All construction must be in accordance with:
1. The attached approved design.
2. All requirements specified in Anchorage Municipal Code Chapters 15.55 and 15.65 and the State of Alaska
Wastewater Disposal Regulations ( 18AAC72 ) and Drinking Water Regulations ( 18AAC80 ).
3. The engineer must notify DHHS at least 2 hours prior to each inspection. Provide notification by calling
(907) 343-4744 ( 24 hours ). ( Not required for a Water Supply Permit only ).
4. From October 15 to April 15, a subsurface soil absorption system under construction during freezing weather
must be either: A. Open and closed on the same day.
B. Covered, sealed, and heated to prevent freezing.

Ref: Well and Septic Design for Gudveina Estates; Lot 1
(Revision of permit # SW000377)
To whom it may concern:
The existing 4 bedroom house is currently served by a private well and septic system. The
existing drainficld is in a state of failure and in need of an upgrade prior to the sale of the house.
During the installation of the previously permited design, groundwater was encountered
preventing the installation. A revision of the design is now necessary. One test hole was
performed by Leroy C. Reid Jr. P.E. on 9/10/86 on the cast portion of the property. The
proposed septic system will be designed within the 30 foot radius of this test hole. We are
proposing that a 1500 gallon S.T.E.P. tank and a five foot wide drainficld be installed.
Comments regarding the proposed design are summarized as follows:
1. SOILS: See the attached log which shows the soil classifications, groundwater monitoring,
and a visual soil rating of 125 ft2 per bedroom. A percolation test will be performed on the day of
construction to confirm the rate of absorption.
2. TRENCH DESIGN:
a. Allowable Application Rate: 1.2 gallons/day/ft2
b. Number of Bedrooms: 4
c. Design Flow: 600 gallons per day
d. Minimum Absorption Area: 500 ft2
e. Total Depth: 7 feet (max.)
f. Effective Depth: 4 feet
g. Width: 5 feet
h. Reduction Factor: 0.5
i. Minimum Length: 52 feet long
j Effective absorption area = 520 ft2

3. SURFACE NVATERS: There are no surface waters within 100 feet of the proposed
upgrade.
4. TOPOGRAPHY: As can be seen on the attached topography site plan, there are no slope
concerns.
We are unaware of any adverse impacts this installation would have on adjacent wells or septic

Dear Mr. Marx:
We are submitting herewith the test boring results and our comments re-
garding soil conditions encountered at the subject site. This investigation
was performed in accordance with your verbal instruction of May 23, 1972
and those procedures outlined in a letter dated September 13, 1971 by Mr.
Rolf Strickland of the Greater Anchorage Area Borough Department of En-
vironmental Quality.
A single test hole was put down within the Lot 1 area for the purpose of
defining general subsurface soil conditions. Excavation was accomplished
with a tractor -mounted backhoe and the test hole was extended to a total
depth of 11.5 feet. One sample was recovered from the bottom of the
test hole and retained for grain size analysis. The final log prepared for
the test hole has been included in Drawing A-01.
The test hole is considered to represent the material lying within the lot
boundaries. The soil profile disclosed by the -test hole indicated six inches
of organic material with silt overlying a brown organic silt which extended
to a depth of 2 feet below ground surface. At this depth, a gray silty sandy
gravel (GM) was encountered and extended to a depth of 4.0 feet below
ground surface. Below this depth, a gray sandy gravel (GW) was found and
continued to the bottom of the hole. The soils were visually classified in
accordance with the Unified Classified System.
Ground water was not encountered in the test hole.

Approval (COSA) based only upon the representations given in paragraph 4 by an independent professional civil
engineer registered in the State of Alaska. Certificates of On -Site Systems Approval are required for the transfer
of title (except between spouses) for properties served by a single-family on-site wastewater disposal andlor
water supply system. DSD also issues COSAs upon request to homeowners. Certificates of On -Site Systems
Approval are valid for 90 days from the date of issue for properties served by a private or Class C well and may
be reissued with new water samples. (Certificates may be reissued for a period of up to one year with valid water

In conducting this evaluation, GEG, LtD. attempted to provide a thorough,
conscientious engineering analysis of the system in accordance with ADEC and MOA
DSD Guidelines & Regulations. The reported results described the performance of the
system under the conditions encountered at the time of the test, and separation
distances measured to readily identifiable features. The operational life of all wells and
septic systems depend on the local soils condition, groundwater levels that may
fluctuate during the year, and the water usage of the family being served by the system.
These conditions are outside the control of the evaluator of the system. Satisfactory test
results do not guarantee future performance of the system, nor do they guarantee that
there are no hidden defects or encroachments. GEG, LTD. can therefore not provide
any warranty or future estimate of how long the system will continue to meet the
operational requirements of the ADEC or MOA DSD. The content of this report is for
