All construction shall be in accordance with:
1. The attached approved design.
2. All requirements specified in Anchorage Municipal code Chapters 15.55 and 15.65 and the State of Alaska
Wastewater Disposal Regulations (18AAC72) and Drinking Water Regulations (18AAC80)
3. The wastewater code requires inspections during the installation. The engineer shall notify the Development
Services Department per AMC 15.65, Provide notification by calling (907) 343-7904 (24/7).
4. From October 15 to April 15, a subsurface soil absorption system under construction during freezing weather
shall be either:
a. Opened and Closed on the same day, or
b. Covered, sealed, and heated to prevent freezing
Special Provisions: Prior to any future COSA, all septic pipes for the 1994 field are to be located and extended
above grade.

SPECIFICATIONS & DESIGN GUIDELINES
Wastewater System Sizing: The current septic tank has failed- the field still functions.
Replace septic tank in same location, decommission old tank per UPC. This lot is near an
acre and on a water well. No adverse impacts are expected from tank replacement.
Easements are depicted on the lot. The slope is negligible in the area of the septic system.
No conflicts to neighbor properties.
Specification Requirements: All components and work must comply with the
Municipality of Anchorage Specifications (AMC) & State of Alaska Drinking Water
Regulations and Wastewater Regulations.
• New 2- compartment, 1000 gallon septic tank. Watertight couplings on inlet & outlet.
• 5' minimum between the tank and trench. 5' to property lines & 10' to house.
• 4' of cover or insulation is required for tank; an equivalent of 1" insulation for 1' foot
soil cover. Tank & solid pipe must be set on well compacted, stable soil.
• No large rocks for tank bedding. Plastic/fiberglass tanks require special bedding per
MOA- sand or pea gravel.
• 4" diameter cleanouts with airtight caps are required 1 ' to 4' from foundation wall,
prior to any 90 degree bend in 4 inch line, in 2" d tank compartment, and two adjacent
opposing cleanouts between the tank and the absorption field, not more than 10' from
the tank positioned to provide cleanout access towards the tank and towards the
absorption field. Manhole Riser required in 1't tank compartment.
• All cleanouts must extend to at least ground level.
• In solid pipe runs, ASTM D-3034 may be used in lieu of cast iron.
• Insulation must be placed over any pipe installed under driveways or parking areas.
• Approved pipe materials include cast iron, PVC ASTM D3034, PVC ASTM F789,
ABS ASTM D2661,
• Sewer Service Line is minimum 2% slope.
• Septic Tank to be pumped every two years or when required.
• Insulation board to be extruded direct burial polystyrene (Dow Styrofoam HI/equal)

Ref: Septic System Upgrade for Lot 2, Block 7,
Elmore S/D, Addn. #2
To whom it may concern:
Attached is the application, site plan, and design drawings
for the subject septic system upgrade. Comments regarding
the proposed system are as follows:
1. TRENCH DESIGN: As can be seen from reviewing the
attached percolation test results, done by Environmental
Management Incorporated (John Simpson, P.E.) the soil
"perked- faster than 1 minute/inch at the location proposed
for the system, therefore, a sand filter will be required.
I am proposing to use an application rate of 1 gpd/ft2.
Since the existing home has 3 bedrooms, the total design
flow is 450 gpd. Based upon this, the minimum amount of
absorption area is 450 ft2. The proposed trench is 5 fleet
wide, 6 inches deep and 90 feet long, providing an
absorption area of 450 ft2. A portion of the trench will be
outside the 30 foot radius of the test hole, however, based
upon soils data from the adjacent lots, it appears that
coarse gravel soils are present throughout this area.
Regardless, I will visually inspect the quality of the soils
during construction. If an additional percolation test will
be required during construction, please specify it on the
permit.
2. SURFACE WATER: None observed
3. TOPOGRAPHY: No slope concerns
4. GROUNDWATER: I monitored for groundwater, in the test
hole done by EMI, on 7/24/94, and found only the bottom of
the test hole to be slightly damp. I have specified that

accordance with the guidelines and regulations established by the Municipality of Anchorage and industry

Separation distances were measured to readily identifiable features. Hidden defects or encroachments may
exist that were not identified during the evaluation. The operational life of all wells and septic systems depend
upon a variety of variables, including (but not limited to) soil conditions, groundwater levels (that may fluctuate
during the year), quality of construction (materials and workmanship), and the water usage of the family utilizing
the system/s. These conditions can vary, and are outside the control of GEG. Satisfactory test results do not
guarantee future performance of the system/s; therefore, GEG makes no warranty (express or implied) regarding
the future performance of the well or septic system. GEG makes no representation whether an alternative well
or septic system can be installed on the property in the event either of the current systems fail to perform

RE: Well and Septic Adequacy Test for Lot 2, Block 7, Elmore Subdivision #2
Our inspection and test of the system located at the above location indicated that it is
functioning adequately to meet its intended use. However, it appears the system was
upgraded without M.O.A. notification. The old system utilized drainage tile that
stopped working in the 1980's. The owner then installed approximately 50 feet of 4
inch perforated pipe at a depth of 3.5 feet in place of a portion of the tile drain field.
From measurements taken from the cleanouts we estimate that it has approximately a
0.01 % slope. If this is the case, only the downhill portion of the drainfield is being
utilized, and premature failure of the system may occur. A monitoring tube was
installed prior to performing the adequacy test. During the installation, a soil sample
was taken at a depth of S feet. The soil appeared to be a poorly graded sand with
gravel. From inspection of the soil and logs from neighboring lots we estimate a soil
rating of 120 ft' per bedroom. Further inspection of data compiled from neighboring
lots shows a high ground water level in the area to be approximately 16 feet below
grade. No evidence of bedrock was encountered in our search through the known
data. Also a foundation cleanout is present inside of the foundation.
Although no permit was issued for this upgrade, it appears to have been functioning
well since the perforated pipe was installed. For passing the standards for a 3 -
bedroom house the system is safe, functional, and adequate for this 2 -bedroom
house. Therefore I recommend approval of the system.
