.)
g. Effective Depth: 4 feet
h. Width: 5 feet
i. Reduction Factor: 0.5
i. Minimum Length: 38 feet long

3. SURFACE WATERS: There are no surface waters within 100 feet of the proposed
upgrade.

4. TOPOGRAPHY: As can be seen on the attached topography site plan, the average
topography of this property is a 5 to 10 percent running from approximately north west to south
east; in short, there are no slope concerns. The trench is to be installed parallel to slope contours.
5. WAIVERS: Due to the lack of useable area available for this septic system upgrade we
are requesting waivers of separation distances: 1 foot from the north lot line and 5 feet from the
west lot line.
We are unaware of any adverse impacts this installation would have on adjacent wells or septic

gal Septic Tank.
While excavating the existing sewer line due to a blockage, it was
discovered the existing 1000 -gallon tank, which has been in place for
18 years, is substantially corroded and in need of replacement.
The homeowner proposes replacing the existing 1000 -gallon septic tank
with a new 1250 -gallon septic tank manufactured by Anchorage Tank and
Welding, and, in addition, replacing the existing sewer line between
the tank and seepage pit.
All proposed work, new materials and clean -outs, and separation
distances shall be in accordance with the provisions of AMC Chapter
15.65 currently in affect at this time.

Thank you for letting us respond to Allen and Kathryn Baker's claim.
From the report by SKLH Consultants (I'm assuming Steve Henslee is the
ENGINEERING STUDIES author, since I did not receive Page 3 of 3 with your letter) it
AND REPORTS appears the deficiencies found at the above address include some
corrosion damage to the septic tank and sludge blockage within the
sewer lines.
WELLINSPECTION We were not contracted to inspect either of these items, nor were we
&FLOW TEST required to do so in order to obtain the Municipal Health Certificate
dated November 5, 1991.
The only feasible way to determine theintegrit'�q, a buried septic
SITE PLANS tank is to uncover it for inspection. If-H9ltt�-has a method for
determining septic tank integrity without excavation, we would be
grateful to know the secret. I suspect the Municpal Health Department
would be equally interested so they could incorporate the inspection
into their Health Authority Approval requirements.
ROAD DESIGN
The extent of our testing was an Absorption Capacity Test (adequacy
test) performed on the leachfield. The only way to detect sewer line
blockage while performing an adequacy test is to add water to the
SOIL TEST system through the septic tank. We consider this to be poor practice
and advise against it for the following reasons:
1. If a large quantity of water is added rapidly into the septic
PERCOLATION tank, as would be needed for adequacy testing, there is a risk
TEST of churning the septic tank contents to the point of allowing
solids to discharge into the leachfield. The addition of
solids from a septic tank into a leachfield is the primary
cause of leachfield failure. Therefore/ in an attempt to test
STRUCTURAL& for line blockage you could be damaging the leachfield.

2. You could add water through the septic tank at a slower rate, but the
time it would take for an adequacy test would make it cost prohibitive.
Also, the results could be deceiving. There could be a partial
blockage within the sewer pipe which would allow water to pass slowly.
Therefo& if water is added slowly without adverse effect, you might
conclude there is no problem when there might actually be a partial
blockage.
There is something about the SKLH report that has me confused. They performed
an adequacy test on the system. They describe a procedure which was identical
to ours. They collected data from the seepage pit as did we. They added water
"directly into the pit" as did we. Their results were consistent with ours.
All this was done without performance of any function which would have detected
any blockage within the sewer lines. Arca yet, they state at the bottom of Page
Two, "I am unable to identify any reasonable scenarios which might explain why a
blockage of such magnitude would not have been evident during an adequacy test."
I started to contact SKLH to ask if I was somehow confused or was their letter
contradictory. However, I refrained for two reasons. The first being, that
when I contacted Steve Henslee on June 16, 1992 to discuss some rumors I'd heard
about problems on this property he said he was not at liberty to discuss the
property with me. The second reason is that if Mc. Henslee was interested in
any sort of professional courtesy communications, he would have contacted me
prior to completing his report.
n At any rate, the deficiencies found on the referenced property are not the type
f' which would be detected by typical inspections and testing performed to obtain a
' Health Certificate. If the situation was as poor as indicated in the—HS(F�S�f1
nlv�report, I suspect the previous owners would have experienced periodic "flooding"
of the home prior to the Buher's purchase. I recommend you inspect the Seller's
1 Disclosure Statement (if one exists) arca contact the sellers, if you haven't
done so already.
Near the conclusion of your October 8, 1992 letter you requested that we forward
the name of our Errors and Omissions insurer or bond holder to you.
Unfortunately we do not have Errors and omnissions insurace or a bond holder.

I represent Allen and Kathryn Buher with respect to the
purchase of their home at 5140 Spruce Creek Circle. The septic
system was approved for a three bedroom home by your engineering
firm on November 5, 1991. 1 am enclosing a copy of the
Certificate of Health Authority Approval for Single Family
Dwelling. The paper work indicates an inspection October 25,
1991.
It is a position by the Buhers that the test performed by
your engineering firm was done negligently and did not reflect
the status of the septic system prior to purchase. As a result
of the septic system being defective (rather than being in good
working order) the Buhers experienced flooding in their home on a
regular basis. At the time of the inspection the septic system
was in need of repair or replacement. An engineers report dated
June 9, 1992 from SKLH Consultants details the problem which
indicate that the system needed extensive repair.
As a result of the negligent inspection by your engineering
firm, the Buhers were damaged to the extent of $4,045.01 plus
attorneys fees and costs incurred.
The purpose of this letter is to formally make a claim apon
your firm for that amount and attorneys fees.

