Ladies and Gentlemen:
I am writing to request a permit to upgrade the septic system for the above referenced property.
The owner would like to upgrade the system to for 5 -bedroom single-family residence. Soil
logs, design calculations, a site plan, design drawings and construction specifications are
enclosed for your review.
Design Calcs: No groundwater observed to a depth of 17.5 feet
below ground surface (8/24/2011)
Soil Rating. From Testhole 8/16/2011

Required Area per Bedroom: 150/ 1.0 =150 sq.ft.
Total area required: 150 x 5 =750 sqft
System: We are proposing a 1500 gallon septic tank, and a deep trench absorption field. The
absorption field will be 60 feet long and 2 feet wide and contain 7.5 feet (900 sq.ft. effective) of
sewer rock. The existing system will be connected to the new system via a bull run valve. The
drain field for the existing system has an effective absorption area suitable for a 5 bedroom
residence (based upon the soils analysis conducted in 1980.
Soils: A test hole was excavated on August 16th, 2011. See the attached soil logs. Ground
water was not observed. The percolation rate of the soils was less than one minute per inch. The
soils were predominately sand with some gravel. Silt lenses were observed starting at a depth of
10 feet. They were generally 6 inches to 12 inches thick and non -continuous along the test hole
walls. With a conservative application rate of 1.0 gallons per day and an effective absorption
area of 900 sq.ft., these silt lenses will not negatively impact the operation of the septic system.

Environmental Consulting and Design
Below a depth of 14 feet silt with sand was observed. This layer is not impervious and should
provide a sufficient filter to treat the effluent as it percolates through the soil column.
Surface Water: No surface waters were observed within one hundred feet of the proposed
septic system.
Topography: The lot in the area of the proposed septic system upgrade is generally level. The
ground surface elevation varies by +/- 1 foot over the length of the proposed trench.

The Municipality of Anchorage Development Services Department (DSD) Issues Certificates of Health Authority
Approval (HAA) based only upon the representations given In paragraph 5 by an Independent professional civil
engineer registered in the State of Alaska. Certificates of Health Authority Approval are required for the transfer
of title (except between spouses) for properties served by a single family on-site wastewater disposal and/or
water supply system. DSD also Issues HAAs upon request to homeowners. Certificates of Health Authority
Approval are valid for 90 days from the date of Issue for properties served by a private or Class C well and may
be reissued with new water sample results less than 30 days old. (Certificates may be reissued for a period of
up to one year with valid water samples.) Certificates are valid for one year for properties served by Class A or B
wells or a public water system. The Municipality of Anchorage is not responsible for errors or omissions in the
professional engineers work.
Note: Alaska Water and Wastewater Consultants, Inc. shall be pald $700.00 at, or prior
to closing for the engineering services provided.
4. STATEMENT OF INSPECTION BY ENGINEER
As certified by my seal affixed hereto and as of the validation date shown below, I verify that my
investigation, based on procedures outlined in the Health AuthorityApprcval Guidelines for this application,
shows that the on-site water supply and/or wastewater disposal system Is(are) safe, functional and adequate
for the number of bedrooms and type of structure Indicated herein. 1 further verify that based on the
Information obtained from the Municipality of Anchorage files and from my Investigation and Inspection, the
on-site water supply and/or wastewater disposal system Is(are) In compliance with all applicable Municipal
and State codes, ordinances, and regulations In effect at the time of installation.

In conducting this evaluation, AWWC, Ina attempted to provide a thorough,
conscientious engineering anaysis of the system In accordance with ADEC and MOA
DSD Guidelines & Regulations. The reported results described the performance of the
system under the conditions encountered at the time of the lest and separation
distances measured to readily identifiable features. The operational rife of all wells and
septic systems depend on the local soils condition, groundwater levels that may
fluctuate during the year, and the water usage of the family being served by the system.
These conditions are outside the control of the evaluator of the system. Satisfactory lest
results do not guarantee future performance of Me system, nor do they guarantee that
there are no hidden defects or encroachments. AV=, Inc. can therefore not provide
any warranty or future estimate of how long the system will continue to meet the
operational requirements of the ADEC or MOA DSD. The content of this report is for
the solo benofit of the owner listed above. Any reliance upon or use of this report by any
other person or party is not authorized, nor will it confer any legal right whatsoever.

To whom it may concern:
The water service line to the residence on the subject property was professionally located by
Terrasat, Inc. (See attached receipt). I performed a site visit on 3/23/01 to visually confirm the
separation distance from the water line to the septic system.
The service line runs from the key box and heads out under the driveway area. The line enters
the house near the right side of the driveway (standing in the driveway and looking at the house).
The measured distance from the edge of the sump clean-out to the closest locate mark (paint on
the ground) was 10'-4". The trench runs somewhat perpendicular to the water service line. The
actual separation distance from the edge of the drainrock, to the water service line (typically
locates are accurate to within 2 feet) is roughly 10 feet.

Dear Mr. Nix:
We are submitting herewith the test boring results and our comments regarding
soil conditions encountered at the subject iste. This investigation was performed
in accordance with your request of August 9, 1972 and those procedures outlined in
a letter dated September 13, 1971 by Mr. Rolf Strickland of the Greater Anchorage
Area Borough Department of Environmental Quality.
A single test hole was put down within the Lot 5 area for the purpose of defining
general subsurface soil conditions for the proposed sanitary system. Excavation
was accomplished with a tractor -mounted backhoe and the test hole was extended
to a total depth of 9 feet below ground surface. The final log prepared for the test
hole has been included in Drawing A-01.
Ground water was not encountered in the test hole,

Dear Mr. Leicht:
Per your realtor's request of June 30f 1980, we conducted a test of the
sanitary sewer system on the above described property.
The septic tank was pumped prior to the performance of the test on the
seepage pit. During the test the liquid level in the seepage pit was
monitored as 645 gallons were removed and 700 gallons were pumped back
into the seepage pit to the inlet level. All liquid levels were measured below
the top of the standpipe and are shown in the following table:

The water level rose 61.2 inches with the addition of 700 gallons of water
indicating a capacity of 11.44 gallons per inch. After twenty four hours the
liquid level was measured again at 5.3 feet. It had dropped 2.3 feet or 27.6
inches. This indicates an average effluent acceptance rate of ;U,6 gallons
per day for the surrounding soils. If the 5 bedroom residence on the
property is to house 10 people, the average load on the system can be
expected to be 750 gallons per day. We can therefore conclude that the
system is not disposing of effluent at acceptable levels to serve a typical 5
bedroom residence.
This test was performed on June 30, 1980 and incidates conditions at the
time tested. Actual system performance over long periods of time depend on
factors which cannot be evaluated by this test thus our office cannot
warrant the suitability or fitness of the system for either an extended time
period or for user demand in excess of the expected flow noted herein.

Factors affecting system performance include:
° Actual use demand on the system;
° Flucuations in groundwater levels;
° Physical conditions of the septic tank, leach field, trench or
seepage pit and soil.
It should be noted that while a septic tank and leach field disposal sytstem
is one of the most reliable methods of sewage treatment and disposal it is
nearly certain that the leach field or seepage pit will fail sometime during
the useful life of the home. Studies indicate leach fields, trenchs and pits
have a life expectancy of ten to twelve years. All septic systems have a
finite hydraulic loading capacity which can be expected to decrease with
time.
