We are submitting an application for the installation of a second waste water disposal system for this
lot. The owner is in the process of constructing a garage with an "Mother In Law" apartment,
containing one bedroom. The existing wastewater disposal system is sized for a 6 -bedroom house,
and can not include this addition. This submittal consists of three (3) drawings showing the present
improvements on the lot and the adjoining properties, (sheet 1/3), the proposed improvements of the
lot, (sheet 2/3), and a schematic of the proposed system, (sheet 3/3). Soil logs and percolation tests
of applicable test holes are also enclosed. The septic system design is based on the following:
Groundwater on November 13, 2003 at 3.5 Ft.

This permit was issued for ffive bedrooms. On advice from Gus Andress, BioCycle Alaska, I am
submitting a new design, showing a 1000 gallon septic tank installed in front of the BioCycle Unit.
The septic tank is used for settlement and sludge accumulation, while all aeration and treatment
takes place in the BioCycle unit. This design has been used successfully in past for 5 and 6
bedroom units.

The Required Size of the Soil Absorption System Is:
DEPTH LENGTH GRAVEL DEPTH WIDTH
The length dimension is the length(in feet) of the trench or drainfield. The
depth of a trench or pit is the distance between the surface of the ground and
the bottom of the excavation(in feet). There is no set width for trenches.
The gravel depth is the minimum depth of gravel between the outfall pipe and
the bottom of the excavation(in feet).

Permit applicant has the responsibility to inform this department during the
installation inspections of any wells adjacent to this property and the number
of residences that the well will serve.

Backfilling of any system without final inspection.and approval by this departmet
will be subject to prosecution.
Minimum distance between a well and any on-site sewage disposal system is 100 fei
for a private well or 150 to 200 feet from a public well depending upon the type
of.public well. Minimum distance from a private well to a private sewer line
is 25 feet and to a community sewer line is 75 feet. Well logs are required
and must be returned to this department within 30 days of the well completion.
Other requirements may apply. Specifications and construction diagrams are
available to insure proper installation.

Report
During 1984, a number of septic systems have been installed and
inspected, prior to the establishment of the foundation(s). AECS has
contacted (or attempted to contact) the applicants concerning the
installation of foundation clean-out and pipe from the foundation to
the septic tank inlet. In many cases, construction will not resume
till in the spring; therefore, precluding any further inspections.
Since the end of the year is near, with the expiration of permits,
AECS is forwarding the inspection reports without a foundation
cleanouts to your office. We understand that this will not eliminate
this situation; nonetheless, the reports reflect the actual inspected
installation at this time. We recommend you accept these as is. The
Health Authority, site evaluation for these properties can confirm the
installation of the cleanout. Conditional approval, based on required
installation in the spring, may be required during the interim.
