All construction shall be in accordance with:
1. The attached approved design.
2. All requirements specified in Anchorage Municipal code Chapters 15.55 and 15.65 and the State of Alaska
Wastewater Disposal Regulations (18AAC72)and Drinking Water Regulations (18AAC80)
3. The wastewater code requires inspections during the installation. The engineer shall notify the Development
Services Department per AMC 15.65. Provide notification by calling (907) 343-7904 (24/7).
4. From October 15 to April 15, a subsurface soil absorption system under construction during freezing weather
shall be either:
a. Opened and Closed on the same day, or
b. Covered, sealed, and heated to prevent freezing
Special Provisions: The entire subsurface disposal field and/or alternate is not within a 30 foot radius of a
percolation test. The Engineer needs to do an additional percolation test prior to the construction of the septic
field. Please submit stamped and signed results with the As-built Inspection Report. If the results require a
design change, construction of the system will stop pending On-Site review and approval. AMC 15.65.060.B.3

Subject: Rampart#1, Lot 3
Septic System Permit Request
Design Narrative
This is a design narrative for a permit to install an upgrade septic system to be issued for this property.The proposed
system will serve an existing four(4) bedroom house, but will be designed and installed for five (5) bedrooms to
accommodate future expansion. Currently the lot is developed. The system will utilize a new 1500g septic tank and a
conventional drain field. This lot and the surrounding lots are served by private wells. The surrounding wells are
over 100' from this system, and the well on this lot is 65' from this system. See waiver request.
1. Soils. One test hole was performed in the vicinity of this system by PES in September 2017, and groundwater was
monitored for at least seven days. Ground water was not observed at a depth of 15' below the surface in the test
hole monitor tube in September of 2017. Bedrock was not encountered in the test hole to 15.0'. Based on the
results of the percolation tests and overall soils appearance; an application rate of 1.2 gallons/day/square was
used for a conventional wastewater system in the area of the test hole. Due to site constraints the proposed
drainfield will be installed mostly outside of the 30' test hole radius. A new test hole will be excavated at time of
construction to verify the percolation rate of the accepting layer.
2. Soil Absorption System Design.
a. See Sheet 1 of the design package.
3. Surface Water:There is no surface water within 100 feet of the proposed septic tank and drain field.The
proposed drain field upgrade will maintain at least 100 feet from all surface water and drainage ditches.
4. Topography: The existing topography on the lot generally slopes from southeast to northwest, with a
slope of approximately 5%-7% in the area of the drain field. There are no steep slopes within 50' of the proposed
drain field. The proposed drain field will maintain 50' separation all steep slopes.
5. Drawing Markings:The Drawings are marked "For MOA Review Only". When written notification that the review
is complete and that there are no further comments is received from MoA On-Site Department, the note will be
removed and "Issued for Construction" drawings will be issued.
6. Waiver: We would like to request a waiver for separation distance for the septic tasystem to the well located on
the subject property. Due to the small lot size and the well location near the center of the lot it is not possible to
install a system outside the 100' well radius.
a. Tank and Drainfield (p)to Well (E):A separation distance waiver between existing well on Lot 3 to the
new septic tank and absorption field also on the same lot. The distance between the existing septic tank
and the well is approximately 75', the new tank is to well will be approximately 71', the new drain field
to well will be approximately 65', and the reserve drain field will be located approximately 60' to the
well. We are requesting a waiver for the septic system to the existing well for a distance of 60' on Lot 3.

i. DEC Criteria:The well on Lot 3 is a private well with no documentation. The nearest well just to
the east at Lot 2 has a depth of 55 feet, with a static level of 35 feet and is cased to 55 feet. Due
to the same terrain and elevation, this well is likely similar to the well on lot 3. The well data for
Lot 2 to the east is summarized as follows:

The ground water in this area typically flows from the southeast to the northwest.The soil cross-
sections are similar across the area and a homogenous cross-section was used to calculate the DEC
criteria for all components. As outlined under 18 AAC 80.020 and 18 AAC 72.021(a), I have calculated
the following points:

Soil sorption below sewage system 2.5
Soil permeability below the system 1.5
Water table gradient 0.0
Horizontal separation 1.4
Total Points 9.9
"The sum of the above values could equate to the following conditions: 8-12 Contamination is
unlikely from bacteria but could be possible from chemicals or virus." If the well were re-drilled over
100 feet from this system the point value would remain in the 8-12 range and provide very little
benefit. Additionally, the nitrates were non-detectable the last time they were tested. In my
opinion, this waiver request meets the above criteria for approval and does not constitute risk to
health. I hope the above information will assist you in determining that the waiver should be
granted.
The proposed installation will not affect the future development of this or the surrounding lots.

