The owner has requested we proceed forward to obtain a septic permit to
upgrade the failed septic tank on the subject lot. The proposed upgrade will

The adjacent lots are served by private water as noted on the design. There is no
surface water within 100' of the proposed tank. We do not expect there to be any
adverse effect on adjacent lots by the development of this tank. If you have any

The Municipality of Anchorage Development Services Department (DSD) Issues Certificates of Health Authority
Approval (HAA) based only upon the representations given in paragraph 5 by an independent professional civil
engineer registered in the State of Alaska. Certificates of Health Authority Approval are required for the transfer of
title (except between spouses) for properties served by a single family on-site wastewater disposal and/or water
supply system. DSD also issues HAAs upon request to homeowners. Certificates of Health Authority Approval are
valid for 90 days from the date of issue for properties served by a private or Class C well and may be reissued with
new water sample results less than 30 days old. (Certificates may be reissued for a period of up to one year with
valid water samples.) Certificates are valid for one year for properties served by Class A or B wells or a public
water system. The Municipality of Anchorage is not responsible for errors or omissions in the professional
engineer's work.

Request a Health Authority Approval be provided for the residence
located on the referenced property. The following documents are
provided for your information:
A. Application for Health Authority Approval Certificate.
B. Health Authority Approval Checklist dated February 1984.
C. A drinking water analysis report for total coliform
bacteria.
The septic system serving the residence located on this property
failed an adequacy test. It is recommended that a soil test be
performed to determine the requirements for an upgrade of this
system. At the time the adequacy test was performed the water
in the crib was visible within the clean-out above the elevation
at the top of the crib. The septic tank was pumped and verified to
have a capacity of 1000 gallons. The crib had to be pumped and
approximately 1600 gallons of water was removed. Approximately
1000 gallons of water was then added back to the crib and after
a period of 24 hours the water level in the crib was higher than
measured the previous day. It is suspected that the water level
in this crib is being influenced by the water table in the
surrounding area. It may be that the upgrading of this system
will require a lift station and a Geepage bed or even a mound system.
Approximately three days after the completion of the adequacy test
the water level inthe seepage pit was again monitored and found to
be at the same/level. f

A well production test was performed on the well located on the
referenced property and was determined that the minimum sustained
yield of the well exceeded 0.104 gallons per minute per bedroom.
However, subsequent reduction in flow or failure of this well
cannot be guaranteed.
