Owner Majestic Valley Estates S/D, Block 4, Lot 4 26008 Imperial Drive Eagle River, Alaska PERCOLATION TEST RESULTS AND GENERAL SITE INVESTIGATION REPORT On July 20, 2012, an adequacy test was performed on the well on the above -referenced 54,000' property. 547 gallons of water was introduced into the septic system for a total of 4 hours. The well passed with a 2.3 GPM. An adequacy test on the septic system was being peformed simultaneously with the well test. During the course of the test it was found that no water was getting to the monitoring tube at the far end. Water was then introduced into the sump/monitoring tube. To make sure water was not coming above the inverted pipe, a stick with a long nail on the end was inserted into the pipe, which showed that water was above the invert of the pipe. No holes could be felt in the lower portion of the sump pipe. It was decided that an excavator should examine the end of the leach field to see if there were any holes or if they were clogged up. On 7/27/12, the excavator, Dean Construction, dug at the end of the existing trench. Your office was contacted by phone at the site when it was realized that both the monitoring tube and drain pipe were clogged with roots from a cottonwood tree located approximately 3' from the leach field, and the drain pipe across the leach field was crushed and not even conncted to the monitoring tube for a distance of 1-1/2'. The drain pipe was extremely thin pipe and the filter fabric was just visqueen. It was also found by your office, as they checked records while on the phone, that the septic system had been installed contrary to the Municipality's permit instructions that stated the effective depth must be 6' instead of 4' as originally requested. See Well and On-site Sewer Permit 4810181 issued by John Kennedy 4/20/81. Please note that on the copy on file at the MOA the figure 4 is scratched out and a 6 is inserted. Your office directed that a new system must be installed to meet the effective depth requirement of 6'. Because of the limited space, we requested approval to remove the old sewer rock and replace it with new rock and Jay Crewdson at M.O.A. stated that it would be necessary to dig a test hole to 16' deep with a 7 -day monitoring tube installed, perform a perc test, and submit the required paperwork for a new system. The test hole was dug to 16' on 7/27/12 and no ground water was found. A 7 -day monitoring tube was then put into the test hole. The ground level elevation was a little over a foot lower than the elevation of the existing trench. A perc test was done the same date with hour-long readings, the last three readings recorded, and a soil log, The perc rate was 4.7 minutes per inch resulting in a trench length of 32'. See attached design drawing. The lot slopes off greater than 25% 39' from the edge of the proposed leach field, which will require a slope setback waiver of 39'. The area is treed with spruce, cottomwood, birch, and alder with spruce the predominate. On-site observation and physical survey show that there are no water wells or private wastewater disposal systems within a 100' radius of the proposed system. There is a small, un -named stream that runs through the east end of the lot approximately 22' to the east of the well on the property, which makes it greater than 100' away from the proposed septic tank and drain field locations. According to the SGS report on the water sample obtained on 7/20/12, arsenic is ND, E. coli, total coliform is negative, Total Nitrate is 0.773. It appears that there is no potential for contamination of adjacent water wells or streams. The proposed system will have no measurable impact on reserve space, surface or subsurface, or on drainage to adjacent lots. Please consider this report as a request for review and approval of the attached design drawings for a replacement on-site wastewater disposal system.