SPECIAL PROVISIONS TO SPECIFICATIONS
1. ALL CONSTRUCTION SHALL BE INSTALLED AS SPECIFIED IN THE MOST CURRENT EDITION OF THE MUNICIPALITY OF
ANCHORAGE STANDARD SPECIFICATIONS (MASS) FOR COMPONENT PARTS AND MATERIALS USED IN CONSTRUCTION OF
ON—SITE WASTEWATER DISPOSAL SYSTEMS AND AND IN ACCORDANCE WITH AMC 15.65 AND 15.55.
2. SCOPE OF WORK: INSTALL NEW SOIL ABSORPTION SYSTEM AND SEPTIC TANK.
3. GROUNDWATER WAS NOT ENCOUNTERED TO A DEPTH OF 15 FEET BELOW EXISTING GRADE AS EVIDENCED BY THE
SOIL TEST HOLE. IF AN APPARENT WATER TABLE IS OBSERVED IN ANY OF THE EXCAVATIONS LESS THAN 15 FEET
BELOW EXISTING GRADE NOTIFY THE ENGINEER IMMEDIATELY.
4. THE CONTRACTOR (BOTH WELL AND SEPTIC SYSTEM CONTRACTORS) HAD THE WELL LOCATION AND SEPTIC LOCATION
STAKED AND ANY LOT LINE AND WELL RADIUS SHOWN ON THE PLAN WITHIN 30 FEET OF THE PROPOSED SYSTEM
STAKED BY A REGISTERED LAND SURVEYOR BEFORE STARTING THE WORK.
5. THE CONTRACTOR IS RESPONSIBLE FOR ALL R.O.W. AND OTHER REQUIRED PERMITS, OTHER THAN THE ATTACHED.
6. THE CONTRACTOR SHALL CALL FOR LOCATING OF ALL BURIED UTILITIES.
7. THE CONTRACTOR SHALL PROVIDE 24 HOUR NOTICE TO THE ENGINEER PRIOR TO START OF WORK. ALL SURVEYING
AND LOCATES WERE IN PLACE PRIOR TO NOTIFYING THE ENGINEER.
8. THE CONTRACTOR SHALL NOTIFY THE ENGINEER OF ANY DISCREPANCY BETWEEN THE APPROVED DRAWINGS AND SITE

NOTE The operational life of all septic systems depends on the local soil conditions, groundwater
levels that may fluctuate during the year, and the water usage of the family being served by the
system. These conditions are outside the control of the evaluator of this septic system. We can
therefore not give any estimate of how long this system will function satisfactorily for current or
future occupants. All septic systems ultimately fail. Some systems last i 5-20 years, others fail after
less than five years.

Dear Mr. Shafer:
Your request for waiver of the required 100 foot separation of
a septic system to a private well has been approved. The
approved separation distance is 95 feet and existing seepage
pit to lot line of 1 foot.
This waiver approval applies to the existing septic system to
well separation only. Any future upgrade to either will
require all separation distances be met or another approval
from this department.

REFERENCE: Lot 3; Rampart Subdivision #1.
Request you issue the attached Health Authority Approval and grant
a waiver for the horizontal separation distance between the private
well and the seepage pit located on the referenced property at a distance
of 95 feet.
The seepage pit was installed in October, 1973 and inspected by a Municipal
inspector. The 95 foot separation was approved at that time. Approvals
were issued on the well and septic system in December, 1973; November,
1975; November, 1976 and December, 1982.
A risk analysis has been performed and it appears that no bacteriological
pollution is possible from this source. Attached for your review are
the following documents:
1. Coliform and nitrate analysis of water taken from the well
located on the reference property.
2. A flow test report for the referenced well.
3. A plot plan showing relative distances between the on site
wastewater disposal system and the well.
4. Risk analysis waiver review worksheet.
5. Letters of non-objection from the utility companies for the
portions of the seepage pit located within the utility easement.
It is our opinion that the horizontal separation distance prescribed
by 18AAC72.021 is not required in this case.

THIS AGREEMENT, made and entered into this 26th day of
July, 1988, by and between CHUGACH ELECTRIC ASSOCIATION, INC., an
Alaska non-profit electric cooperative membership corporation of
Anchorage, Alaska, hereinafter called 'First .Party', and
Federal Home Loan Mortgage Corporation of. Anchorage, Alaska,
hereinafter called 'Second Party', WITNESSETH THAT~ ...
WHEREAS, First Party occupies an easement on the North
side of Lot Three (3), Rampart Subdivision, Anchorage Recording
District, Alaska; and
WHEREAS, Second Party has constructed a septic clean
out and seepage pit which encroaches approxi~ately Ten feet (10')
onto the foregoing easement.
NOW, THEREFORE, for and in consideration of their
mutual agreements and other good and valuable consideration, the
receipt of which is hereby acknowledged, FirSt Party, for itself,
its successors and assigns, waives its objections to Second
Party's encroachmeht on the said easement for the purpose men-
tioned, subject to the following conditions=
1. Second Party will hold First Party harmless from
liability for any injury or damage to any' person or property
which may result from such encroachment.
2. The said encroachment will be without prejudice to
First PArty's full enjoyment of any and all rights it may have in
and to such eas.ement.
3. Any facilities or property .of First Party which
are damaged or destroyed as a result of such encroachment will be
repaired or replaced at no cost to First Party.
4. Second Party will comply with all applicable
safety codes and regulations. ~
5. Any expenses which First Party' may incur in the
relocation or modification of its facilities to accommodate the
said encroachment will be paid by Second Party.
By" its execution of this Agreement, Second Party
agrees, for itself, its successors, and assigns, that its en-
croachment upon the named easement as her~inaboVe mentioned will
be subject to the foregoing conditions.
IN WITNESS WHEREOF, the parties have caused this Agree-
ment to be executed by their duly authorized representatives and
agents as of the day and year first above mentioned.

The Anchorage Telephone Utility (ATU) has no objection to the
encroachment into a platted easement located on the north property line
of Lot 3 Rampart Subdivision #1 with a on-site septic system, as
depicted on the as-built drawing attached.
Acceptance and use of this letter of non-objection by yourself, your
heirs, your assigns, or your successors, will constitute agreement to
the following stipulations:
ATU will be held harmless, now and forever, for any damages
or injury to any person or property as a result of this
enchroachment.
Any ATU facility damaged or destroyed as a result of this
encroachment will be repaired at no cost to ATU.
Any costs incurred by ATU for special construction
necessitated by this encroachment will be borne by the
property owner.
Ail applicable safety code regulations will be observed and
maintained°
This letter of non-objection will in no way preclude ATU from
full use and enjoyment of its rights within any portion of
its right-of-way.
