4. From October 15 to April 15, a subsurface soil absorption system under construction during freezing weather must either:
A. Open and Close on the same day.
B. Covered, sealed, and heated to prevent freezing.
Special Provisions: The existing 4 bedroom house must be demolished before the inspection
report will be approved.

Septic System Permit Upgrade Request
Ladies and Gentlemen:
I am writing to request a permit to construct an upgraded septic system be issued for this property. The
proposed systems will serve a new three-bedroom house. Currently the lot is developed. The existing house
will be demolished and a new house constructed. The lot is served by a private water supply. The existing
septic system is in failure and will be reused. A new drain field will be installed south of the existing field.
The surrounding developed lots are served by private water systems. The private wells are located over 100
feet from the proposed soil absorption system.
1. Soils. A single test hole was excavated by A+ Home Services in September of 2010. See the attached
soil log. Ground water was monitored for seven days. No ground water was monitored to a depth of 18 feet
below ground level after the monitoring period. Bedrock was not encountered in the test hole. It is my
opinion, based on the results of the percolation tests and overall soils appearance; an application rate of 1.2
gallons/day/square feet should be used, using a conventional wastewater treatment system.
Soil Absorption System Design.
See Design Drawings sheet 2 of 3 for design calculations.
3. Surface Water: There is no surface water within 100 feet of the proposed septic tank and the
existing drain field. The proposed drain field upgrade will maintain at least 100 feet from all surface water
and drainage ditches.
4. Topography: The average topography in the area of the proposed septic system is approximately 2
percent based on the survey information. The proposed installation will be greater than 50 feet from the
shoulder of the steep slopes.
5. Drawing Markings: The Drawings are marked "For MoA Review Only". When written notification
that the review is complete and that there are no further comments is received from MoA On-Site

The proposed installation will not affect the future development of the surrounding or existing lots. There
are no wells or septic systems within 100 feet of the proposed septic location.

Dear Mr. Besse:
At your request of August 18, 1977, we conducted a subsurface soils
investigation at the proposed location of the sanitary sewer system on
the subject lot. The investigation complied with those procedures
required by the Municipality of Anchorage Department of Health and
Environmental Protection.
This investigation, which was accomplished on August 22, 1977, consisted
of a test hole drilled to a depth of 17.5 feet below the existing ground
surface. The test hole was sited according to your instructions and its
location is shown in attached Drawing A-01. Drilling was accomplished
with a rotary drill rig using continuous flight solid-stem auger with an
outside diameter of 6 inches. A sample was taken at the depth shown on
the soils log in Drawing A-01. The sample will be held in storage in
our lab for approximately six months. In addition, all material brought
to the surface by the augers was continuously monitored by an experienced
engineering geologist.
The topography at the drilling site is generally rolling. At the time
of the investigation the site had original vegetation consisting of
grass. The top of the test hole was located at original ground surface.
The soils encountered in the bore hole are shown in the test hole log in
Drawing A-01. The symbols used in Drawing A-01 are explained in Drawings
B-01, and B-02. This log displays specific conditions encountered at
the test location. However, subsurface conditions may vary in other
parts of the lot without any apparent surficial evidence of the change.
Groundwater was not encountered in the test hole. Bedrock was not
encountered. At the time the hole was drilled seasonal frost was not
present and permafrost was not encountered.
Based on the visual classification of the soil and the requirements set
forth by the Municipality of Anchorage, a percolation test was not
necessary within the test hole on the subject lot.

CLASSIFICATION! CONSISTENCY AND SYMBOLS
CLASSIFICATION: Identification and classification oE the so~ is accomplished in
accordance with the Unified Soll Classification System. Normally, the grain size
distribution determines classification of the soil. The soft is defined according to
major and minor constituents with the minor elements serving as modifiers of the
major elements. For cohesive soils, the clay becomes the principal noun with the
other major soll constituents used as modifier; i.e. silty clay, when the clay particles
are such that the clay dominates soft properties. Minor soil constituents may be
added to the classification breakdown in accordance with the particle size proportion
listed below; i.e. sandy silt w/some gravel, trace clay.

SOIL CONSISTENCY - CRITERIA: Soll consistency as defined below and determined
by normal field and laboratory methods applies only to non-frozen material'. For
these materials, the influenc~ of such factors as soll structure, i.e. fissure
systems, shrinkage cracks, sllckensldes, etc., must be taken into consideration
in making any correlation with the consistency values listed below. In permafrost
zones, the consistency and strength of frozen soils may vary significantly and
unexplainably with ice content, thermal regime and soll type.
