Ref: Amendment to SW010157 - Septic System Design for Lot 7, Heritage Heights Subdivision
To whom it may concern
The undocumented drainfield on the lot was exposed to confirm size adequacy and was
determined to be less than sufficiant for the existing house. Test holes were excavated on the
property to determine an area suitable for a drainfield. We are proposing that a new septic system
will be designed around the 30 foot radii of test hole #1 and #2. We are proposing that a 1250
gallon S.T.E.P. tank and a deep trench type drainfield be installed. Comments regarding the
proposed design amendment are summarized as follows:
1. SOILS: See the attached logs which shows the soil classifications, groundwater monitoring,
and the percolation test results. It is our opinion that due to the overall appearance of the soils, an
application rate of 1.2 gallons/day/ft2 should be used.
2. TRENCH DESIGN:
a. Percolation Rate: <1 minutes/inch
b. Allowable Application Rate: 1.2 gallons/day/ft2
c. Number of Bedrooms: 3
d. Design Flow: 450 gallons per day
e. Minimum Absorption Area: 375 ft2
f. Total Depth: 12.0 feet (max.)
g. Effective Depth: 8.0 feet
h. Width: 2.04- feet
i. Reduction Factor: N/A
i. Minimum Length: 25 feet long

3. SURFACE WATERS & WELLS: There are no surface waters within 100 feet of the
proposed septic upgrade. All of the lots in Heritage Heights Subdivision are served by public

· water. The properties to the east are served by private wells, none of which are within 100 feet
of the proposed septic upgrade.
4. TOPOGRAPIIY: As can be seen on the attached topographical drawing, the lot has a general
slope of about 10 to 20 percent, from east to west, in the area of the proposed upgrade. There
does not appear to be any slope concerns.
We are unaware of any adverse impacts this installation would have on adjacent wells or septic

Ref: Septic Tank Upgrade and Documentation of "Bootlegged" Seepage Pit for Lot 7, Heritage
Heights Subdivision.
To whom it may concern:
AWWC was retained to evaluate/test the septic system on the subject property and obtain a
Municipal health authority approval. During our evaluation it became apparent that the
drainfield location was inconsistent with the location shown on the DHHS records (dated
10/I 1/76). It was then brought to our attention that the septic system had been upgraded only
several years earlier. We were given the name of the contractor so we could obtain information
regarding the work that was done and whether it was a permitted installation. After several failed
attempts by the contractor to locate the engineering paperwork, he remembered that there was no
engineering paperwork. In short, the septic system installation was undocumented".
According to the installer "Chucks Backhoe", the upgrade consists of two concrete seepage rings,
one stacked on top of the other, surrounded by drainrock. The perimeter of the system is
supposedly 12 feet by 12 feet. We measured the depth to the bottom of the seepage pit (through
the clean-out) and determined it to be approximately 18 feet below grade. On 5/17/01 Discovery
Drilling augured a hole (within the 30 foot radius of the seepage pit) to a depth of 25 feet and
installed a slotted monitoring tube. The soils were Iogged....no groundwater or impermeable
soils were encountered. Seven days later (5125101) the monitoring tube was still dry. In short, it
appears that the seepage pit does not encroach upon bedrock, impermeable soils, or groundwater.
Our proposed course of action is as follows:
The septic tank (made ofsteel and installed in 1976) was not replaced. Given the age of the
tank, it is virtual certain that it is structurally unsound. We are proposing to replace the septic
tank with a new 1000 gallon tank and place it outside the driveway area.
· Have an excavator expose the perimeter of the seepage pit so that we can confirm its size.

Perform a percolation test in the natural soils adjacent to the seepage pit.
If it is confirmed that the seepage pit is adequately sized for the number of bedrooms (three),
we will prepare as-built documentation of the system. If it is determined that the system is
inadequately sized, we will prepare a design and submit it to your department for review and
approval.
SURFACE WATERS & WELLS: There are no surface waters within 100 feet of the proposed
septic tank upgrade or the undocumented seepage pit. All of the lots in Heritage Heights
Subdivision are served by public water. The properties to the east are served by private wells,
none of which are within 100 feet of the proposed septic tank or the undocumented seepage pit.
TOPOGRAPHY: As can be seen on the attached topographical drawing, the lot has a general
slope of about 10 to 20 percent, from east to west, in the area of the undocumented seepage pit.
There is a moderate cutbank (34 feet) on the east side of the driveway; however, given the depth
ofthe seepage pit, there is less than 25% slope from the top of the concrete rings to the bottom of
the subject cutbank. There is a steep cutbank to the west of the driveway. Again, due to the
deepness of the seepage pit there does not appear to be any slope concerns.
I am unaware of any adverse impacts this installation would have on adjacent wells or septic

Dear Mr. Ferrell:
On June 4, 2001, this office issued a permit to install a new septic tank for this property.
As part of the review process, we discovered an additional undocumented wastewater
disposal field.
After further investigation we received evidence that you constructed the wastewater
disposal field approximately two years ago. Our files have no documentation of any soil
tests, engineering design or inspections on the disposal system. Our records also
indicate that this is not the only time your company has constructed wastewater
disposal systems without a permit required by Anchorage Municipal Code. Construction
of a wastewater disposal system without a permit is a violation of AMC15.65.030, A.
Enclosed with this letter is a Uniform Summons and Complaint. This document requires
you to appear before a magistrate at 303 K Street within 5 working days from receipt of
this citation.
You are hereby notified that any further construction of wastewater disposal systems
without the required permits within the Municipality of Anchorage will result in a
disqualification of your firm from being an approved excavator to install these systems
within the MOA.

up to one year with valid water samples.) Certificates ara valid for one year for properties served by Class A or B
wells or a public water system. The Municipality of Anchorage Is not responsible for errors or omissions In the
