Upgrade Septic Tank Permit Request Narrative
osP22t235
This is a design narrative for a permit to install a new septic tank on this property. The existing tank is 41
years old. The homeowner is desiring to replace the tank. The proposed system will serve an existing
three bedroom house. The existing septic tank will be removed and disposed of and a new septic tank
will be installed. Double cleanouts will be provided downstream of the tank.
l. Soils. The soil in this area is composed of sandy gravel. A test hole had previously been dug
during the month of June to a depth of 8'. Groundwater was encountered at 8' below grade.
Seasonal adjusted groundwater for the month of June is 7,.
2. Soil Absorption System. In June 2022,the eastern trench had 28" of water in it. The western
trench had 0" of water in it. This suggests that the splitter valve is not working effectively. This
will be corrected when the tank is replaced.
3. Wells. This lot and the surrounding lots are served by a public water system. There are no wells
within 200'of the proposed tank location.
Neighboring Wastewater systems. lmmediate neighboring septic systems are all 5, distant from
property lines.
Surface Water: There is no surface water within 1OO' of the tank. The proposed tank upgrade
will maintain at least 100'from all surface water and drainage ditches.
Topography: The existing topography in the area of the tank is flat with slopes less than 2% in
any direction within 50'of the tank.
The proposed installation will not affect the future development of this or the surrounding lots.

Backfilling of any system without final inspection and approval by this department
will be subject to prosecution.
Minimum distance between a well and-any-on-site sewage disposal system is 100 feet
for a private well or 150 to 200 feet from a public well depending upon the type
of public well. ' Minimum distance from a private well to a private sewer line
is 25 feet and to a community sewer fine is 75 feet. Well logs are required
and must be.returned to this'department within 30 days of the well completion.

In conducting an adequacy test, I attempt to provide a thorough, conscientious engineering analysis of the system in accordance with MoA COSA
guidelines and regulations. The reported results describe the performance of the system under the conditions encountered at the time of the test,
and separation distances measured to readily identifiable features. The operational fife of all wells and septic systems depend on the local soil
condition, ground water levels that may fluctuate during the year, and the water usage of the family being served by the system. These conditions
are outside the control of the evaluator of this system. All systems eventually fail and satisfactory test results do not guarantee future
performance of the system, nor do they guarantee that there are no hidden defects or encroachments. Therefore we cannot provide any warranty
for future performance, nor can we estimate remaining life of the system. The content of this report is for the sale benefit of the owner listed
