LEGAL: ROCKHILL BLOCK 2, LOT 19
The property owner has requested we obtain a permit to upgrade the existing septic tank to an
Advantex tank system for the above referenced lot. We propose to install one 2500-gallon
AdvanTex tank with 2 AX20 pods and reconnect to the existing fields per the attached design
thus creating a 5BR septic system with primary and reserve fields.
The area in general has no groundwater issues. The lot and area are served by private water. The
design will not impact any of the neighboring properties. Please contact us if you have any
questions.

2. Maintenance. ReDairs and Alterations.
(Owner is required to read, understand and initial each section)
Throughout the tem ofthis Agreement, the Owner shall enter into a service agreement
with an AWWTS service and maintenance provider approved by the Municipality or the
manufacturer’s representative. The AWWTS sha11 be maintained in a satisfactory
COndition capable of peIforming as designed and producing treated septic e触uent in
accordance with the equlPment’s approval for operation in the Municipality
It shall be the responsibility ofthe Owner during the tem ofthis Agreement to pay for all
repair(S), maintenance, a句ustment(S), rePlacement costs, and inspection costs. This
includes an annual maintenance fee (typically $400 to $60O).
Owner agrees that only maintenance and repair persomel approved by the Municipality
Or the manufacturer’s representative will inspect and make any necessary maintenance,
repalrS Or Pemitted alterations to the system.
Owner acknowledges that regular maintenance of an AWWTS reduces the potential
failure ofthe system, Which could include sewage backup and costly repalrS Or drainfield
repl acement.

Owner acknowledges that the Municipality may request records of maintenance and
repairs from the manufacturer’s representative or maintenance provider.
Owner acknowledges that the fine for failing to maintain and repair an AWWTS may be
assessed in accordance with AMC 14.60 030.
Owner agrees to grant the Municipality reasonable access to test and inspect the
AWWTS. The Municipality wi= give at least 24-hour notice.
Owner agrees that any sale or transfer oftitle of the property will not occur without a new
Ce正ficate of On-Site Systems Approval.
Owner agrees血at the AWWTS insta11ation and maintenance requlrementS aS PrOVided
by the AWWTS vendor/installer and approved by the Municipality are the govemmg
guidelines for the construction, maintenance and repair ofthe Owner’s AWWTS.
Owner agrees to maintain remote monitoring of the AWWTS as required by the
AWWTS approval.
3. ±塑. The tem of皿s Agreement sha11 begin on the date ofapproval by the
Municipality to operate the installed system, Or uPOn tranSf訂oftitle, and shall ∞ntinue
While the AWWTS is operational or until title is transferred.
4. Nonwaiver. The failure ofthe Municipality at any time to enforce a provision ofthis
Agreement shall in no way constitute a waiver of血e provisions, nOr in any way a餓3Ct
the validity ofthe Agreement or any part hereof; or the right ofthe Municipality
thereafter to enforce every provision hereof
5. Amendment. This Agreement sha11 only be amended by authorized representatives of
the Owner and Municipality. Any attempt to amend this agreement by either an
unauthorized representative or unauthorized means shall be void.
6. Jurisdiction: Choice ofLaw. Any civil action arising from this Agreement shall be
brought in the Superior Court for the Third Judicial District of the State of Alaska at
Anchorage. The laws ofthe State ofAlaska sha11 govem the rights and obligations ofthe
parties under this Agreement.
7. Severabil迫y. Any provisions ofthis Agreement decreed invalid by a co血ofcompetent
jurisdiction shall not invalidate the rema量mng PrOVisions ofthe Agreement_