mined by cutting the line in half. Slutce was also present at tiie tank
outlet. Apparently, the sludge .filled the entire length of the line
which is approximately 22 feet in leng.h. In addition, n e existing
tank was found to be substantially c:'rroded and in dire reed of re-
placement. The manhole lids had several corrosion holes, the baffle
separating the first and second camoe.rtments was lose, and there was
some evidence of leakage at the tank outlet. Based upon what was found
it was determined that both the existing line and the existing 1000-
gallor+ tank needed to be replaced. The tank and line was replaced in
their entirety on 5 June 1??2.
ADEQUACY TEST OF THE EXISTING SScPAGE PIT
Due to the problems you have been having with the septic systemand the relatively short time you have owned the house, you also au-
thorized me to perform an adequacy test on the existing seepage pit to
determine its performance.

the morning of 5 June 1992. Prior to
performing the test, I measured 1.3

seepage pit which leaves 4.7 feet
of free absorption
area as the pit
is six (6) feet in total depth.
After recording the

As the purpose of the adequacy test is to determine the quantity
of water absorbed in a 24-hour period, the water, level in the pit was
measured approximately 24 hours later on 6 June 1992. The water level
was found to have dropped 1.1 feet for a final depth of 1.7 feet. A
drop of 1.1 feet corresponds to approximately 532 gallons absorbed.
The required absorption rate for a 3 -bedroom house in a 24-hour period
is 450 gallons. Based upon the results of the adequacy test, it is my
professional opinion that the existing seepage pit meets the perfor-
mance requirements established by the Municipality of Anchorage.
OBSERVATIONS AND CONCLUSIONS
As previously mentioned the existing septic tank was in need of
replacement due to internal c=-rrosion. A cursory inspection of the ex-
terior of the tank showed that the exterior was in relatively good
condition considering that it had been buried for approximately 1B
years. The corrosion present in the tank's interior is relatively con-
sistent with expectations, as the interior of the tank, especially the
area above the water level, can be very corrosive. However, the corro-
sion may also have been accelerated by not pumping the tank at regular
intervals as there was 1 to 2 inches of sludge adhering to the tank
walls and the tank baffle.
Finally, in regards to the testing done on the system last Octo-
ber 1991, there is no information available for roe to draw any opin-
ions concerning -the testing used to evaluate the functionality of the
system or whether there was any evidence of a possible problem with
the system prior to your purchase. However, I era also unable to iden-
tify any reasonable scenarios which might explain why a blockage of
such magnitude would not have been evident during an adequacy test.

Approval (HAA) based only upon the representations given in paragraph 4 by an independent professional civil
engineer registered in the State of Alaska. Certificates of Health Authority Approval are required for the transfer
of title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or
water supply system. DSD also issues HAAs upon request to homeowners. Certificates of Health Authority
Approval are valid for 90 days from the date of issue for properties served by a private or Class C well and may
be reissued with new water samples. (Certificates may be reissued for a period of up to one year with valid
water samples.) Certificates are valid for one year for properties served by Class A or B wells or a public water
system. The Municipality of Anchorage is not responsible for errors or omissions in the professional engineer's

In conducting this evaluation, GEG, Ltd. attempted to provide a thorough,
conscientious engineering analysis of the system In accordance with ADEC and MOA
DSD Guidelines B Regulations. The reported results described the performance of the
system under the conditions encountered at the time of the test, and separation
distances measured to readily Identifiable features. The operational life of all wells and
septic systems depend on the local soils condition, groundwater levels that may
fluctuate during the year, and the water usage of the family being served by the system.
These conditions are outside the control of the evaluator of the system. Satisfactory test
results do not guarantee future performance of the system, nor do they guarantee that
there are no hidden defects or encroachments. GEG, Ltd. can therefore not provide

Dear Mr. Williams:
Per Cher Brown's request of September 16, 1980, Dan Cooper of R&M
Consultants conducted a test of the sanitary sewer system on the above
described property.
The test indicates acceptance rates determined under conditions at the time
tested. Actual system performance over long periods of time depend on
factors which cannot be evaluated by this test thus our office cannot
warrant the suitability or fitness of the system for either an extended period
of time or for user demand in excess of the expected flow noted herein.
Factors affecting system performance include:
Actual use demand on the system;
Fluctuations in groundwater levels;
Physical conditions of the septic tank, leach field, trench or
seepage pit and soil.
It should be noted that while a septic tank and leach field disposal system is
one of the most reliable methods of sewage treatment and disposal it is
nearly certain that the leach field or seepage pit will fail sometime during
the useful life of the structure. Studies indicate leach fields, trenchs and
pits have a life expectancy of ten to twelve years under optimum conditions.
All septic systems have a finite hydraulic loading capacity which can be
expected to decrease with time.
Because the house on the lot is occupied, we assume that the seepage pit
was at its normal degree of saturation. During this test the liquid levels in
the septic tank and seepage pit were monitored as water was added to the
system. The measurements are summarized in the following table:

The meter used during the test was a Rockwell 5/8" standard water meter
which had previously been calibrated by R&M Consultants, Inc.

The total daily load for the three bedroom residence is four -hundred fifty
(450) gallons; surge capacity is three -hundred sixty (360) gallons.
The Municipality requires the system to accept a surge capacity of 80% of the
total daily load at a rate of ten (10) gallons per minute and to accept the
total daily load plus fifty (50) gallons in twenty-four (24) hours. Systems
must accept the above loads on two consecutive days.
Failure Criteria is as follows:
1. Water rises excessively in the system at or before 80% of total load
added.
2. Effluent overflows system.
3. Water level does not return to within one inch of initial static level.
During both days of testing, the system accepted five -hundred (500) gallons
of water and returned to the original static level by the next day.
We can therefore conclude that the system is disposing of effluent at an
adequate rate for a three bedroom residence.
