Gentlemen:
My firm was contacted to investigate the possibility of installing of a new system at the
above referenced property. Currently the lot is undeveloped. A single test hole was
excavated on January 8, 2000. The soil report and percolation test results are attached.
Ground water was monitored for seven days. No ground water was encountered or
monitored. No bedrock was encountered in the test hole.
The lot is approximately 1.2 acres in size. Lot 13 slopes to the west at a rate of
approximately 8 to 14 percent The proposed installation will be located in the center of
the lot on a flat bench The proposed location will be greater than 100 feet away from the
proposed well serving this property and 25 feet from the water service lines. The
surrounding systems are located greater than 200 feet from the proposed installation,
the lots to the north, south and east are developed, but do not conflict with the proposed
installation. The proposed installation shall not impact the future development of the
surrounding or existing lots. See attached design. The new system will maintain over
six feet vertical separation to the bedrock and over four feet vertically to the ground
water. The mwner is an experienced excavator, and plans to install the system himself.

To Whom It May Concern,
I was asked by Brent Western of First Water to address some concerns raised about our well and septic.
Well Manhole—The well manhole was installed over well compacted Type IIA material and a 1-
%" flexible drain pipe was installed from the manhole floor to the 4" foundation drain on the
house. The foundation drain daylights on the Southwest corner of the house in the grove of
spruce trees to the right of the driveway. The concrete surrounding the manhole is 5" thick with
rebar and woven wire mesh with 4" of rigid foam insulation just under the concrete. The entire
slab is heated. The lid for the manhole is the highest point (the concrete slopes away from the
manhole) and has a full rubber gasket. We have never had any issues with water entering into
the manhole in the +-22 years we have been in the home.
2. Septic System —The parking area over the septic system has generally existed since we built the
home. The septic has never frozen in those +-22 years. There have been no frost issues with the
septic system. I will forward pictures of the large boulders when they are placed around the
septic tank.

Starting at 20 years of age the MOA issues Advisory's for steel septic tanks The septic tank for
this COSA / property is 22 nears old. A leaking septic tank may be a source of contamination to
the aquifer. Typical replacement costs range from $10,000 to $15,000
This advisory must be attached to all copies of the subject Certificate of On -Site Systems
Approval.
This is an example of a 16 -year-old septic tank in failure and should be replaced.

To Whom It May Concern,
I was asked by Brent Western of First Water to address some concerns raised about our well and septic.
1. Well Manhole —The well manhole was installed over well compacted Type IIA material and a 1-
%" flexible drain pipe was installed from the manhole floor to the 4" foundation drain on the
house. The foundation drain daylights on the Southwest corner of the house in the grove of
spruce trees to the right of the driveway. The concrete surrounding the manhole is 5" thick with
rebar and woven wire mesh with 4" of rigid foam insulation just under the concrete. The entire
slab is heated. The lid for the manhole is the highest point (the concrete slopes away from the
manhole) and has a full rubber gasket. We have never had any issues with water entering into
the manhole in the +-22 years we have been in the home.
2. Septic System —The parking area over the septic system has generally existed since we built the
home. The septic has never frozen in those +-22 years. There have been no frost issues with the
septic system. I will forward pictures of the large boulders when they are placed around the
septic tank.