Reft Septic System Upgrade for Lot 19, Block 2, Rockhill Subdivision
To whom it may concern:
The proposed 4 bedroom house will be served by a private well and septic system. Two test
holes were excavated in the northwest quarter of the lot. The proposed septic system will be
designed around the 30 foot radius of test hole "B'. We are proposing that a new 1250 gallon
septic tank and a deep trench type drainfield be installed. Comments regarding the design are
summarized as follows:
1. SOILS: See the attached logs which show the soil classifications, groundwater monitoring,
and the percolation test results. It is our opinion that due to the overall appearance of the soils, an
application rate of 1.2 gallons/day/ft2 should be used.
2. TRENCII DESIGN:
a. Percolation Rate: < 1.0 minutes/inch
b. Allowable Application Rate: 1.2 gallons/day/ft2
c. Number ofBedrooms: 4
d. Design Flow: 600 gallons per day
e. Minimum Absorption Area: 500 ft2
f. Total Depth: 9.5 feet (max.)
g. Effective Depth: 6 feet
h. Width: 2.5 feet
i. Reduction Factor: N/A
j. Minimum Length: 42 feet
k. Effective absorption area = 504 ft2
3. SURFACE WATERS: There are no surface waters within 100 feet of the proposed
upgrade.

4. TOPOGRAPIIY: As can be seen on the attached topography site plan, the lot is generally
flat, there are no slope concerns. The trench is to be installed parallel to slope contours.
I am unaware of any adverse impacts this installation would have on adjacent wells or septic

If this is a well permit and you have drilled the well, a well log must be sent to this office
for documentation ofthe installation and to close the permit.
If this is an on-site wastewater system and a licensed Professional Engineer has inspected
the installation, the original as-built inspection report must be sent to this office for
review, approval and documentation. All inspection reports must be submitted within 30
days of construction completion·
A new permit must be obtained fi.om this office for a well and/or on-site wastewater
system NOT installed by the expiration date· However a new permit can be issued free of
charge for a second year if the application for the renewal is received on or before the
date of expiration of the original permit for which a fee was paid.
When applying for a new permit after the original permit has expired or for more than a
second year, the fees are: $320·00 for an on-site wastewater permit and $120.00 for a well
permit.

Ref.' Well Location and Septic System Design for Lot 19, Block 2, Rockhill Subdivision
To whom it may concern:
The proposed 4 bedroom house will be served by a private septic system and a private well. The
proposed septic system will consist of a 1250 gallon septic tank and shallow trench type
drainfield. Comments regarding the proposed design are summarized as follows:
1. SOILS: Attached are the soil logs which shows the soil profile, and the percolation test
results. In both test holes, the soils below the organic layers are a SP/SM & SM material to a
depth of 7.5 feet in TH#1 and 12.5 feet in TH#2 (bottom of TH#2). At 7.5 feet in TH#l, the
soils transition to primarily a SM material to a depth of 11.5 feet (bottom of TH#l). The soils at
the bottom of both test holes are a cemented silt with some angular rock which was visually
classified as impermeable. No groundwater was encountered during the excavation of the test
holes. In TH#I, a percolation test was performed between the depth orS.0 feet to 5.5 feet which
had a percolation rate of 20 minute/inch. In TH#2, a percolation test was performed between the
depth orS.0 feet to 5.5 feet which had a percolation rate of 9 minute/inch.
2. TRENCH DESIGN:
a. Percolation Rate: 9 & 20 minutes/inch
b. Allowable Application Rate: 0.6 gallons/day/112
c. NumberofBedrooms: 4
d. Design Flow: 600 gallons per day
e. Minimum Absorption Area: 1000 tt2
f. Total Depth: 5.5 feet (max.- on uphill side)
g. Effective Depth: 3 feet
h. Width: 5 feet minimum
i. Reduction Factor: 0.58
j. Minimum Length: 2 ~ 60 feet Ion~each (120 feet total length)
k. Effective absorption area = 1035 11'~
3. SURFACE WATERS: There are no surface waters within 100 feet of the proposed
upgrade.
4. TOPOGRAPIIY: As can be seen on the attached topography site plan, the lot is generally
flat in the area of the proposed septic system; in short, there are no slope concerns.
5. LOT LINE WAIVER: We request that your department issue a 5 foot lot line waiver from
the east property line to the proposed drainfields. We are unaware of any adverse effects this will
have on the neighboring property.
I am unaware of any adverse impacts this installation would have on adjacent wells or septic
