Health & Human Services Please find attached as -built data for the referenced on-site services. This includes the on-site wastewater disposal system and well inspection report, as - built elevations, a scaled site plan of these installations, the water well test results, and the water well log. Please note that absorption field size was increased to four lines due to an installation error noted during the site inspection of June 11, 1992. The trench widths were revised to meet the EPA Design Manual criteria for shallow beds by fax communication on June 5, 1992 between your office and our office. The contractor had constructed the three lines from an unrevised edition and accordingly, was revised in the field. The total available leach area is 300 square feet per the unrevised edition plus an additional 125 square feet for the additional lateral, for a total of 425 square feet. The pressure line from the S.T.E.P. to the leach field is protected with a minimum of 2.5 feet of ground cover, with 4 inches of rigid styrofoam insulation in the ditch. In addition, a bentonite/soil (native SILT) ditch plug was installed to prevent effluent from returning down the ditch line to the curtain drain and septic tank area. A curtain drain, 7 feet deep was installed as shown between the house and the leach field prior to septic system construction to dewater the home site and septic tank installation area. The leach field was located approximately 50 feet from the curtain drain, however, the actual system was constructed so that the separation distance varies between 42 and 53 feet from the field. General drainage along the curtain drain and from the leach area is to the south southwest, making the possible water path from the leach field to the drain on the order of 50 feet. In regard, we request that the curtain drain be approved as installed. The platted water well location is shown in the south west corner of the lot. The actual water well was installed on the west side of the lot in the location shown. The well was moved to this location due to drill rig access which included a flatter slope and relatively dry ground support conditions. The Page 2. M.O.A., D.H.H.S. September 15, 1992 proposed site was across a steep drainage ditch and in very soft, wet ground. At this time, there are no other existing wells and/or septic systems in this subdivision. The relocated well does not appear to conflict those platted well locations and septic set backs in borderings lots and in Tracts A and B. We request that this well be approved as constructed.
